PANDORA: Computer-aided Decision Support for the Management of Comorbidity in Patients with Chronic Diseases

Beslissingsondersteuning bij multi- en co-morbiditeit
Chronische ziekten komen steeds meer voor, o.a. door toename van het aantal ouderen. Ongeveer tweederde van de patiënten ouder dan 65 jaar heeft minstens twee chronische ziekten tegelijk. Dat maakt het lastig patiënten effectief te behandelen. Het probleem doet zich met name voor in de huisartsenpraktijk. Huisartsenrichtlijnen gelden namelijk voor enkelvoudige aandoeningen. Hierdoor doen zich soms conflicterende adviezen voor. In het project PANDORA worden bestaande richtlijnen onder de loep genomen en verwerkt in een systeem voor computergebaseerde beslissingsondersteuning (CDDS) bij multi- en co-morbiditeit.

Doelstelling
Aanpassing van de huidige huisartsenrichtlijnen voor chronische ziekten, zodanig dat ze kunnen worden verwerkt in een systeem voor computergebaseerde beslissingsondersteuning voor patiënten met co-morbiditeit. Het te ontwikkelen systeem richt zich met name op de behandeling van patiënten met een cognitieve stoornis, zoals de ziekte van Alzheimer, die ook kampen met een cardiovasculaire aandoening.

Het aantal ouderen in de samenleving neemt toe waardoor chronische ziekten steeds meer voorkomen. Ongeveer tweederde van de patienten ouder dan 65 jaar hebben minstens twee chronische ziekten tegelijkertijd (comorbiditeit), wat het lastig maakt om deze patienten effectief te behandelen. Met name in de huisartsenpraktijk is dit een groot probleem, omdat behandelingen vaak worden bepaald door raadpleging van richtlijnen ontwikkeld door het Nederlands Huisartsen Genootschap (NHG), die gelden voor enkelvoudige aandoeningen. Hierdoor kan de huisarts worden geconfronteerd met conflicterende adviezen voor patienten met meerdere aandoeningen. Onderzoek heeft aangetoond dat het tegelijkertijd volgen van meerdere richtlijnen ongewenste effecten kan hebben voor de patient. In dit project is op basis van grote databestanden van representatieve huisartspraktijken en statistische machine learning getracht meer inzicht te krijgen in hoe ziekten elkaar onderling beinvloeden. Daarnaast is onderzocht of informatie uit medische richtlijnen gecombineerd kan worden met resultaten van data-analyse om tot betere richtlijnen te komen. Expertise uit afdelingen voor geriatrie, interne geneeskunde en huisartsengeneeskunde vormenden de basis voor het identificeren van problemen rondom comorbiditeit en voor het ontwerp van computermethoden.

Samenvatting van de aanvraag

According to the Greek mythology, Pandora opened a jar, referred to as "Pandora's box", releasing all the evils of mankind, in particular diseases. Comorbidity can be seen as a Pandora's box, which explains the name chosen for the project. The aging of the population is associated with an increase in prevalence of chronic diseases. About two-third of the patients older than 65 years have at least 2 chronic conditions at the same time, making the treatment of these patients a clinical challenge. The terms 'comorbidity' and 'multimorbidity' are often used to describe presence of multiple disorders. These terms have slightly different meanings. Feinstein [7] originally described comorbidity as "any distinct additional entity that has existed or may occur during the clinical course of a patient who has the index disease under study". Van den Akker and colleagues [37] further refined both concepts, reserving the term 'multimorbidity' to describe the co-occurrence of two or more chronic conditions. Unfortunately, much confusion still exists in the literature, where the two terms often seem to be used interchangeably. For the purpose of this proposal, for both terms 'comorbidity' and 'multimorbidity' the focus will be solely on chronic diseases. To save space, these terms are sometimes taken synonymously in this application. In particular the general practitioner (GP) is faced with the problem of treatment of patients with multiple disorders, where directions about optimal medical management come from multiple clinical guidelines, in the Netherlands also known as the Dutch College of General Practitioners (NHG) standards. Unfortunately, clinical practice guidelines for chronic diseases focus on the management of isolated disease and, thus, when a general practitioner is confronted with a patient having two or more diseases, exactly as many guidelines need to be consulted from the more than 80 a Dutch GP has available. These guidelines may give conflicting recommendations, and it is thus hard for the GP to decide what to do. It has been shown that adherence to multiple clinical guidelines may have undesirable effects for the treatment of patients with comorbidity or multiple morbidity. The main aim of the research proposed in this application to redesign existing GP guidelines concerning chronic disease management for use within a computer-based decision support system such that it is possible to apply them in the case of comorbidity or multimorbidity. NHG standards for the treatment of cardiovascular disease and dementia, and their interaction, will be taken as a point of departure for this research. The standards will be redesigned towards a computer-based decision support system that is especially aimed at supporting general practitioners in the management of patients with cognitive disorders, such as Alzheimer's disease, in the presence of cardiovascular disease. Expertise available within the the Departments of Geriatrics, Internal Medicine and General Practice will act as the basis for the identification of problems around comorbidity for the redesign of guidelines towards a decision-support system. The decision-support system will embody general principles that will allow coping with conflicting management options in comorbidity. The main aims of the research described in this grant applications are: (1) to develop methods for identifying, representing and reasoning about disease interactions and conflicting clinical interventions found when combining clinical guidelines, using methods from logic, probability theory (in particular, probabilistic graphical models), decision theory, and computer-based executable clinical guidelines; (2) to redesign NHG standards on the management of chronic diseases using ideas from computer-based, executable guidelines, and the representation and reasoning methods mentioned at (1); (3) to develop a decision-support system that is able to guide and support GPs in the management of chronic disease in the face of comorbidity; (4) to develop methods for the evaluation of a decision-support system for comorbidity; (5) to develop methods for the integration of the decision-support system into the care process, taking into account the NICTIZ information architecture; (6) evaluation of the decision-support system using data from actual patients.
