Consequences of polyfarmacy and Comorbidity on adherence of older depressed patients in general practice

Oudere mensen met een depressie lijden relatief vaak daarnaast nog aan andere chronische ziekten. Als gevolg daarvan slikken ze vaak veel medicijnen (polyfarmacie). De combinatie van polyfarmacie en depressie zou consequenties kunnen hebben voor de therapietrouw, zowel wat betreft lichamelijke chronische ziekten als depressie.
Doel en werkwijze
In onze studie onderzoeken we de kans dat depressieve ouderen meerdere ziektes en polyfarmacie hebben, en gaan we na welke ziekten samengaan met depressie. Vervolgens gaan we na welk risico er is dat combinaties van depressie en chronische ziekte een negatieve invloed hebben op de therapietrouw. Tenslotte gaan we na of huisartspraktijken onderling verschillen in de mate van polyfarmacie. We onderzoeken dit met bij huisartspraktijken uitgespoelde gegevens uit het EPD gekoppeld aan het apothekersdatabestand SFK. De resultaten worden ingebracht in de huisartsenopleiding opdat in het toekomstig curriculum beter aandacht besteed kan worden aan deze problemen bij oudere patiënten.
Resultaten
We onderzochten of ouderen met depressie meer chronische ziekten hebben, meer verschillende medicijnen chronisch gebruiken, waaraan ze minder therapietrouw zijn. Dit werd bevestigd en de associaties waren onafhankelijk van elkaar. Therapieontrouw gold voor medicatie voor chronische somatische ziekten, zoals hart- en vaatziekten, en voor antidepressiva.
Aanbevelingen
Oplossingen moeten gezocht worden in:
- minder medicatie (bijvoorbeeld cognitieve gedragstherapie in plaats van antidepressiva)
- leefstijlinterventies
- periodieke reviews van het medicatiegebruik door huisarts in overleg met apotheker en mogelijk de inzet van baxterrollen
Dit soort aanbevelingen zijn neergelegd in een voorlichtingsfolder voor huisartsen en meegedeeld aan huisartsen in opleiding tijdens terugkomdagen. Ook volgden publicaties in de wetenschappelijke en vakliteratuur, in een proefschrift, en tijdens een symposium voorafgaand aan de verdediging van dat proefschrift.

Ouderen met een depressie hebben meer chronische aandoeningen en gebruiken meer medicijnen dan ouderen die niet depressief zijn. Let daarom bij deze groep extra goed op welke medicijnen je voorschrijft.

This project aims to contribute to the quality of care for older people with a depression. Depression among older people is strongly associated with a) the presence of somatic comorbid conditions and, consequently, b) medication related problems due to poly-pharmacy, and especially c) medication adherence problems. Because of the complexity of therapy and the increased risk of non-adherence it is important to get a better understanding about the prevalence of comorbid chronic somatic conditions in depressed elderly patients, the poly-pharmacy in which such comorbidity results, and the adherence to antidepressants and to other medication as well as in the distribution of these phenomena among gender, age groups, urbanization and socio-economic status. This will provide clues for health care professionals to improve the quality of care for depressed older people in daily practice. The problem of multi-morbidity and poly-pharmacy among depressed elderly will only increase in the future. Multi-morbidity and poly-pharmacy as such have been identified as some of the greatest challenges of general practice the coming years. The resulting non-adherence has major cost implications as well. We propose a series of empirical studies, centered around a comprehensive database that will consist of morbidity and GP-care (especially prescription) data from NIVEL Primary Care Database (NZR), combined with pharmacy dispensing data from the Foundation of Pharmaceutical Statistics (SFK) and socio-demographic and income data from Statistics Netherlands (CBS). From this database, data on multi-morbidity, poly-pharmacy and adherence will be retrieved of at least 2000 depressed elderly, matched with patients with a psychological problem not being depression, and matched with controls without any psychological problem in the past year. These data will enable us to answer the following research questions: 1. What is the risk for depressed elderly on somatic multi-morbidity and poly-pharmacy as compared to non-depressed elderly? 2. What is the risk for depressed elderly on non-adherence to medication for somatic diseases as compared to non-depressed elderly? 3. Do multi-morbidity, poly-pharmacy and depression interact in their associations with anti-depressant and somatic medication adherence? 4. What is the impact of GP-practice variation on poly-pharmacy and adherence? The study will be carried out by a PhD student, who also is a vocational trainee in general practice, thus linking the general practice research department to the educational department. The advisory team around the study is composed in such a way that also the fields of pharmacy and elderly medicine are represented. Part of the study is a discussion about the results in focus groups of vocational trainees and practicing professionals (GPs, pharmacists, nursing home care) in order to arrive at practical recommendations for daily practice. Implementation activities are aimed at a dissemination of the results to vocational trainees in general medicine, nursing home care and pharmacy and to practicing GPs, pharmacists and nursing home professionals. This distribution will be done by publications in scientific and professional journals, by a symposium and by an information booklet. The study will be reported in the form of a thesis, that will consist of scientific papers on the four research questions, a literature review and a paper on the implementation activities, described above.
