Multimodal treatment of perianal fistulas in patients with Crohn's disease: a multicenter randomised controlled trial

De ziekte van Crohn is een chronische darmziekte die vooral jong volwassenen treft. Ongeveer 25% van de patiënten ontwikkelt hierbij een peri-anale fistel (abnormale verbinding tussen anale kanaal en peri-anale huid). Dit gaat gepaard met abcessen, pijn, en soms zelfs kringspierdestructie. De behandeling kan bestaan uit dure chronische medicatie (anti-TNF) en/of chirurgie (seton drainage of verschuivingsplastiek). In de PISA studie is gekeken wat de meest (kosten-)effectieve behandeling is voor patiënten met een Crohnse perianale fistel: setondrainage vs medicamenteuze behandeling met anti-TNF vs chirurgische sluiting. De hypothese was dat bij setondrainage het aantal reinterventies lager zou zijn, aangezien een seton voorkomt dat pus ophoopt in het fistelkanaal.
Na de behandeling van 91 patiënten bleken de resultaten volledig tegenovergesteld: het aantal reinterventies was het hoogst in de setongroep! Vanwege patiëntveiligheid is hierna de studie vervroegd gestopt. Bij nadere analyse bleek er wel een verschil te zijn tussen patiënten die hadden geloot voor setonbehandeling in vergelijking met de patiënten die hier zelf voor gekozen hadden (hierbij bleken de resultaten van setondrainage vergelijkbaar met de overige behandelstrategieën). Gezien de resultaten wordt een setonbehandeling voor Chttps://mysignins.microsoft.com/security-inforohnse fistels niet aangeraden, tenzij een patiënt hier zelf voor kiest. Een vervolgstudie zal moeten uitwijzen of anti-TNF of chirurgisch sluiten het meest effectief is voor fistelsluiting.

SAMENVATTING NEDERLANDS DOEL In de huidige praktijk is geen richtlijn voor de behandeling van perianale Crohnse fistels. De meeste patiënten krijgen anti-TNF (>25.000 euro pp/jaar) maar er zijn nauwelijks lange termijnresultaten van deze medicatie beschreven. Ook is de effectiviteit niet vergeleken met chirurgische behandelopties. Middels deze studie hopen wij tot een consensus voor de dagelijkse praktijk te komen met een besparing in medische kosten. HYPOTHESE Wij veronderstellen dat na 18 maanden geen verschil gevonden wordt in het aantal patiënten met een producerende fistel na anti-TNF of chirurgische behandeling (seton of advancementplastiek). Aangezien seton drainage een eenvoudige en goedkope behandeling is waarmee recidief of abcesvorming kan worden voorkomen, verwachten wij dat deze behandeling zeer kosteneffectief is. STUDIE OPZET Multicenter gerandomiseerde trial STUDIEPOPULATIE/DATABRONNEN Alle Crohn patiënten met een nieuw gediagnosticeerde hoge fistel (>2/3 externe kringspier) met enkele interne opening komen in aanmerking voor de studie. INTERVENTIE Groep I: chronische seton (seton drainage voor 1 jaar) Groep II: anti-TNF (seton drainage voor 4 weken met anti-TNF, gevolgd door verwijderen seton waarbij medicatie wordt gecontinueerd voor 1 jaar) Groep III: advancementplastiek (seton drainage met anti-TNF. Na 4-6 weken advancementplastiek onder anti-TNF scherm voor 3 maanden) UITKOMSTMATEN De primaire uitkomstparameter is het aantal patiënten dat fistel-gerelateerde interventie(s) nodig heeft. Secundaire uitkomsten zijn het aantal patiënten met gesloten fistels (MRI-score) na 18 maanden, de Perianal Disease Activity Index, kwaliteit van leven en kosten. SAMPLE SIZE BEREKENING In de literatuur worden reïnterventies beschreven tot 50% met anti-TNF medicatie en na advancementplastiek. Voor deze studie wordt een reductie in interventies met 30% klinisch relevant en haalbaar geacht. Om dit te verschil te detecteren is een sample size nodig van 42 patiënten per groep (alpha 0.05, power 80%, en 5% uitval). KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Een KEA en BIA van de drie groepen zal worden verricht over 18 maanden. De primaire uitkomsten zijn de kosten per patiënt voorkomen reïnterventie(s) en QALYs. Een voorlopige BIA laat zien dat als anti-TNF de lange termijnuitkomsten niet verbetert, 2,5 tot 6,5 miljoen euro per jaar kan worden bespaard. TIJDPAD De studieduur is 4 jaar (2 maanden voorbereiding, 2 jaar patiënteninclusie, 1,5 jaar follow-up, en 4 maanden data-analyse). ABSTRACT ENGLISH OBJECTIVE Currently, there is no guideline for the treatment of perianal Crohn's fistulas. Most patients receive anti-TNF, but no long-term results of this medication (>25,000 Euros pp/year) have been described, nor has its efficiency been compared to surgical strategies. With this study, we hope to provide treatment consensus for daily practice with reduction in costs. HYPOTHESIS It is hypothesised that after 18 months no differences will be found in the number of patients with draining fistulas for medical and surgical treatment strategies. Since surgical seton drainage is the only treatment preventing recurrences or abscesses, we expect this strategy to be highly cost-effective. STUDY DESIGN Multicenter randomised controlled trial POPULATION All Crohn’s patients with newly diagnosed high tract fistula (>2/3 of external sphincter), will be eligible. INTERVENTION Group I: chronic seton (seton drainage for 1 year) Group II: anti-TNF (seton for 4 weeks under anti-TNF, followed by seton removal with continuation of medication for 1 year) Group III: advancement plasty (seton drainage under anti-TNF. After 4-6 weeks, surgical closure with advancement plasty under anti-TNF for 3 months) OUTCOME MEASURES The primary outcome parameter is the number patients needing fistula-related re-intervention(s). Secondary outcomes are the number of patients with closed fistulas (based on MRI) after 18 months, Perianal Disease Activity Index, quality of life, and costs. SAMPLE SIZE CALCULATION/DATA ANALYSIS Based on the literature with re-interventions in 50% of patients on anti-TNF and after advancement plasty, a reduction with 30% of patients needing a re-intervention in the seton group is considered clinically relevant and feasible. The sample size needed to detect this difference equals 42 patients per group (alpha 0.05, power 80%, and 5% drop-out). COST-EFFECTIVENESS ANALYSIS/BUDGET IMPACT ANALYSIS The analyses of the three groups will be performed with a time horizon of 18 months. The primary outcomes are costs per patient with re-interventions prevented and the costs per QALY. A provisional budget impact analysis reveals that if anti-TNF does not improve long-term outcome, 2.5 to 6.5 million Euros per year can be saved. TIME SCHEDULE The study period is 4 years (2 months preparations, 2 years patient inclusion, 1.5 years follow-up, and 4 months data analysis).
