Treatment effects and cost-effectiveness of Autonomy Enhancing Treatment: A person-centred approach to anxiety disorders.

Angststoornissen beperken mensen in hun dagelijkse leven. Ze komen vaak voor en gaan vaak samen met andere stoornissen. Cognitieve gedragstherapie (CGT) richt zich op directe angstconfrontatie. Dit werkt goed, maar bij 1/3 van de patiënten niet en 1/5 stopt voortijdig. Autonomie versterkende behandeling (AET) richt zich niet op angst, maar op iemands autonomie: zelfsturend vermogen in relaties met anderen. Dat verlicht mogelijk ook andere klachten en AET is geschikt voor een gevarieerde populatie.

Wij vergeleken de (kosten-)effectiviteit van AET en CGT. 129 patiënten met een angststoornis volgden CGT of AET in groepsverband. Beide therapievormen bleken even goed te werken, qua angst, autonomie, zelfwaardering, algemene psychopathologie, depressie, kwaliteit van leven. Qua zorggebruik en productiviteitsverlies deed AET het iets beter dan CGT. AET is dus een goed alternatief voor CGT, bijv. als men een meer persoonsgerichte behandeling en/of geen exposure wil.

Samenvatting van de aanvraag

DOEL/VRAAGSTELLING Het doel is om Autonomy Enhancing Treatment (AET), Cognitieve Gedragstherapie (CGT) en een wachtlijst controleconditie te vergelijken in termen van angststoornis-specifieke en persoonsgerichte uitkomstmaten en kosteneffectiviteit: Zijn AET en CGT bij angststoornissen superieur aan de wachtlijst controleconditie qua angststoornis-specifieke uitkomstmaten? Is AET bij angststoornissen superieur aan CGT en een wachtlijst controleconditie qua persoonsgerichte uitkomstmaten, comorbide symptomen en kosteneffectiviteit? HYPOTHESE (i) AET en CBT zijn superieur aan de wachtlijst controleconditie wat betreft angststoornis-specifieke uitkomstmaten, (ii) AET is superieur aan CGT en de wachtlijst controle conditie wat betreft persoonsgerichte uitkomstmaten, comorbide symptomen en (iii) kosteneffectiviteit STUDIE OPZET Multicenter RCT bij een groep patiënten met angststoornissen met 3 condities: 15 sessies groeps-AET, 15 sessies groeps-CGT en een wachtlijst controleconditie STUDIEPOPULATIE/DATABRONNEN Patiënten met een DSM-IV-TR angststoornis INTERVENTIE AET is een gender-sensitieve, innovatieve, persoonsgerichte groepsbehandeling die zich in de eerste plaats richt op het versterken van autonomie-gehechtheid, en niet primair op de angststoornissymptomen GEBRUIKELIJKE ZORG/VERGELIJKING Groeps-AET, groeps-CGT (gebruikelijke zorg) en een wachtlijst controleconditie UITKOMSTMATEN Symptoomgericht: angstklachten. Persoonsgericht en comorbide symptomen: algemene psychopathologie, depressieve symptomen, autonomie-gehechtheid, zelfvertrouwen, kwaliteit van leven. Kosteneffectiviteit: QALYs, directe en indirecte kosten (zorggebruik/kosten, productiviteitsverlies) SAMPLE SIZE BEREKENING/DATA-ANALYSE N = 153 (51 per groep) op basis van een medium effect size en een power van .8 KOSTENEFFECTIVITEITSANALYSE/BUDGET IMPACT ANALYSE De KEA, gedaan vanuit het maatschappelijke perspectief, omvat kosten voor zorg, voor de patiënt en productieverliezen. De BIA wordt gebaseerd op de kosten gemoeid met de opzet en uitvoering van AET en het aantal angststoornispatiënten in Nederland die AET beoogt te bereiken. TIJDPAD Start: 10/2016. Voorbereidingen: eerste 6 maanden. Inclusie, interventie, follow-up: volgende 24 maanden. Analyse/schrijven: laatste 6 maanden. Start implementatie: vanaf jaar 1. Totaal benodigde tijd: 36 maanden. OBJECTIVE(S)/RESEARCH QUESTION(S) The objective is to compare Autonomy Enhancing Treatment (AET), Cognitive Behavioral Therapy (CBT) and a waitlist condition regarding disorder-specific and person-centred outcomes as well as cost-effectiveness: Are both AET and CBT for anxiety disorders superior to the waitlist condition with regard to disorder-specific outcomes? Is AET for anxiety disorders superior to CBT and a waitlist control condition with regard to person-centred outcomes, comorbid symptoms, and cost-effectiveness? HYPOTHESIS (i) AET and CBT are superior to a waitlist control condition regarding disorder-specific outcomes, (ii) AET is superior to CBT and a waitlist control condition with regard to person-centred outcomes, comorbid symptoms, and (iii) cost-effectiveness STUDY DESIGN Multicenter RCT in a group of anxiety-disordered patients comparing 3 conditions: 15 session group-AET, 15 session group-CBT and a waitlist-control condition STUDY POPULATION(S)/DATASETS DSM-IV-TR anxiety disordered patients INTERVENTION AET is a gender-sensitive, innovative, person-centred therapy with autonomy-connectedness deficits, instead of anxiety disorder symptoms, as its main focus USUAL CARE/COMPARISON Group AET versus group CBT (usual care) versus a waitlist control condition OUTCOME MEASURES Symptom specific: anxiety symptoms. Person-centred and comorbid symptoms: general psychopathology, depressive symptoms, autonomy-connectedness, self-esteem, quality of life Cost-effectiveness: QALYs, direct and indirect costs (health care use and work absence/productivity loss) SAMPLE SIZE CALCULATION/DATA ANALYSIS N=153 (51 per group) on the basis of a medium effect size and a power of .8 COST-EFFECTIVENESS ANALYSIS/BUDGET IMPACT ANALYSIS The CEA that will be undertaken from a societal perspective includes costs associated with health care, costs associated with the patient, and productivity costs. The BIA will be based on costs associated with adopting and implementing AET, and the number of anxiety disordered patients in the Netherlands that AET aims to target. TIME SCHEDULE Start: 10/2016. Preparations: first 6 months. Inclusion, intervention, follow-up: next 24 months. Analysis/writing: last 6 months. Implementation: starting from year 1. Total time needed: 36 months.
