De Convalescent plasma (ConvP) and COVig PK/PD studie: een onderzoek naar het nut van behandeling met antilichamen uit plasma ter behandeling of preventie van het coronavirus (SARS-CoV-2)

Projectomschrijving

In de ConvP/COVIg PK/PD studie hebben we proefpersonen die zelf geen antistoffen aanmaken
tegen het coronavirus behandeld met convalescent plasma of hyperimmune globulines (COVig).
Voorheen waren er nog geen studies die onderzochten hoeveel plasma of COVig gegeven moet
worden om ervoor te zorgen dat een patiënt in de weken of maanden na toediening voldoende
antistoffen heeft in zijn/haar bloed.

In deze studie werd na toediening van het plasma of COVig op vaste tijdstippen bloed afgenomen
en onderzocht hoeveel antistoffen er tegen het coronavirus aanwezig waren en hoelang. Op basis
van deze metingen hebben we een model gebouwd dat, na toediening van plasma of COVig, de
concentratie van antistoffen kan voorspellen in het bloed.

Het onderzoek toonde aan dat het plasma dat in het eerste jaar van de pandemie van COVID19
genezen patiënten werd afgenomen te weinig antistoffen bevatte om mensen zonder antistoffen
langdurig te kunnen beschermen.

Het ontwikkelde model kan wel een methode vormen die bij het design van toekomstige studies
naar het preventieve of therapeutische gebruik van deze middelen kan worden gebruikt (bv. tegen
andere virussen).

Samenvatting van de aanvraag

We designed the Cov-Early study to evaluate the effect of convalescent plasma (convP, i.e. plasma from ex-COVID-19 patients that contains virus neutralizing anti-SARS-CoV2 antibodies) on a 5-point disease severity scale ranging from symptom free over hospital admission and death. The Cov-Early study is designed as a nationwide multicenter double-blind randomized clinical trial that focuses on patients early in their COVID-19 disease course. 700 SARS-COV-2 RT-PCR positive patients will be randomized to the standard of care plus 300ml of non-convalescent fresh frozen plasma or 300ml or convP with a virus neutralizing antibody titer of 1/320 or higher. Patients included are outpatients, symptomatic for <8 days and fulfill predefined criteria that have been demonstrated to be associated with a higher risk of a poor outcome (i.e. age 70 or older or age 50 in combination with certain comorbidities). Simultaneously with the COV-Early trial we plan a phase I pharmacokinetic study. Although convalescent plasma (convP) from healthy ex-COVID19 donors is being used as therapy for COVID19 in many parts of the world and mostly outside the context of a clinical trial no phase 1 pharmacokinetic data are available as a guidance towards the appropriate dosing. This has led to extremely different approaches in current treatment protocols ranging from 250 in a Chinese study to 1000ml in the Belgium protocol, with other protocols using volumes in between these extremes. An equally variable approach towards the anti-SARS-COV-2 antibody levels that were required is observed as well ranging from plasma from recovered COVID-19 patients without any further antibody testing, over “any detectable antibody” to the use of plasma with high levels of virus neutralizing antibodies only. This illustrates the urgent need for data on which recommendations towards the optimal volume of ConvP as well as antibody titers that are needed. The study will consist of 4 groups of 6 volunteers (age 55-75y). If Sanquin is able to produce a sufficient amount of hyperimmune anti-COVID19 serum (COVig) in time, the pharmacokinetic propoperties of COVig will be studied in 2 additional study arms.
