Patiënten die een ongeval doormaken noemen we ook wel ‘traumapatiënten’. De zorg voor traumapatiënten is bij uitstek ketenzorg. Dat betekent dat alle schakels in de keten, vanaf het moment dat de ambulance bij de patiënt arriveert, tot de behandeling op de spoedeisende hulp, de operatie, de ziekenhuisopname en de revalidatie goed op elkaar moeten aansluiten voor een optimaal resultaat. Traumarevalidatie vindt voor een belangrijk deel buiten het ziekenhuis plaats en tijdens deze fase is er momenteel weinig contact met het ziekenhuis. Uit een eerdere pilotstudie in VUmc Amsterdam bleek dat zorguitkomsten verbeterden na de invoering van het Transmurale Trauma Zorg Model (TTZM), een nieuwe manier van traumarevalidatie waarbij er geregeld contact is tussen de behandelaars binnen en buiten het ziekenhuis. Met behulp van deze ZonMw subsidie zullen in 9 Nederlandse ziekenhuizen de kosten en zorguitkomsten van het TTZM vergeleken worden met die van de gebruikelijke traumarevalidatie.

Samenvatting van de aanvraag

DOEL Is revalidatie volgens het Transmurale Trauma Zorg Model (TTCM) (kosten)effectief vergeleken met de huidige revalidatie voor patiënten met één of meerdere botbreuken ten gevolge van een trauma? HYPOTHESE We verwachten dat TTCM kosteneffectief is t.o.v. de gebruikelijke zorg op: Kwaliteit van Leven (QALY), fysiek functioneren & pijn STUDIE OPZET Gecontroleerde voor-na studie POPULATIE Mensen met 1 of meerdere fracturen t.g.v. trauma INTERVENTIE: TTCM model kent 4 componenten: 1) Intake & follow up door gespecialiseerde trauma fysiotherapeut en traumachirurg tijdens poliklinisch spreekuur; 2) coördinatie van zorg door dit team (trauma-fysio=case manager) en vaststellen individuele behandeldoelen; 3) Revalidatie in 1e lijn door gespecialiseerd trauma fysiotherapie netwerk; 4) e-health support en behandeling volgens protocollen. GEBRUIKELIJKE ZORG Alleen traumachirurg ziet patiënt, ad hoc verwijzing naar 1e lijns-fysiotherapie zonder specifieke trauma expertise; geen coordinatie door case-manager UITKOMSTMATEN Primair: QALYs (EQ 5D-5L), pijn(NRS) & fysiek functioneren Secundair: Patiënt Specifieke Function Scale (PSFS); Ervaren Herstel (7-punts schaal); Patiënt tevredenheid (NRS) Kosten: maatschappelijk & gezondheidszorg kosten. Uitkomsten worden op 3, 6 en 9 maanden gemeten. SAMPLE SIZE /ANALYSE n=600 (a=0.05;beta=90%;ICC=0.01;20% drop out). De intention-to-treat multilevel analyse (linear mixed model/generalized mixed model, inclusief multiple imputation) analyseert de data gebruikmakend van de responses op baseline en 3, 6 en 9 maanden (levels: ziekenhuis, patiënt, meetmoment) KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE (BIA) De economische evaluatie wordt uitgevoerd vanuit maatschappelijk en gezondheidszorg perspectief. Gebruik van resources en kosten worden gemeten d.m.v. kosten-vragenlijsten en gewaardeerd middels Handleiding Voor Kostenonderzoek. ICERs worden geschat en de multilevel analyse verdisconteert de ‘cluster effecten’. Onzekerheid rondom ICERs wordt geschat d.m.v bootstrapping. Sensitiviteits-analyses onderzoeken de robuustheid van de resultaten. De BIA (uitgevoerd volgende de Nederlands richtlijn) hanteert een horizon van 5 jaar. TIJDPAD Maanden: (0-7) voorbereiding (logistiek, METC, lokale uitvoerbaarheid, data management); (8-14) inclusie control patiënten; (9-23) follow up controle patiënten; (24-30) ziekenhuizen switchen naar TTCM; (26-32) inclusie TTCM patiënten; (27-41) follow up TTCM patiënten; (41-48) implementatie activiteiten; analyse & publicatie) Summary RESEARCH QUESTION Is the Transmural Trauma Care Model (TTCM) (cost)effective compared to regular care in post-clinical rehabilitation of trauma patients? HYPOTHESIS TTCM is (cost)effective for QALYs (EQ 5D-5L), pain (NRS) and ‘disease-specific’ physical functioning (Patient Reported Outcomes: PROMs) compared to regular care DESIGN A controlled before-after study, using propensity score weights POPULATION Patients with 1 or more fracture(s) as a result of a trauma INTERVENTION: TTCM consists of: 1) Intake/follow-up consultations by multidisciplinary team (trauma surgeon & trauma physiotherapist [PT]) 2) Coordination by hospital team (Trauma PT = casemanager) & individual goal setting 3) Primary care network of trauma PTs 4) E-health support and treatment according to protocols COMPARISON Only trauma surgeon sees patient; no structured referral to primary care PT (without trauma expertise) & no coordination OUTCOMES Primary: QALYs (EQ 5D-5L), pain(NRS) and ‘disease-specific’ physical functioning Secondary: patient-specific Function Scale(PSFS); Perceived recovery (7-point Global Perceived Effect Scale); Patient satisfaction (11 point NRS) Costs: Societal & health care costs Outcomes are measured at 3, 6 and 9 months. SAMPLE SIZE/ANALYSES n=600 (a=0.05;beta=90%;ICC=0.01;20% drop out). The intention-to-treat multilevel analysis (linear mixed model/generalized mixed model using multiple imputation) will use the responses at baseline and 3, 6 and 9 months (levels: hospital, patient, time) COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS The economic evaluation will be conducted from the health care and the societal perspective. Resource utilization and costs are measured using cost questionnaires and valued using the Dutch manual of costing. Incremental Cost-Effectiveness Ratios (ICERs) will be calculated and linear multilevel analyses take into account possible clustering of data. Bootstrapping techniques will be used to estimate the uncertainty. Sensitivity analyses will be performed to test the robustness of the study results. The budget impact analysis will follow the Dutch guidelines, using a time horizon of 5 years.
