From infancy to childhood: the intersection of gastrointestinal microbial communities, diet and health

Een goede ontwikkeling van het darmmicrobioom (alle micro-organismen in onze darmen) gedurende de eerste levensjaren is van groot belang voor onze gezondheid. In dit onderzoek werd de invloed van voeding op de ontwikkeling van het darmmicrobioom onderzocht in twee geboortecohorten in Nederland en Duitsland. In totaal werd de bacteriesamenstelling van meer dan 2,500 ontlastingsmonsters onderzocht. Een natuurlijke bevalling en het geven van borstvoeding bleken een gunstig effect op het darmmicrobioom van zuigelingen te hebben, maar ook het moment van introductie van vaste voeding en de diversiteit en samenstelling van de vaste voeding bleken bij te dragen aan de ontwikkeling van het darmmicrobioom. Verder bleek een hogere diversiteit aan bacteriën en de aanwezigheid van specifieke bacteriesoorten zoals Faecalibacterium prausnitizii de kans op allergieën te verkleinen. Vervolgonderzoek moet aantonen of we de diversiteit en deze specifieke bacteriesoorten positief kunnen beïnvloeden.

Een goede ontwikkeling van het darmmicrobioom, alle micro-organismen in onze darmen, gedurende de eerste levensjaren is van groot belang voor onze gezondheid. In dit onderzoek werd de invloed van voeding op de ontwikkeling van het darmmicrobioom onderzocht in twee geboortecohorten in Nederland en Duitsland. In totaal werd de bacteriesamenstelling van meer dan 2,500 ontlastingsmonsters onderzocht. Een natuurlijke bevalling en het geven van borstvoeding bleken een gunstig effect op het darmmicrobioom van zuigelingen te hebben, maar ook het moment van introductie van vaste voeding en de diversiteit en samenstelling van de vaste voeding bleken bij te dragen aan de ontwikkeling van het darmmicrobioom. Verder bleek een hogere diversiteit aan bacteriën en de aanwezigheid van specifieke bacteriesoorten zoals Faecalibacterium prausnitizii de kans op allergieën te verkleinen. Vervolgonderzoek moet aantonen of we de diversiteit en deze specifieke bacteriesoorten positief kunnen beïnvloeden.

De ontwikkeling van het microbioom gedurende het eerste levensjaar speelt een cruciale rol en is mogelijk geassocieerd met het optreden van ziekten later in het leven. De overgang van zuigelingenvoeding naar vaste voeding speelt een belangrijke rol bij de ontwikkeling van het darm microbioom, maar onduidelijk is in hoeverre de soort vaste voeding en het moment waarop deze vaste voeding voor het eerst wordt gegeven en borstvoedng wordt gestopt hierbij een rol speelt.
In dit project wordt in twee geboortecohorten met in totaal meer dan 400 kinderen onderzocht wat de invloed is van de introductie van vaste voeding en het stoppen van borstvoeding op de ontwikkeling van het microbioom en hoe dit vervolgens samenhangt met het risico op de ontwikkeling van allergieën en overgewicht.
Door op regelmatige basis ontlastingsmonsters te verzamelen gedurende het eerste anderhalf levensjaar wordt de ontwikkeling van het microbioom nauwkeurig in kaart gebracht. Herhaalde vragenlijsten en medische gegevens worden gebruikt om voeding en gezondheid te meten.
Binnen het PAPS cohort zijn de ontlastingsmonsters van ruim 300 kinderen op de leeftijd van 5, 13, 21 en 31 weken en op de leeftijd van 6 jaar verzamelt. Alle ontlastingsmonsters zijn inmiddels geanalyseerd op hun bacteriesamenstelling met behulp van 16S rRNA gen sequencing. Momenteel worden deze data geanalyseerd en wordt er gekeken hoe voeding samenhangt met de bacteriesamenstelling van de ontlastingsmonsters en hoe deze bacteriesamenstelling samenhant met de ontwikkeling van atopisch eczeem, allergische sensitisatie en astma.
Binnen het tweede cohort, LucKI Darmflora Studie, is de inclusie van deelnemers inmiddels ruim over de helft en wordt verwacht het beoogde aantal van 100 kinderen in het voorjaar van 2018 te bereiken.

Samenvatting van de aanvraag

Our consortium of experienced researchers in the area of infant and child health and the gastrointestinal (GI) microbial community brings together 4 cohorts from distinctly different biological, cultural and geographical areas to answer the question: What is the influence of the timing and nature of solid food introduction in infancy on colonization, succession and stability of GI microbial communities and consequent health outcomes, particularly fat accretion and allergic response? The first cohort is recruiting 240 Canadian infants born to low-risk pregnant women who have experienced minimal intervention during birth and have a high rate of breastfeeding. The second Canadian cohort will recruit 60 preterm infants. Both Canadian cohorts plan to follow participants until 3-years of age. The third cohort is composed of 606 German born infants followed until the age of 6-years, half of whom were randomly allocated to receive oral bacterial lysates in infancy. The fourth cohort is embedded in the Dutch Child andYouth Health Care system where 100 healthy term infants will be recruited and followed until the age of 1-year. All four of the included cohorts have collected or plan to collect repeated faecal samples as well as detailed information about perinatal exposures, dietary intake, medication and supplement use. Microbiologists from McMaster and Maastricht Universities will collaboratively undertake the analyses and interpretation of findings. In earlier work, consortia members (Lau et al) determined that reduced diversity in the early faecal microbiota of infants in the first week of life may be causally linked with atopic eczema appearing during the first 18-months of life. Early results from the Canadian team suggest that altered early microbiome may influence fat accretion by 6-months of life. We anticipate results of this work will contribute to understanding how differences in early life events (with a particular focus on dietary intake) influence GI microbial communities, the sustainability of that influence and associations of GI microbial community makeup with health outcomes, particularly relating to obesity and atopic disease. In addition, we will work to refine approaches to understanding the GI microbiome and its role on health outcomes through novel analytic approaches.
