Het toenemend aantal chronische patiënten leidt tot een toenemende vraag naar zelfmanagementondersteuning (ZMO). ZMO is een kerntaak van de verpleegkundige. In het onderzoeksprogramma NURSE-CC is geconstateerd dat (student)verpleegkundigen het bieden van ZMO moeilijk vinden en de juiste competenties veelal ontbreken. NURSE-CC 2.0 richt zich op herontwerpen en implementeren van instrumenten voor ZMO om een open communicatie met de patiënt op een gestructureerde wijze ondersteunen. De doelgroep bestaat uit chronische patiënten met diverse sociaal-culturele en sociaal-economische achtergronden. Het einddoel is om (student) verpleegkundigen te ontwikkelen tot bekwame professionals die patiënten de gewenste ZM-ondersteuning bieden.
Methoden
- Evaluatie van het testen, aanpassen en op kleine schaal implementeren van ZMO-instrumenten bij chronische patiënten.
- Effect meten van het gebruik van ZMO-instrumenten op a) verpleegkundige competenties, en b) systematische en transparante verpleegkundige ZMO-documentatie.

ZM-Doc (Zelfmanagement en Documentatie) / NURSE-CC 2.0 van Hogeschool Rotterdam was een vervolg op het onderzoeksprogramma NURSE-CC. De doelstellingen waren 1) het implementeren van een gesprekshulpmiddel voor zelfmanagementondersteuning in verschillende settings en de bruikbaarheid hiervan te evalueren en 2) het gebruik van het gesprekshulpmiddel te koppelen aan de verpleegkundige verslaglegging om zo de (inter)professionele communicatie te verbeteren.
In drie instellingen werden gesprekshulpmiddelen (Zelfmanagement Web en ZelfredzaamheidsRadar) geïmplementeerd. Verpleegkundigen rapporteren weinig over wat zij doen aan zelfmanagementondersteuning of over de voorkeuren van patiënten.
Het gebruiken van een hulpmiddel om met patiënten te bespreken wat zij belangrijk vinden en welke doelen zij willen stellen lijkt eenvoudiger in een poliklinische setting dan op een verblijfsafdeling geïmplementeerd te kunnen worden. Voorwaarden voor succesvolle implementatie lijken multidisciplinaire afspraken, een veilige cultuur waarin geëxperimenteerd kan worden en de aanwezigheid van verpleegkundigen die een bepaalde mate van verpleegkundig leiderschap tonen.
Samenvatting van de aanvraag
SUMMARY The rise in chronic diseases demands collaborative patient-centred care with facilitating patients’ self-management (SM) as key component. Providing Self-Management Support (SMS) is a core element of nursing care. The diversity in cultural and socio-economical backgrounds of chronic patients requires nurses to tailor SMS. However, research showed a gap between feeling competent and actual provision of SMS. This project focuses on adaptation and implementation of SMS-tools for open guided patient-nurse communication in a variety of settings. The aims are to evaluate: 1) the testing, adaptation, and small-scale implementation of SMS tools for guided and open patient-nurse communication about patient needs and/or collaborative goal setting in various care-settings with chronic patients with diverse cultural and socio-economical backgrounds 2) the effects of using SMS tools on (student)nurse SMS-competences 3) the effects of using SMS tools on systematic and transparent nursing SMS-documentation. The ultimate goal is to equip (student) nurses with adequate competences to provide tailored SMS, act as change agents, and practice evidence-based care This project reinforces and strengthens the regional nursing research infrastructure. RESEARCH QUESTIONS and METHODS The three research questions correspond with three different studies with different research methods. The research design is exploratory, practice-oriented and participative, using quantitative and qualitative methods. RQ I) What are the experiences in the different learning communities with testing, adaptation, and small-scale implementation of a tailored SMS tool for guided and open patient-nurse communication about patient needs and/or collaborative goal setting? METHOD. Predominantly qualitative including, interviews, gathering/analyzing self-reported data on the SMS-process, and team meetings, using the Adaptive Design principles. RQ II) How do nurse competences related to SMS change over time in the learning communities after the implementation of the selected SMS tool(s)? METHOD. A before/after measurement of the effect of using an SMS-tool on nursing SMS-competencies by the SEPSS-36 (Self-efficacy and performance) questionnaire. (student)nurses fill out the questionnaire before and after implementation of the SMS-tool. RQ III) How does documentation of SMS change after the implementation of the selected SMS tool(s)? METHOD. Before/after study, evaluating the effect of implementation of an SMS-tool on nursing documentation (SMS nursing process), using the D-Catch instrument. The D-Catch measures the quality and the quantity of five parts of nursing documentation: 1) nursing diagnoses, 2) planned outcomes of care, 3) interventions, 4) evaluations and 5) recording of the nursing care process. SETTINGS The project includes 4 different learning communities, representing different settings; hospital, home and rehabilitation care, where (student)nurses of different educational levels (level 4 = mbo-V and 5 = hbo-V), educators, researchers, and practitioners collaborate. INTERVENTION SMS-tools e.g. the Zelfredzaamheidsradar or the SMSWeb that support an open patient-nurse communication about patients’ needs, motivation and preferences are tested and implemented (small scale) within various care settings. The chosen SMS-tool will be evaluated on usability and adaptability by (student)nurses and patient representatives from that setting. PHASE 1 Orientation and selection of the intervention in the learning communities and exploring the current situation (M1-M8). This includes: a) literature study on SMS-tools and on nursing documentation of SMS, b) evaluation of nursing SMS-competencies and of documentation of the SMS nursing process c) selection and first adaptation of the SMS-tool(s) by a small team representing the local setting. Staff nurses and managers will be informed in a meeting. DELIVERABLES 1) Network meeting to present the results of the 1st phase 2) Publication in (international) nursing journal/Kennisplein Chronische Zorg PHASE 2: Small scale testing and adaptation of the SMS-tool (M9-M19) (Student)nurses receive a brief training on the use of the selected SMS-tool and provide feedback on daily use of the SMS-tool. In iterative steps the SMS-tool will be adapted to make it effective in that specific setting. DELIVERABLES 1) Network meeting to present the results of the 2nd phase 2) Publication in (international) nursing journal/Kennisplein Chronische Zorg 3) Training developed by Vilans and Van Kleef Institute PHASE 3: Second evaluation of nurse SMS-competences and documentation of the SMS nursing process (M19-M24) The research team will execute the 2nd measurement of SMS-nursing competences, student nurses will screen patient records on SMS-documentation.
