Advanced meta-analytical mthods for medical data

Wetenschappelijk bewijs van een medische behandeling wordt vaak gevonden op basis van meta-analyses. Een meta-analyse vat talloze individuele en onderling vergelijkbare onderzoeksprojecten systematisch samen tot algemeen geldende conclusies. Daaraan kleeft een aantal methodologische en statistische haken en ogen. Zijn onderzoeksprojecten bijvoorbeeld wel goed onderling vergelijkbaar, hoe gaan de diverse studies om met meetfouten, hoe zit het met de gevoeligheid van de gebruikte diagnostiek, hoe kunnen gegevens over overlevingsduur met elkaar worden vergeleken? Dit heeft geleid tot een aantal fundamentele verbeteringen in de methodiek voor meta-analysen. Ook tot betere statistische tests voor de enorme hoeveelheid gegevens die uit het genetisch onderzoek komen. Een deel van de nieuwe statistische technieken is ook bruikbaar op andere terreinen.

Samenvatting van de aanvraag

Meta-analysis is the systematic and quantitative review of the results of a set of individual medical studies all concerning the same or a closely related research question, intended to integrate their findings. Meta-analysis, by summarizing the evidence in the medical literature for the efficacy of treatments, plays a dominant role in evidence-based medicine. Statistical methods for the situation of one outcome variable per study are well established. However, in many practical cases the outcome to be analysed is of multivariate nature. For many of these situations appropriate methods are lacking or underdeveloped. The overall aim of this proposal is to develop innovative methods for a number of these situations. More specifically, we propose (1) To develop new methods for the application of meta-analysis results in clinical practice (2) To develop new methods for investigating the relationship between treatment effect and baseline risk in clinical trials meta-analysis (3) To develop new methods for ROC curve meta-analysis (4) To develop new methods for survival curve meta-analysis (5) To develop new methods for meta-analysis of genetic linkage studies
