Effect en veiligheid van COVID-19 vaccinaties in patiënten met primaire immuundeficiënties (VACOPID)

Projectomschrijving

In deze studie wordt onderzocht of patiënten met een aangeboren afweerstoornis na vaccinatie beschermd zijn tegen COVID-19. Patiënten met deze aangeboren stoornissen in het afweersysteem hebben een verhoogd risico op het krijgen van infecties en op een ernstiger beloop van infecties. Zij dijn daardoor mogelijk ook kwetsbaarder voor ernstig beloop van COVID-19. Het is daarom belangrijk dat zij beschermd worden. Echter is nog niet bekend hoe veilig en effectief de COVID-19 vaccins zijn in de groep.

Doel

In dit project wordt onderzocht of patiënten met een aangeboren afweerstoornis na vaccinatie daadwerkelijk beschermd raken tegen COVID-19.

Achtergrond

Bij patiënten met aangeboren stoornissen is het afweersysteem minder goed in staat om infecties te bestrijden. Hierdoor is de kans op infecties hoog. Ze zijn daardoor mogelijk ook kwetsbaarder voor een ernstig beloop van COVID-19. Bescherming tegen COVID-19 door vaccinatie is daarom cruciaal voor deze patiënten, maar door de verminderde afweerfunctie kan het zijn dat het vaccin minder effectief is of minder lang bescherming geeft.

Onderzoeksopzet

Na vaccinatie wordt op een aantal momenten bloed onderzocht om te zien of er in het afweersysteem bescherming optreedt tegen COVID-19 en of deze bescherming voor langere tijd blijft bestaan. Bovendien zal onderzocht worden of deze patiënten toch COVID-19 krijgen ondanks vaccinatie.

Te verwachten resultaten

Inzicht of coronavaccins veilig en effectief werken bij patiënten met aangeboren stoornissen. Dit kan zich vertalen in het individualiseren van de vaccinatie strategie, inclusief het geven van additionele booster vaccinatie voor degene die onvoldoende beschermd zijn.

Uitvoerende partijen

Het onderzoek wordt uitgevoerd door de 7 Nederlandse academische ziekenhuizen, verenigd in de Werkgroep Immuun Deficiënties (WID).

Context

Dit onderzoek is één van acht studies naar de werking van COVID-19 vaccinaties bij patiënten met een verstoord immuunsysteem. Deze mensen lopen een hoger risico op complicaties bij een COVID-19 infectie. Bovendien weten we nog niet goed genoeg of zij voldoende én voldoende lang worden beschermd na vaccinatie.

ZonMw faciliteert deze onderzoeken zodat patiënten, behandelaars en beleidsmakers met de kennis uit deze studies goede keuzes kunnen maken voor de vaccinatiestrategie. De studies werken met gestandaardiseerde protocollen en meetmethoden. Dat wil zeggen dat ze hun data op dezelfde manier verkrijgen en analyseren. Daardoor kunnen de resultaten uit de verschillende studies (bij verschillende patiënten) met elkaar worden vergeleken.

Samenvatting van de aanvraag

Background and research question: Primary immune deficiencies (PIDs), also known as Inborn Errors of Immunity (IEI), are clinically characterized by an increased risk of severe infections. Therefore, these patients may also have an increased risk of adverse outcome of COVID-19 or may experience protracted course of disease. Effective SARS-CoV-2 vaccination would therefore be of great clinical importance in PIDs. However, until now not much is known about the efficacy and safety of SARS-CoV-2 vaccination in these vulnerable patients. In PID patients, the underlying disease may have a significant impact on the ability to develop an effective immune response after SARS-CoV-2 vaccination. Therefore this study aims to assess efficacy and safety of SARS-CoV-2 vaccination in patients with various forms of PID Hypothesis: PID patients show diminished B and T cell response after SARS-CoV-2 vaccination and these patients require intensified vaccination regimens Study design: Prospective, controlled multicenter study Study population: 350 patients with an isolated antibody deficiency (175 patients with IgG subclass deficiency and 175 patients with SADNI),150 patients with either CVID, CID, CGD or XLA and 200 controls (partners or siblings of patients). Intervention: At baseline and after SARS-CoV-2 vaccination blood will be drawn at 4 different time points (baseline, at day 28, and 6 and 12 months post-vaccination). Nasal and oral swabs will be collected at baseline and in case of clinical signs suggestive for COVID-19 infection after vaccination. Questionnaires will be used to monitor for COVID-19 infection and disease outcome despite SARS-CoV-2 vaccination. Outcome measures: Primary endpoint is the antibody response in patients with an isolated antibody deficiency on day 28 after vaccination. Secondary endpoints include safety, monitoring of occurrence of infection despite vaccination, development of vaccine escape variants of the virus, durability of the immune response at 6 and 12 months and levels of SARS-CoV-2 specific T and B cell responses. Sample size Patients with IgG subclass deficiency or SADNI versus controls: Due to underlying PID we expect a lower vaccination efficacy in patients with PADs than in controls. However, we consider a vaccination efficacy of 65% as clinically relevant, and therefore this is taken as the boundary for non-inferiority. Assuming an efficacy of 90% in controls and 65% in PID patients with a non-inferiority limit of 25%, with alpha 0.05 and beta 0.2, two groups including 172 participants per group are required to demonstrate non-inferiority. Patients with CVID, CID, CGD and XLA: CVID, CID, CGD and XLA constitute of a heterogeneous group of clinical pictures. However, it is of utmost importance to better understand the response to SARS-CoV-2 vaccination in this vulnerable patient group. Therefore for these patients an explorative analysis will be performed to evaluate efficacy of SARS-CoV-2 vaccination in this patient cohort, to gain more insights in the per-patient response after vaccination. No power calculation can therefore be provided for this patient cohort. Data analysis A description of the participant population will be included in a statistical output report. Participants who are seropositive to SARS-CoV-2 at baseline and participants who have not received at least one dose of the vaccine will be excluded from the immune response analyses, including the primary analysis. All subjects who received at least one administration of the vaccine will be included in the analyses of AEs. Baseline characteristics of study participants will be reported for each patient cohort and control group. Continuous variables will be presented as mean ± standard deviation in case of a normal distribution or median (interquartile interval) in case of non-normal distribution. Categorical variables will be presented as numbers (percentage). Characteristics of each patient cohort will be compared with control groups using t-test or Mann-Whitney U-test (whichever appropriate based on variable distribution) for continuous variables and Pearson chi-2 test for categorical variables, including evaluation of adverse effects and incidence of Covid-19 infection after vaccination. Durability of antibody response in the different groups will be compared using McNemar test. Evaluation of neutralizing antibodies, presence of antibodies against spike protein or other SARS-CoV-2 proteins, T cell response and antibody titers over time will be analyzed using repeated measures ANOVA, or generalised estimating equations (GEE). The association between baseline (immune) parameters and vaccination response measures (both antibody and T cell responses) will be evaluated using correlation analyses as well as multivariable regression analyses.
