Aspirine en/of Clopidogrel voor de optimale behandeling van patiënten met perifeer vaatlijden

Patiënten met pijnklachten bij het lopen, in rust of met wonden ten gevolge van slagadervernauwingen kunnen een dotterbehandeling ondergaan. Bij een dotterbehandeling worden vernauwde slagaders middels een ballon weer opengemaakt. Na een dotterbehandeling krijgen patiënten altijd een nabehandeling middels bloedverdunners. Het is vooralsnog onduidelijk welke bloedverdunner of combinatie het beste werkt. Dit leidt tot veel praktijkvariatie.

Onderzoek

In deze studie worden de bloedverdunners clopidogrel en aspirine onderzocht. Deze bloedverdunners worden al decennia lang voorgeschreven na een dotterbehandeling. Dit onderzoek heeft als doel de effectiviteit van al bestaande bloedverdunners of een combinatie daarvan te toetsen.

Verwachte resultaten

Uiteindelijk zal dit onderzoek worden verwerkt in de nationale en internationale richtlijnen maar boven alles tot het juiste gebruik van bloedverdunners bij deze patiëntengroep. Doelstelling: de juiste bloedverdunner (combinatie) voor de juiste patiënt.

Samenvatting van de aanvraag

Long-term antiplatelet therapy is the standard of care in patients with peripheral arterial disease (PAD), because it reduces atherothrombotic events. Despite this the atherothrombotic events remain high in PAD patients. In both coronary and cerebrovascular disease dual antiplatelet therapy has proven to reduce atherothrombotic events. However, these results cannot be directly applied to PAD patients and there is an evidence gap in antiplatelet therapy in PAD patients. The most important gap is the evidence in antiplatelet therapy after endovascular angioplasty. This is a group of patients with high events after treatment. Post-treatment optimization is therefore mandotory. Substantial part of this optimization is the post-treatment antiplatelet therapy. The most recent guidelines show discrepancies in the choice for dual or monotherapy after endovascular therapy. Related to this, different surveys among vascular surgeons show different use in antiplatelet prescription patterns. Current practice is based on the expert opinion of the physician treating the patient that induces the risk of incoherent choices. The main goal of the current study is to establish whether dual therapy with clopidogrel/aspirin will lead to a lower occurrence of atherothrombotic events in patients following endovascular intervention compared to clopidogrel alone. We will also evaluate the effect on quality of life and cost-effectiveness.
