The Brain - Adipose Tissue axis: new connections to stay in control

In dit onderzoek hebben we onderzocht hoe de hersenen metabolisme aansturen. Het hersengebied dat hiervoor verantwoordelijk is, is de hypothalamus. De hypothalamus bevindt zich helemaal onderaan uw brein, net boven het verhemelte. In het menselijk brein is de hypothalamus slechts een klein onderdeel, minder dan 1% van het totale volume. Om al deze processen in het lichaam aan te sturen gebruikt de hypothalamus verschillende wegen, zo kan het rechtstreeks ons (eet en drink) gedrag aansturen, maar via hormonen en het autonome zenuwstelsel kan het ook metabole processen in organen zoals de lever, spier, pancreas en vetweefsel moduleren. We hebben oa kunnen aantonen dat de hypothalamus via het autonome zenuwstelsel de productie en afgifte van vetdeeltjes in de lever zowel kan remmen als stimuleren. Het mooiste resultaat is dat we in een diermodel met vetzucht en hypertriglyceridemie hebben laten zien dat remming van de sympathische autonome activiteit de hypertriglyceridemie kan voorkomen.

Eindverslag

De incidentie van overgewicht neemt een epidemische omvang aan in de geindustrialiseerde landen. Helaas is dit niet alleen een kosmetisch, maar ook een belangrijk gezondheids probleem. Overgewicht is namelijk niet alleen nauw geassocieerd met een verhoogd risico op type 2 diabetes, maar ook met coronaire hartziektes, artrose, luchtweg aandoeningen en sommige vormen van kanker. De belangrijkste oorzaak voor deze diabetes/overgewicht epidemie is nog steeds niet geheel duidelijk, maar één van de hoofdverdachten is een onvoldoende controle van de energiebalans door de hersenen en dan met name een onderdeel van de hersenen genaamd de hypothalamus.

Het moge duidelijk zijn dat een onvoldoende verwerking van de overmaat aan vet in ons dieet hierbij een belangrijke rol spelt. Anatomen en fysiologen hebben het lichaamsvet eeuwenlang links laten liggen aangezien het werd beschouwd als alleen maar een opslag depot voor vet met verder weinig interessante eigenschappen. In de laatste jaren is echter duidelijk geworden dat het lichaamsvet niet alleen maar vrije vetzuren afgeeft, maar ook een actief endocrien en paracrien orgaan is dat een grote variëteit aan biologisch actieve stoffen afgeeft, de zogenaamde adipokines. Naast het vetweefsel is de lever een belangrijk orgaan als het gaat om de verwerking van de overmaat aan vet. Vrije vetzuren worden opgenomen uit de circulatie en kunnen dan verbrand worden of als voorloper dienen voor de synthese van lever triglyceriden. Echter, de opslag van vrije vetzuren op plaatsen die daar niet voor bestemd zijn kan resulteren in lever steatose en insuline ongevoeligheid. In het huidige project hebben we, met behulp van dierexperimenteel onderzoek, geprobeerd de hypothalame mechanismen te ontravelen die betrokken zijn bij de regulatie van het suiker en vet metabolisme.

Samenvatting van de aanvraag

The incidence of obesity is reaching epidemic proportions in industrialized countries. Unfortunately, this is not merely a cosmetic problem, but also a major health problem. In particular, obesity is associated not only with the development of type 2 diabetes, but also with coronary heart disease, osteoarthritis, respiratory complications, and certain forms of cancer. The main drive of this “diabesity” epidemic is still incompletely understood, but it is obvious that the inadequate handling of the dietary lipid overload plays an important role. White adipose tissue (WAT), i.e. body fat, has been ignored by anatomists and physicians for centuries, as it was considered to be an inert lipid storage depot with only few interesting attributes. In recent years it has become clear that WAT is not only secreting free fatty acids (FFAs), but that WAT is an active endocrine and paracrine organ that releases a large number of bioactive mediators also called adipokines. Adipokines such as leptin, adiponectin, interleukin-6 (IL-6), resistin, MCP-1, RBP4 and tumor necrosis factor-alpha (TNF-alpha), not only influence body weight homeostasis but also insulin resistance, diabetes, lipid levels, blood pressure, coagulation, fibrinolysis, inflammation and atherosclerosis. MRI and CT studies have shown that, specifically, abdominal fat accumulation is associated with health problems, independent of body mass index (BMI). In addition to WAT, the liver is an important organ for handling excess lipid levels. Free fatty acids are taken up from the plasma and can either be oxidized or serve as precursors of hepatic triglyceride (TG) synthesis. However, ectopic deposition of FFA may lead to liver steatosis and insulin resistance. Previous tracing studies in the NIN proved the existence of neural pathways between the brain and the liver, as well as between the brain and different WAT compartments. The tracing experiments also showed that separate sets of neurons in the brain and, more specifically, the hypothalamus, are in control of either subcutaneous or intra-abdominal WAT, thus providing a distinct neuroanatomical perspective for differential regulation of separate WAT depots. Pilot data from physiological follow-up experiments indicate the functional significance of these neural connections with respect to glucose metabolism both in the liver and WAT. The present proposal aims to study, in rats and mice, the importance of the brain control of lipid metabolism in WAT and liver. The three main hypotheses that will be tested in the present proposal are: 1) The hypothalamus regulates endocrine and metabolic activity in white adipose tissue through the autonomic nervous system; 2) The brain determines where the fat will accumulate; 3) An altered balance of the autonomic input to the liver may induce a fatty liver and hepatic insulin resistance.
