ACT your way: A Transdiagnostic treatment approach for multiple disorders in Transitional-age youth.

Onderzoek laat zien dat psychische problemen het meest voorkomen bij jong volwassenen (15-25 jaar). Een effectieve behandeling voor deze groep lijkt dus hard nodig. In deze studie wordt ACT your way (AYW) onderzocht, een behandeling specifiek voor deze doelgroep.
AYW is gebaseerd op Acceptance en Commitment Therapie (ACT) waarbij de focus niet ligt op het verminderen van de klachten, maar op het verhogen van kwaliteit van het leven. Onze verwachting is dat AYW in vergelijking met reguliere zorg; betere effecten laat zien, minder kost, zorgt voor een betere therapeut-cliënt relatie, minder uitval heeft en meer behandeltevredenheid laat zien. Om dit te toetsen gaan we een onderzoek uitvoeren waarin AYW wordt vergeleken met reguliere zorg bij 116 jong volwassenen met verschillende psychische problemen. Er worden een voormeting, nameting en follow-up meting na 6 maanden gedaan met zowel interviews als vragenlijsten bij de jong volwassene, de ouder en hulpverlener.

Transitional age youth (TAY) has recently been identify as a group in need of especial attention in mental health services. Studies show that psychological problems are most common in this age range (15-25). So an effective treatment for this specific age group seems warranted. However, there is a lack of treatments specifically developed for this Transitional-age youth. We propose a transdiagnostic treatment that can be used for a wide variety of psychopathy and can also be used for chronic and/or recurrent problems for this specific age group. A randomized effectiveness trial will be carried out in which ACT your way will be compared to Treatment As Usual (TAU). Clients will be monitored by pre, post and 6-month follow-up multi-informant measurements. In total, 134 TAY who are referred to a mental health care facility for psychopathology (including chronic, comorbid and recurrent diagnoses) will be included. We expect ACT your way to be more cost-effective compared to TAU. Besides ACT your way will lead to better treatment alliance, less drop-out and more treatment satisfaction. Samenvatting / Summary • OBJECTIVE(S)/RESEARCH QUESTION(S) Transitional age youth (TAY) has recently been identify as a group in need of especial attention in mental health services. Studies show that TAY (15-25) experience more psychological problems than other age groups. An effective treatment for this specific age group seems warranted. However, there is a lack of treatments specifically developed for this age group. We propose a transdiagnostic treatment, ACT your way, that can be used for a wide variety of psychopathologies and can also be used for chronic and/or recurrent problems for this specific age group. • HYPOTHESIS We expect ACT your way to be more cost-effective compared to TAU. Besides ACT your way will lead to better treatment alliance, less drop-out and more treatment satisfaction. • STUDY DESIGN A randomized effectiveness trial will be carried out in which ACT your way will be compared to Treatment As Usual (TAU). Clients will be monitored by pre, post and 6-month follow-up using multi-informant measurements • STUDY POPULATION(S)/DATASETS In total, 134 transitional age youth who are referred to a mental health care facility for diverse psychopathology (including chronic, comorbid and recurrent diagnoses) will be included. • INTERVENTION ACT your way, a transdiagnostic treatment based on Acceptance and Commitment Therapy • USUAL CARE /COMPARISON Treatment as usual • OUTCOME MEASURES The primary outcome measure is the presence of the primary disorder and number of diagnoses, measured by the K-SADS, a semi-structured diagnostic interview. Secondary outcome measures at pre, post and follow-up include; psychological flexibility, quality of life (EuroQol), young adult psychopathology (YSR), global functioning (CGAS), parent-child communication, conflicts, treatment expectancy (PETS), treatment satisfaction , alliance (TASC & TPOCS), emotion regulation, self-esteem, and self-compassion. • SAMPLE SIZE CALCULATION/DATA ANALYSIS Statistical power analysis assumes a total sample size of 134, 62 per group and a type I error rate of .05, actual power of 0.80 to detect an effect size of f2=0.10, a small effect. • COST-EFFECTIVENESS ANALYSIS Cost-effectiveness analyses will be done by comparing costs and effects of ACT your way versus TAU and will be expressed in 1) incremental costs per QALY and 2) incremental costs per young adult free of diagnosis. Costs are measured with a cost questionnaire that has been used in various studies (Bodden et al., 2008; Bodden et al., 2018). • TIME SCHEDULE Preparation phase (4 months), Training phase (2 months), Inclusion phase (24 months), Posttreatment measurements, 6-month follow-up measurements and End phase (9 months).
