Cost effectiveness of an E-health module on Return to Work embedded in collaborative occupational health care for depressive disorders

Common mental disorders, zoals depressie, angst en somatisatie, gaan vaak gepaard met langdurig verzuim. Dit heeft grote gevolgen voor zowel de maatschappij als de individuele werknemer. ECO is een interventie gericht op werkhervatting en bestaat uit een internetcursus voor de werknemer (Return@Work) en een beslisondersteuning per email voor de bedrijfsarts. In een gerandomiseerd onderzoek is onderzocht of de ECO-interventie leidt tot een snellere terugkeer naar werk in vergelijking met de gebruikelijke verzuimbegeleiding.

Resultaten:
Uit het onderzoek blijkt dat de werknemers die de ECO-interventie kregen, een snellere eerste moment van werkhervatting (gedeeltelijk of volledig) hadden dan de werknemers die de gebruikelijke verzuimbegeleiding ontvingen. Daarnaast leidt de inzet van de ECO-interventie tot een kosten reductie ten opzichte van de gebruikelijke zorg.

Depressie komt vaak voor in Nederland en gaat bovendien vaak gepaard met langdurig ziekteverzuim. Omdat verzuim niet alleen hoge kosten voor de maatschappij met zich meebrengt, maar ook grote consequenties voor het leven van de individuele werknemer heeft, is aandacht voor werkhervatting van groot belang. In Nederland heeft de bedrijfsarts een centrale rol in de begeleiding van verzuimende werknemers. Hoewel er richtlijnen bestaan voor de begeleiding van depressieve, verzuimende werknemers, is de aanwezigheid van die richtlijnen alleen niet voldoende voor een goede uitvoering ervan. Een gebrek aan tijd en vaardigheden belemmeren de begeleiding, evenals een gebrek aan communicatie tussen de bedrijfsarts en andere behandelaars. Om de werkhervatting bij depressieve, verzuimende werknemers te bevorderen, is ECO ontwikkeld: E-health embedded in Collaborative Occupational health care. ECO is een E-health interventie die bestaat uit een internetcursus gericht op werkhervatting voor de werknemer (Return@Work), en een decision aid die de bedrijfsarts duurzaam ondersteunt in de verzuimbegeleiding van de depressieve, verzuimende werknemer. In dit onderzoek wordt de kosteneffectiviteit van ECO vergeleken met de kosteneffectiviteit van de gebruikelijke zorg. De deelnemende werknemers worden een jaar lang gevolgd. Het primaire doel van ECO is om de duur tot werkhervatting te verkorten. Daarnaast zal ook gekeken worden naar de ernst van de depressieve klachten van de deelnemende werknemers.

Samenvatting van de aanvraag

.Background: Relevant stakeholders in the return to work (RTW) process, as employers, the government, care- and income insurance agencies, have been slow to adopt effective depression treatment programs due to lack of applicability and lack of evidence for cost-effectiveness from their perspective. However, several RCTs have evaluated effects of such programs on depression and functioning outcomes, such as collaborative care. Those focused on primary care samples rather than on workplace samples, that would result in health gain by employer-based screening and disease management efforts reducing depression related indirect costs. Therefore, the proposed RCT addresses this issue. The chosen themes are E-mental health and Organisation of care. .Aim: Primary objective of this trial is to estimate the outcome in terms of Return To Work (RTW) of an E-health RTW module embedded in Collaborative Occupational health care (ECO), for workers with major depressive disorder (MDD). Secondary objectives are to evaluate outcomes in terms of severity of MDD, and in terms of costs. .Methods/design: Two-armed cluster-randomized trial in which an E-health RTW module embedded in Collaborative Occupational health care (ECO) for MDD will be compared to usual care (UC). Randomization at the level of Occupational Physicians (OPs) in order to prevent contamination and thus dilution of the effect. Follow up at 3, 6, 9 and 12 months. .Study population: Workers with MDD who are on sick leave for between 4 and 12 weeks .Intervention: The ECO-intervention consists of two components, a combined E-mental health module aimed at RTW and a collaborative care program supported by a webbased tracking system. The intervention aims to enhance the recovery on RTW and on symptom level, by tailoring care management of workers with MDD who are at risk of long-term work incapacity by OP, GP and mental health specialists in collaboration with worker and employer. Screened workers that receive ECO will be facilitated and advised on an effective mental health care and RTW treatment, according to clearly defined set points for stepping up to higher treatment levels. The E-health RTW module and Collaborative care model is applied in the Occupational health setting (ECO) and comprises: 1) detection of Major Depressive Disorder (MDD) amongst workers on sick leave by screening and 2) supporting diagnosis and treatment by occupational physicians (OPs), both by a webbased tracking system, 3) empowerment of the worker by an E-health RTW module, 4) facilitating collaborative and stepped care of MDD by evidence-based treatment recommendations given by training and supervision of OPs and supported by the tracking system and consultant psychiatrist 5) relapse prevention, and 6)work interventions, including consultation with employer and eventual workaccomodations. .Outcome measures: Primary outcome measure is time to fulltime RTW. Secondary outcome measures are severity of depressive symptoms, as measured with the Patient Health Questionnaire (PHQ9), and Costs: Direct health care and indirect productivity loss (average daily wage multiplied by number of sick leave days) costs will be collected. A cost-minimalisation analysis will be performed. .Sample size calculation/data analysis: The primary outcome is RTW. In order to calculate the sample size we assumed that a Hazard Ratio (HR) of 2.0 is the smallest clinical and societal relevant ratio. Based on the assumption that a HR of 2.0 on days until full RTW for workers in the intervention versus the control group, and that a minimum of 67% of the participants achieve a full RTW during the follow-up, the calculation showed that a sample size of 98 completers is needed. As MLA will be performed, 120 will be needed. When taking into account a loss to follow-up of 10%, 132 workers will have to be enrolled in the study for 120 completers.(alpha = .05; power 0.90) To assess the effects on RTW, intention-to-treat analysis will be performed by Multi Level Analysis. To analyse the Hazard Ratio of the RTW rates the Cox Proportional hazard model will be used. In order to estimate the number of days to RTW in both experimental conditions, a survival analysis will be used. .Economic evaluation: A cost minimalisation analysis will be conducted according to the intention-to-treat principle. Costs will be calculated for both experimental conditions and analysed which intervention leads to lowest costs. Hence, all relevant effects and costs due to resource utilisation within the healthcare (direct medical costs) and costs due to production losses (productivity costs) will be included. 95% confidence intervals (CIs) of the difference in mean costs in both groups will be computed by bias corrected and accelerated (Bca) bootstrapping with 2000 replications. .Time schedule: Entire study duration four years. Preparation: 6 months. Inclusion and intervention phase: 24 months. FU 12 months. Data analysis and writing 6 months.
