The effectiveness of Nicotine Replacement Therapy for young people aged 12 to 18 years

Door middel van een gerandomiseerde studie is door onderzoekers van de Universiteit Utrecht en de Radboud Universiteit de effectiviteit van nicotine vervangende therapie (NRT) onderzocht onder 265 jongeren van 12-18 jaar die ofwel nicotinepleisters ofwel placebopleisters kregen. Het proefschrift bevestigt eerdere bevindingen dat er onvoldoende bewijs is voor de effectiviteit van nicotinepleisters voor adolescente rokers. Hoewel enkele positieve effecten werden gevonden op de korte termijn, verdwenen deze effecten na 6 en 12 maanden. Ook vonden de onderzoekers dat de therapietrouw laag was, er nauwelijks vermindering in de drang naar roken en ontwenningsverschijnselen werd gerapporteerd, en dat terugval naar roken moeilijk te voorspellen is. Deze resultaten benadrukken de problemen die adolescenten ondervinden in hun pogingen om te stoppen met roken. Er is meer onderzoek nodig naar de effectiviteit van farmacotherapeutische behandelingen, zoals de combinatie van nicotinepleisters met een snellere vorm van NRT of met een bepaalde gedragsinterventie. Ook is het noodzakelijk dat adolescenten, sterker dan tot nu toe het geval is, preventief gewaarschuwd worden voor de verslavende effecten van roken.

Samenvatting van de aanvraag

There is increasing evidence that Nicotine Replacement Therapy (NRT) may help adolescents to successfully quit smoking. However data on the effectiveness and safety of NRT among adolescents are limited and a large clinical trial of nicotine replacement therapy is warranted to be more conclusive about the effects of NRT among adolescents (2005). The present study will be conducted in response to this need. The main aim of this study is to determine the effectiveness and safety of nicotine replacement therapy (NRT) in achieving long-term smoking cessation among young smokers aged 12 – 18 years. In order to do so, a double blind, randomized, placebo-controlled trial will be conducted testing the effectiveness of nicotine gum versus nicotine patch therapy, in comparison to a placebo condition. About 360 participants will be recruited and randomly assigned to one of three conditions: (1) an active patch and a placebo gum condition, (2) an active gum and a placebo patch condition, and (3) a placebo gum and a placebo patch condition (the control condition). The main outcome measures of this study are prolonged abstinence at 6- and 12-month follow up, but short-term indicators of successful quitting will be measures as well. To validate self-reported smoking abstinence, saliva cotinine levels will be determined among a sample of participants.
