Capturing changes in cognition: developing a measure for progression in dementia

Doel

Het doel van dit project was om het verloop van dementie beter in kaart te brengen. 

Resultaten

We hebben een nieuw, kort meetinstrument samengesteld gericht op zowel het denkvermogen als het dagelijks functioneren: de Cognitive-Functional Composite (CFC). We vonden dat de CFC het geheugen, het probleemoplossend vermogen en het dagelijks functioneren betrouwbaar meet. Daarnaast bleek dat de CFC deze functies nauwkeuriger en sneller meet dan veelgebruikte bestaande tests. Ook kon de CFC veranderingen over tijd detecteren, en daarmee is het een geschikte maat om het verloop van dementie in kaart te brengen. De CFC kan worden ingezet om gericht meer inzicht te krijgen in de progressie van dementie. 

De resultaten van dit onderzoek dragen daarmee bij aan betere zorg voor mensen met dementie, betere communicatie tussen zorgprofessionals en het effectiever evalueren van onderzoek naar nieuwe behandelingen.

Eindverslag

Het neuropsychologisch onderzoek bestaat uit tests waarmee cognitieve functies, zoals het geheugen, aandacht en planning worden gemeten. Met de resultaten van dit onderzoek kunnen we vaststellen hoe snel dementie zich bij iemand ontwikkelt, en of een behandeling het gewenste effect heeft. Deze resultaten zijn daarmee van groot belang voor mensen met dementie, hun naasten en de zorgverleners. In dit project hebben wij een nieuwe, samengestelde maat ontwikkeld die veranderingen in de cognitieve functies op een betrouwbare en valide manier meet. Deze maat bestaat uit cognitieve tests en een maat voor het alledaags functioneren, en is ontwikkeld in samenspraak met mensen met dementie, hun mantelzorgers, neuropsychologen en andere zorgprofessionals. Onze eerste studies laten zien dat het nieuwe meetinstrument betere testeigenschappen heeft dan de bestaande, en dat het als minder belastend wordt ervaren dan de bestaande testen door mensen met dementie. Momenteel onderzoeken we in meerdere geheugenpoliklinieken in Nederland en in de UK of de maat gevoelig is voor verandering. Het resultaat van dit onderzoek levert een kort en relevant meetinstrument op, dat een belangrijke bijdrage levert aan het verminderen van de belasting van het neuropsychologisch onderzoek. Daarnaast zal hierdoor de communicatie tussen arts, patiënt, mantelzorger en andere zorgprofessionals verbeteren, wat uiteindelijk zal leiden tot betere zorg voor mensen met dementie. We werken actief aan het betrekken van zorgprofessionals, zodat de resultaten geimplementeerd zullen worden in de praktijk. Het project verloopt volgens plan.

Samenvatting van de aanvraag

Neuropsychological testing is burdensome to patients with dementia. However, it is necessary for establishing diagnoses and essential for the monitoring of disease progression. In clinical practice as well as in research, cognition is considered the key change to be observed in people with dementia. The latter is of particular relevance in the evaluation of treatment effects. However, deciding whether a treatment is effective or not, is currently hampered by the fact that commonly used neuropsychological tests have quality limitations, were not designed to monitor disease progression and do not translate to everyday functioning. As a result, patients undergo extensive and inadequate neuropsychological testing. To solve this problem, we hypothesize (I) that we can combine neuropsychological tests with an everyday functioning questionnaire into one composite measure. We aim to identify relevant and sensitive measures and create this novel composite measure. Next, we hypothesize that we can capture clinically relevant changes with this composite measure. We therefore aim to perform a longitudinal validation study. To test hypothesis (I), we will be the first to develop a framework for a cognitive-functional composite measure. We will compose this framework in our workgroup and with input from patients and caregivers. To test hypothesis (II), we will investigate the psychometric properties of the composite. We will focus on the longitudinal validity, which is an essential aspect of the measurement of disease progression. Following this, we will test the composite in patients with Alzheimer’s disease in memory clinics in the Netherlands and the United Kingdom (n=300). We will evaluate changes after 3, 6 and 12 months and perform a construct validation. Using linear mixed modeling, we will compare changes on the cognitive-functional composite to changes in global cognition, caregiver burden, global cortical atrophy and quality of life. We expect changes on the composite to relate to these other measures of disease progression. Finally, we will also address the cultural, educational, gender and age differences and provide norms if necessary. This study will result in a substantial improvement of longitudinal cognitive measurements, which is an essential part of evaluating treatment effects and monitoring disease progression. A short and clinically relevant composite will reduce patient burden, advance evaluation of treatments, improve communication across health care providers and ultimately lead to a higher quality of care for patients with dementia.
