Beslissen met kwaliteitsindicatoren

Coördinatie en management van zorg voor kwetsbare ouderen is niet optimaal. Er ontbreekt een proactieve benadering voor het verbeteren van de ouderenzorg. Bovendien hebben ouderen geen handvatten waarmee ze de ontvangen zorg kunnen beoordelen.

Doel
Het project zet computerprogramma’s in voor het toepassen van indicatoren voor kwaliteit van zorg. Doel hiervan is het gedrag van zorgprofessionals op tijd te verbeteren door feedback te geven, bijvoorbeeld herinneringen of alerts. Ook ouderen en hun familie krijgen inzicht in de indicatoren.

Werkwijze
De onderzoekers vertalen internationaal erkende kwaliteitsindicatoren voor zorg naar de Nederlandse situatie. En als dat nodig is zoeken ze naar nieuwe kennis. Hiermee ontwikkelen ze systemen die professionals helpen bij beslissingen in de zorg. De werking van de systemen wordt geëvalueerd. Bij positieve evaluatie maken de onderzoekers het systeem toegankelijk voor andere netwerken.

Eindverslag

Probleem: In Nederland zijn er miljoenen contacten tussen dokters en patiënten per jaar. Dat betekent dat er een enorm aantal beslissingen worden genomen, soms ook de verkeerde. De kans op verkeerde of suboptimale beslissingen is vooral groot bij ouderen omdat hun zorg complex is.

De kwaliteit van de ouderenzorg kan gemeten worden aan de hand van kwaliteitsindicatoren. In de VS zijn in 2000 de ACOVE kwaliteitsindicatoren ontwikkeld. Een ACOVE indicator wordt uitgedrukt als een ALS-DAN regel waarbij een voorwaarde (ALS gedeelte) wordt gekoppeld aan een actie (DAN gedeelte). Momenteel wordt de kwaliteit van zorg volgens deze regels slechts incidenteel gemeten en alleen nadat de zorg was al lang verleend. Er ontbreekt een proactieve benadering voor het verbeteren van de kwaliteit van de ouderenzorg.

Oplossing: In het ICOVE project ontwikkelden we een systeem dat huisartsen tijdens een consult alerts stuurt met tips en adviezen die gebaseerd zijn op een geselecteerde verzameling van de ACOVE kwaliteitsindicatoren. Het idee is dat dit systeem het gedrag van zorgprofessionals tijdig kan verbeteren. Informatie over deze indicatoren wordt ook toegankelijk gemaakt voor de oudere cliënten en hun families. Gelijksoortige systemen hebben we ook ontwikkeld in twee ziekenhuizen.

Aanpak: Middels enquêtes en focusgroepen werden de toekomstige gebruikers van het systeem (de artsen) gevraagd naar de kwaliteitsindicatoren waarvoor ze behoefte hebben aan ondersteuning. De indicatoren werden via een stapsgewijze methode vertaald in een formaat die geschikt is voor computer-gebaseerde beslissingsondersteuning. We hebben drie beslissingsondersteunende systemen ontwikkeld, geïmplementeerd, en geëvalueerd in drie verschillende settings: bij ongeveer 40 huisartsen in Amsterdam Zuid Oost met een populatie van ruim 46000 patiënten inclusief 5500 ouderen; bij poliklinieken van de Interne Geneeskunde van het AMC, en bij de afdeling Interne Geneeskunde van het Spaarne ziekenhuis in Hoofddorp. Naast deze systemen die de zorgprofessional ondersteunen is er ook een website ontwikkeld om de kwaliteitsindicatoren inzichtelijk te maken voor ouderen, cliënten en hun mantelzorgers en advies geeft omtrent medicatiegebruik. Een andere tak van het ICOVE project had ten doen het genereren van nieuwe relevante kennis middels het schrijven van (Cochrane) reviews van de literatuur.

De belangrijkste conclusie van dit project is dat computer-gebaseerde beslissingsondersteuning het gedrag van artsen in de ouderenzorg positief kunnen beïnvloeden. We hebben state-of-the-art technieken en methoden geïmplementeerd in dit project om de slagingskansen van de beslissingsondersteunende systemen te vergroten maar acceptatie van deze systemen is niet vanzelfsprekend en de implementatie van beslissingsondersteunende systemen blijft complex.

Probleem: In de huidige organisatie van de gezondheidszorg hebben kwetsbare, oudere patiënten met complexe problematiek vaak te maken met suboptimale coördinatie en management van zorg. Hierbij wordt de kwaliteit van zorg slechts incidenteel en alleen evaluatief gemeten. Er ontbreekt een proactieve benadering voor het verbeteren van de kwaliteit van de ouderenzorg en ouderen hebben geen handvatten waarop ze de aan hen verleende zorg kunnen beoordelen.

