The Effect of Appendectomy in Ulcerative Colitis patients with active disease: COlonic Salvage by Therapeutic Appendectomy (COSTA study)

In Nederland hebben ongeveer 50.000 mensen colitis ulcerosa. De meeste patiënten worden voornamelijk behandeld met medicatie. Toch ontkomt 20-30% van de patiënten niet aan een operatie waarbij de dikke darm moet worden verwijderd (colectomie). Uit eerdere onderzoeken blijkt dat de blindedarm een rol speelt bij colitis ulcerosa. Wanneer de blindedarm wordt weggehaald (appendectomie), kan dit mogelijk een milder beloop van de ziekte geven (minder klachten, minder medicijngebruik en minder ziekenhuisopnames) en eventueel zelfs het verwijderen van de dikke darm worden voorkomen. In dit onderzoek zal het effect van een appendectomie op het ziektebeloop van patiënten met actieve colitis ulcerosa ondanks behandeling met een biological (zoals bijvoorbeeld infliximab) worden onderzocht. Ook wordt gekeken naar het effect van een appendectomie op de kwaliteit van leven, waarbij de resultaten (en kosten) vergeleken zullen worden met een colectomie of behandeling met experimentele medicatie.

Samenvatting van de aanvraag

(in het Nederlands) DOEL/VRAAGSTELLING Het doel van deze studie is het vergelijken van de rol en de kosteneffectiviteit van een appendectomie met standaard zorg in patiënten met actieve colitis ulcerosa (CU). HYPOTHESE Een appendectomie vermindert de mucosale inflammatie en voorkomt het opschalen van medicatie en een eventuele colectomie. Daardoor verwachten wij dat een appendectomie een kosteneffectieve behandeling is. STUDIE OPZET Deze studie is een multicenter prospectieve cohort studie waarbij patiënten met actieve CU ondanks standaard step-up behandeling worden geïncludeerd. Patiënten zullen worden voorgelicht over een appendectomie en de reguliere behandeling (waarbij alle patiënten zullen worden gevolgd). STUDIEPOPULATIE/DATABRONNEN Patiënten die in aanmerking komen voor inclusie zijn CU patiënten (>17 jaar) met actieve ziekte ondanks standaard step-up behandeling inclusief (een) biological(s). INTERVENTIE De laparoscopische appendectomie zal worden verricht in dagbehandeling met gebruik van een laparoscopische endostapler waarbij de coecumbodem wordt meegenomen. GEBRUIKELIJKE ZORG /VERGELIJKING Tweedelijns (experimentele) medicatie (zoals biologicals of JAK remmers) of chirurgische resectie met een restauratieve proctocolectomie en ileoanale J-pouch anastomose. UITKOMSTMATEN Primaire uitkomstmaat: aantal patiënten in remissie binnen één jaar follow-up. Secundaire uitkomstmaten: kosten-effectiviteit, tijd tot remissie, endoscopische ziekteactiviteit, vermindering van medicamenteuze therapie, falen (opschalen medicatie of colectomie), kwaliteit van leven, patient self-reported assessment of symptomatology (MYMOP score) en histologische appendix karakteristieken voorspellend voor respons. SAMPLE SIZE BEREKENING/DATA-ANALYSE Berekening van de groepsgrote is gebaseerd op een toename van het aantal patiënten in remissie van 17% in de standaard zorg groep naar 40% in de appendectomie groep. Met een power van 80%, een 2-zijdige alpha van 0,05, en 10% verlies tijdens follow-up resulteert dit in 67 patiënten per groep. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Patiënten met actieve CU die standaard zorg behoeven kosten €12.314,- per jaar. Naar verwachting zal 53% van de patiënten minder medicatie gebruiken na appendectomie, wat zal leiden tot een kostenbesparing van €5.673,- per patiënt. Een KEA en BIA zal worden verricht met als primaire uitkomsten de kosten per patiënt in remissie en QALYs. TIJDPAD De studie periode is drie jaar en vier maanden (één maand voorbereidingen, twee jaar patiënten inclusie, één jaar follow-up, en drie maanden data-analyse en schrijven). (in het Engels) OBJECTIVE(S)/RESEARCH QUESTION(S) The objective of this study is to analyse the role and cost-effectiveness of appendectomy in patients with active ulcerative colitis (UC), compared to usual care. HYPOTHESIS Appendectomy decreases mucosal inflammation and prevents upscaling of medical treatment and even colectomy, resulting in a cost-effective treatment approach. STUDY DESIGN This study is a multicentre prospective cohort study including patients with active UC despite standard step-up treatment. Patients will be counselled for an appendectomy or usual care (all patients will be followed according to study protocol). STUDY POPULATION(S)/DATASETS Patients eligible for inclusion are UC patients (>17 years) with active disease despite standard step-up treatment including (a) biological(s). INTERVENTION A laparoscopic appendectomy will be performed in day care setting using a laparoscopic endostapler including the coecal base. USUAL CARE /COMPARISON Second-line (experimental) medication (i.e. biologicals or JAK inhibitors) or surgical treatment by a restorative proctocolectomy with ileal J-pouch anastomosis. OUTCOME MEASURES Primary outcome: remission rate within one year follow-up. Secondary outcomes: cost-effectiveness, time to remission, reduction of medical therapy, endoscopic disease activity, failure (upscaling medication or colectomy), quality of life, patient self-reported assessment of symptomatology (MYMOP score), and histological appendix characteristics predictive of response. SAMPLE SIZE CALCULATION/DATA ANALYSIS Group size calculations are based on an improvement of the remission rate from 17% in the usual care group to 40% in the appendectomy group. With a power of 80%, a 2-sided alpha of 0.05, and accounting for 10% loss to follow-up, a total of 67 patients is required in each group. COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS Patients with active UC requiring usual care cost €12.314,- per year. After an appendectomy 53% of these patients are expected to reduce medication use, which leads to a cost saving of €5.673,- per patient. A CEA and BIA will be performed with costs per UC patient in remission and QALYs as primary endpoints. TIME SCHEDULE The study period is three years and four months (one month preparations, two years patient inclusion, one year follow-up, three months data analysis and writing).
