Bewegen is onmisbaar voor het behoud van dagelijks functioneren. Niettemin bewegen veel ouderen te weinig. Het stimuleren van bewegen kan onderdeel zijn van de dagelijkse zorg die ouderen ontvangen. Zo kunnen thuiszorgmedewerkers ouderen stimuleren om dagelijkse activiteiten (m.b.t. zelfverzorging/ huishouden) zo zelfstandig mogelijk uit te voeren. Veel medewerkers hebben echter de neiging om handelingen over te nemen en besteden weinig aandacht aan het stimuleren van bewegen. In de training ‘Blijf Actief Thuis’, bestaande uit een startbijeenkomst en een reeks teambijeenkomsten, leren medewerkers hoe ze deze verandering in de dagelijkse praktijk vorm kunnen geven. Daarnaast zullen wekelijkse nieuwsbrieven en een opfrisbijeenkomst ervoor zorgen dat de nieuwe werkwijze beklijft. In dit onderzoek wordt gekeken of ‘Blijf Actief Thuis’ een positieve invloed heeft op gezondheid, welzijn en zorggebruik van ouderen. Bekijk hier de factsheet van dit project.

Eindverslag

Een actieve leefstijl draagt bij aan het succesvol ouder worden. Echter veel ouderen bewegen te weinig, vooral als ze zorgafhankelijk worden. Idealiter sluit de zorg aan bij de eigen kracht van ouderen. Echter, in de praktijk wordt veel zorg overgenomen. Het trainingsprogramma Blijf Actief Thuis is ontwikkeld om het gedrag van thuiszorgmedewerkers te veranderen. In een experiment met 313 medewerkers en 264 ouderen is met beweegmeters, vragenlijsten en fysieke tests naar het effect van Blijf Actief Thuis gekeken. Bovendien is de uitvoering van het programma gemonitord en zijn de ervaringen van medewerkers met interviews in kaart gebracht. Uit het experiment bleek dat het programma volgens protocol is uitgevoerd en dat medewerkers enthousiast waren. Echter zijn geen positieve effecten aangetoond ten aanzien van het beweeggedrag van ouderen, hun gezondheid en het welzijn en zorggebruik/ -kosten. Het wordt aanbevolen om Blijf Actief Thuis vóór de verdere uitrol eerst door te ontwikkelen.

Samenvatting van de aanvraag

BACKGROUND: In the Netherlands, the proportion of older adults (>65 years) will rise from 3 million in 2015 to 4.7 million in 2060; a serious challenge for the sustainability of the Dutch healthcare system. It is well established that physical activity plays a key role in the prevention of negative health outcomes such as chronic diseases, falls, obesity, metabolic syndrome, mental disorders, and musculoskeletal diseases. However, older adults, especially those with low socioeconomic status, have a highly sedentary lifestyle. In general, older adults spend approximately 80% of their awake time in sedentary activities which represents 8 to 12 hours per day. In the past, numerous stand-alone exercise programs have been developed. However, it is challenging to persuade older adults to become and maintain physically active. Reasons may be a lack of motivation, fear (of falling), depression or a poor understanding of the long-term benefits of physical activity. Consequently, physical activity should be embedded in the daily life of older adults to reduce their sedentary time, prevent negative health consequences and facilitate ageing in place. INTERVENTION: ‘Stay Active at Home’ is not an additional, (classical) exercise programme; physical activity is integrated in usual care. In the Netherlands, many older adults receive regularly homecare services or visits from a practice nurse of their GP. However, professionals tend to meet their clients’ needs by task completion rather than by stimulating self-management and active engagement in tasks. Thereby they deprive older adults of their opportunities to engage in a routine range of movements resulting in further functional decline. To prevent these negative consequences ‘Stay Active at Home’ aims to change the behaviour of healthcare professionals from ‘doing things for older adults’ to ‘engaging older adults’ in daily life in order to improve physical activity among older adults by reducing their sedentary time. For example, washing the upper body and face independently; changing the pillowcase, while professional changes bedcover; and motivating clients to join a dancing class at the community centre. Thereby older adults will be continuously stimulated during regular care moments, in a one-to-one relationship to participate in daily and physical activities and to decrease their sedentary time. It is assumed that this approach facilitates a long-term behavioural change in older adults. ‘Stay Active at Home’ is systematically developed based on international evidence in close collaboration with Dutch stakeholders. The feasibility of the programme and the proposed study design are evaluated in two pilot studies prior to the grant period. However, the (cost-) effectiveness of ‘Stay Active at Home’ is not yet known. STUDY AIM: The aim of the proposed study is to provide evidence about the (cost-) effectiveness of ‘Stay Active at Home’ prior to dissemination and implementation of the programme. Alongside the trial an extensive process evaluation will be conducted. DESIGN: A quasi-experimental design with 12 months follow-up will be conducted comparing two districts in the region Eastern South Limburg. In one district all homecare teams from MeanderGroep Zuid-Limburg and the practice nurses of the local GP practice will be trained in ‘Stay Active at Home’ (intervention group). A comparable district will deliver usual care (control group). Data will be collected from participating healthcare professionals and older adults. Older adults have to fulfil the following inclusion criteria: 1) receiving homecare services; and 2) having a high level of sedentary behaviour (> 10 hours/ day; measured by means of the LASA questionnaire). DATA COLLETION AND ANALYSES: The primary outcome is sedentary behaviour (min/day) measured by means of a wrist-worn accelerometer. Secondary outcomes (i.e. physical and psychological functioning, falls, daily functioning, health-related quality of life, nursing home admission, healthcare utilisation/costs and mortality) will be collected by performance-based and self-reported measures. Data for the economic evaluation will be collected by means of a self-developed cost questionnaire, which will be assessed at baseline and after 6 and 12 months. For the process evaluation a mixed-methods design will be applied. Qualitative and quantitative techniques will be used to analyse the data. KNOWLEDGE UTILISATION: Relevant stakeholders will be incorporated in all research phases from study preparation (including two pilot studies prior to the grant period) until dissemination/implementation. By incorporating their experiences, findings are more likely to be relevant and the likelihood of successful implementation increases. Furthermore, knowledge utilisation at local, national and international level will be facilitated by making use of our strong professional and scientific network.
