Preact to lower the Risk of Falling by Customised Rehabilitation (PRECISE)

Jaarlijks valt een op de drie senioren in de eigen woning. Voor deze mensen heeft dit vaak grote impact. Maar ook de organisatie en financiering van zorg worden hier flink door geraakt.

Om vallen te voorkomen, worden senioren gestimuleerd om te bewegen. Ook kan vaak een persoonlijk programma worden aangeboden door bijvoorbeeld een fysiotherapeut. Het dilemma hier is echter dat sommige programma’s te algemeen zijn. Een persoonlijke begeleiding op basis van een goede screening is echter weer kostbaar.

PRECISE ontwikkelt een tool waarmee een zorgprofessional of mantelzorger een oudere kan screenen. Vervolgens wordt een passende interventie voorgesteld.

De tool werkt op basis van een algoritme, met behulp waarvan het risico op een valincident nauwkeurig kan worden voorspeld. Daarvoor wordt onder meer gebruik gemaakt van het platform van DigiRehab, dat met goede resultaten in de thuiszorg wordt gebruikt.

PRECISE: 8 partners uit 5 landen. Financiering komt mede vanuit het AAL-EU programma.

Samenvatting van de aanvraag

Dit is de aanvraag voor twee NL deelnemers in het AAL-project PRECISE. Hierbij de Engelstalige samenvatting: Falls are a major global public health problem. In fact, 30% of people aged 65 or older experience 1 fall a year on average – making these a huge burden for healthcare systems and society in general. Presently there is no data-driven decision support systems (DSS) that are able to assess the risk of falling accurately – and no preventive targeted interventions are taking place. Based on the findings accomplished so far, PRECISE aims to build an effective Artificial Intelligence-Machine Learning (AI-ML) DSS platform, that uses an extensive data collection (screening and local data) from different sources and end-users' groups across EU. Our approach will enable the prediction of the risk of falling in elderlies along with the prevention of falling through a customized physical training scheme upon a prior assessment. The outcomes of PRECISE comprise a fully mature and accurate DSS platform to evaluate the risk of falling in the elderly age group. This assessment will enable tailored and customized interventions according to a specific individual profile. Leveraging from the DGR training platform and the available app, the solution will allow a continuous monitoring and reporting of results after a specific training timeframe. Importantly, public and private homecare providers will be offered a broader and qualified service, that optimizes care services and allocation of man force. Caregivers will be able to deliver a more professional service, more efficient and providing the right insight to each individual patient. Lastly, PRECISE will also set up flexible business models to each targeted country – so they rightly fit into customers and end-users needs and requirements. Ultimately, primary end- users (elderlies) will see their self-reliance, physical condition, motivation, and sense of purpose improved significantly - thus reducing healthcare costs and the use of medicine, while avoiding clogging healthcare services. DigiRehab is a Danish SME founded in 2010 that currently employs 12 people. Behind DigiRehab, there is a team composed by highly skilled individuals with vast experience in physical rehabilitation of elderlies, welfare technology, IT development and business development. DigiRehab will be the commercialization leader of PRECISE ´s outcomes - responsible for taking the lead of selling the solution (DSS platform) along with the DGR training platform, and the DigiRehab adjuvant training app. DigiRehab ´s vision is to ensure quality of life and self-reliance for as many elderly people in homecare as possible – in a cost-efficient way. By 2017, DigiRehab has successfully launched the DigiRehab training platform in Denmark (DK) and Norway (NO), yielding very high and positive impact in the improvement of elderly’s life quality and significantly decreasing the provider’s homecare costs. Presently, DigiRehab deploys the DigiRehab training platform in the Danish and Norwegian markets. Based on the insights taken from the last few years, we envision that the new solution will have its market entry by mid-2024 in these two Nordic countries. Leveraging from PRECISE partnerships, we envision preliminary sales to occur in the Netherlands (NL), Poland (PL), and Italy (IT) simultaneously. A 2nd commercialization phase over 2025-26 is expected to happen smoothly, as we expand to the other Nordic countries (Sweden and Finland) that include comparable homecare assistance systems with DK and NO. Belgium and Austria will be part of the second wave, as our preliminary market assessment reveal a similar approach as the one with NL. Finally, a 3rd commercial phase by 2027-28 includes further expansion to the rest of Europe (EU), as the solution increases its maturity level – and depending on the rate of success, US and Japan will follow. Dissemination will be carried out through several instruments: i) a project website to generate public awareness; ii) Posters, leaflets, and brochures; iii) scientific publications that contribute to the EU pool of knowledge; iv) educational training events to allow a hands-on approach of our solution to the general public; v) conferences and exhibitions to effectively share the results with the industry and academia; and vi) popular and social media, as it is an easy and fast way to disseminate information to broader audiences. Target groups include: i) primary end-users (elderlies); ii) secondary end-users (e.g., homecare providers, formal and informal caregivers); iii) tertiary end-users (e.g., municipalities and insurance companies); iv) policy makers and regulators/authorities (i.e., EU Commission and Health & Social National/Regional authorities); v) research community & educational institutions & key opinion leaders (i.e., within universities and research centres); vi) media groups, journalists, communicators; and vii) general public and non-profit organisations.