Oplossing: Het gebruik van indicatoren voor kwaliteit van zorg door computerprogramma’s om het gedrag van zorgprofessionals tijdig te verbeteren door proactieve feedback aan hen te geven (zoals alerts en herinneringen). Een indicator wordt uitgedrukt als een ALS-DAN-WANT relatie waarbij een voorwaarde (ALS gedeelte) wordt gekoppeld aan een actie (DAN gedeelte) met een motivatie of bewijsvoering voor de indicator (WANT gedeelte). Informatie over deze indicatoren wordt ook toegankelijk gemaakt voor de oudere cliënten en hun families.

Doelgroep: (Kwetsbare) ouderen met complexe problematiek en hun zorgverleners in de 1e en 2de lijn in de regio Amsterdam Zuid Oost.

Aanpak:
− Vertaling van internationaal erkende kwaliteitsindicatoren voor zorg voor kwetsbare ouderen naar de Nederlandse situatie.
− Genereren van nieuwe kennis waar nodig middels (Cochrane) reviews van literatuur.
− Ontwikkelen van op kwaliteitsindicatoren gebaseerde beslissingsondersteunende systemen die professionals ondersteunen bij beslissingen in de zorg voor (kwetsbare) ouderen.
− Evalueren in hoeverre het werken met beslissingsondersteuning het gedrag van artsen verbetert.
− Inzichtelijk maken van kwaliteitssysteem voor ouderen, cliënten en hun mantelzorgers.

Samenvatting van de aanvraag

Background: Within the present healthcare organisation, vulnerable elderly patients with complex problems often receive sub-optimal care coordination and management. The complexity of their problems prompts fragmented and uncoordinated delivery of care eventually leading to deterioration of quality of life. Vulnerable elderly people are in need of an effective, continuous, integrated and comprehensive package of preventive and curative health care. A necessary condition for this is a continuous measuring and improvements of quality of care. A promising comprehensive quality-of-care (QoC) assessment system is provided by ACOVE (Assessing Care Of the Vulnerable Elder) with 392 process quality criteria developed and judged by clinical experts to improve patient outcomes on the basis of clinical evidence and professional opinion. A quality indicator (QI) is expressed as an “IF-THEN” statement, linking a condition to a conclusion that should be met, and explains the importance of the QI. The QIs cover the Prevention, Diagnosis, Treatment and Follow-up stages of care and pertain to various. ACOVE is emerging as an international standard for QoC: there are increasingly more studies translating the QIs to the setting at hand. In the Netherlands we are aware of the following substantial translation efforts: The Trimbos Institute translated 108 QIs on general practice (GP) care for vulnerable elders, covering eight conditions. The Academic Medical Centre at the University of Amsterdam is completing the translation of QIs pertaining to pharmacological care for hospitalized vulnerable patients, which resulted in 76 QIs. Problem: QIs are merely used for assessment. Currently there is no systematic way for pro-actively using the QIs to improve care for the vulnerable older patients. A second problem is that there are lacunas in the knowledge about care of the vulnerable elderly patient. A third problem is that ACOVE in its current intended use is not accessible to the older clients and their families, and they remain at loss when they have questions about whether they are getting the care they are entitled to. Similarly there is no automated educational resource on ACOVE for caregivers. Objective and approach: The primary objective is the systematic improvement of primary and secondary care and the handover of care from hospital to GP for older persons by increasing adherence to the merged ACOVE-based QIs (from Trimbos and the AMC), and in turn patient outcomes such as functional status or prevention from adverse drug events. To cover hospital as well as GP care we focus on acutely admitted older hospital patients (at AMC) who are at risk for functional decline after identification by a risk assessment instrument (The ISAR, Identification of Seniors At Risk) and then collect their GP data retrospectively (three months) before and prospectively (three months) after discharge. To attain improvement we intend to use clinical decision support systems (CDSSs) that use QIs in combination with patient and treatment data to pro-actively support health care professionals to make the right decisions at the right time. We will implement this intervention in Internal Medicine wards at the AMC, at general practices of the GAZO centra, and at Amsterdam thuiszorg (homecare). Consider for example launching an alert informing the GP or practical assistant at the time of medication prescription that the system is unaware of any indications for the prescribed drug or reminding a physician of performing renal function assessment after 3 months of performing a diagnostic test. In addition, we aim to write Cochrane systematic reviews where gaps in knowledge were identified. This knowledge can be used to enhance the CDSS. The secondary objective entails empowering the older clients and especially their (informal) caregivers by providing them with a web-based resource giving them useful, concrete, and easy to understand information about the aspects of quality of their care coordination and management. Research design: A pre-post controlled trial. Research population: 448 patients (224 in each group) 65 years or older, with positive ISAR score (cut-off at 2), with an acute hospitalization for more than 24 hours at an internal ward. Intervention: A clinical decision support system reminding and alerting caregivers about quality indicators. Primary outcomes: adherence to QIs in the period of three months before hospital admission till three months after hospital discharge as quantified by the average adherence quality score (QS) per patient, and two adjustments (for frequency and saliency) of this score. Secondary outcomes: (1) Functional status as measured by Katz’s ADL (Activities of Daily Living) index score (2) Perceived quality of life as measured by the EuroQol and the Minimal Data Set (3) Number of hospital readmissions within 3 months after discharge.
