Health checks zijn testen op ziekte of risicofactoren bij gezonde mensen. Aan health checks zitten voordelen, zoals mogelijke gezondheidswinst, maar ook nadelen en risico’s, zoals onnodige ongerustheid bij een foute uitslag of gezondheidsschade door vervolgonderzoek. Moeten health checks voor iedereen toegankelijk zijn? En hoe kunnen persoonlijke health checks beoordeeld worden?

Onderzoek

Opvattingen van aanbieders en gebruikers van health checks zijn in kaart gebracht middels expertmeetings, interviews, focusgroepen en een literatuurreview. Met deze input en bestaande ethische criteria en frameworks is een ethisch kader ontwikkeld.

Uitkomst

Er is een ethisch kader ontwikkeld voor de beoordeling van persoonlijke health checks. Dit kader gaat uit van de effecten van health checks op de waarden: gezondheid, gelijkheid in gezondheid, geestelijk welbevinden, autonomie, vertrouwen in artsen en overheden, de verdeling van verantwoordelijkheid voor gezondheid tussen individuen en andere betrokken actoren, solidariteit en privacy. Bij het beoordelen van health checks moet ook gekeken worden naar het totale aanbod van health checks. Daarnaast moet gekeken worden of andere gezondheidsmaatregelen niet effectiever zijn.

De geruststellingsindustrie. Gezondheidstests en -checks bij je zorgverzekering, bij een sportclub, aangeboden door je werkgever, via het internet of gewoon bij je eigen huisarts. De mogelijkheden om je preventief te laten onderzoeken nemen toe. Behalve voordelen zoals geruststelling of het tijdig opsporen van een behandelbare ziekte, zitten er ook nadelen aan. Bijvoorbeeld foute positieve uitslagen, die angst en onnodig onderzoek opleveren. Daarom is de vraag of deze checks goed zijn voor iedereen, of dat er duidelijke, ethisch geladen verschillen zijn tussen de checks. In dit project wordt een ethisch kader ontwikkeld om gezondheidschecks te kunnen beoordelen. We onderzoeken eerst wat aanbieders en gebruikers vinden van gezondheidschecks en aan welke ethische criteria goede checks zouden moeten voldoen. De bevindingen uit het eerste deel worden ethisch geanalyseerd in samenhang met theoretische kaders ontleend aan public health ethics en professionele ethiek.

Samenvatting van de aanvraag

The Bijenkorf department store offered discounted Total body scans during the recent Dwaze Dagen. There are commercial offers of health checks direct to consumers that find an enthusiastic clientele. This indicates a growing market for preventative tests, promises of health and reassurance. Such offers of health checks are everywhere: prevention consultations at GPs, Bronovo online health check and free offers of coronary arteries imaging by the UMCU at the Health Fair (nrc, 21-1-2012). Employers offer their employees free health checks. How should these new health checks be judged ethically? The options range from a permissive stance – let the market regulate these developments - to forbidding particular health checks. There certainly are important differences with government screening programmes, thus ethical investigation is needed beyond the adjusted Wilson&Jungner criteria (GR, 2005). It is essential to conceptually define health checks, identify the ethical concerns raised by health checks and to develop criteria which are suitable for the ethical evaluation of health checks. There are many different tests and checks. In this project we focus on health checks offered by GPs and commercially. The central question of the project is: How should these health checks be evaluated ethically, and by which criteria? For an ethical analysis additional and different considerations are needed, which include the expectations of those undergoing testing, particularly the value of reassurance; the role health checks play with respect to trust in health care (e.g. when patients feel their health is not adequately monitored); and the question whether and how the ethics of such interventions change due to the commercial nature of many providers. In order to evaluate health checks ethically, we start with an inventory of the scientific literature and the results of previous ZonMW projects. We will use different theoretical starting-points for the ethics of health checks: public health ethics, business ethics and professional ethics. We use both empirical and ethical analysis to answer the following questions: What do different health checks measure and how does that relate to health outcomes? What are the aims of those offering health checks? What do consumers of these health checks hope to achieve? What are the responses of consumers and GPs to test results? What ethical concerns do the consumers and providers see? An important focus in the empirical-ethical analysis will be the arguments of both providers and consumers for their involvement in these checks. It seems likely that providers and consumers will have different aims (such as profits or lifestyle changes versus reassurance). Moreover, we would expect differences between different contexts in which health checks are offered. The aims are ethically significant as are the arguments given to participate in the practice of health checks. We will collect and analyse the arguments of both providers and prospective consumers of health checks through qualitative research, which is well-suited to uncover considerations that may have been overlooked in the theoretical debates. The aims, concerns and arguments will be compared with those known from theory and we will consider the overlap and divergence between the different groups. We come to conclusions about the strength of the arguments raised. We aim to develop criteria for the ethical evaluation of health checks, based on these theoretical ethical approaches and ethical concerns raised in the empirical study. This is developed into an ethical framework to be useful in practice for policy makers in this area. To enhance the ethical framework and ensure its implementation, we will engage policy makers with improving and fine-tuning of the framework. Policy makers will be encouraged to consider the framework for policy making in the future. We further consider what policy responses may be needed in the Netherlands to health checks whilst taking into account and evaluating the existing legal framework. Conclusions may be drawn about the desirable direction of policy making, given the culture and history of health care in the Netherlands. The conclusions will be shared in a symposium, which we hope to organise in collaboration with the medical professional organisations and policy makers. Besides discussion and evaluation of possible policy responses, we envisage a range of implementation activities; we hope to be able to contribute the Dutch and European discussion on Health Checks. In addition, a teaching module to discuss the ethics of health checks for GPs will be developed and piloted during the implementation phase.
