Risk profiling for colon cancer

Dikkedarmkanker komt relatief veel voor. Hoe vroeger dikkedarmkanker wordt opgespoord, hoe beter de overlevingkansen zijn. De beste opsporingsmethode is colonoscopie (een onderzoek met een dunne kijkbuis). Colonoscopie is een belastend onderzoek met kans op ernstige complicaties. Daarom wordt colonoscopie bij voorkeur alleen uitgevoerd bij een verhoogd risico. Een andere onderzoeksmethode is de ontlastingtest. De ontlasting wordt daarbij onderzocht op microscopische spoortjes bloed. Helaas wordt met deze ontlastingstest een aanzienlijk aantal tumoren gemist. Daarom wordt in dit onderzoek uitgezocht of er, naast de ontlastingtest, ook andere risicofactoren zijn die het zinvol maken om een colonoscopie te ondergaan. Bij alle deelnemers aan een lopende screeningsstudie naar dikkedarmkanker worden de bekende risicofactoren geëvalueerd, zoals leeftijd, darmkanker in de familie, roken, overgewicht en alcohol. Op basis hiervan wordt een betrouwbaar overzicht gemaakt die helpt om het risico op dikkedarmkanker beter in te schatten.

Samenvatting van de aanvraag

Colorectal cancer is one of the most common and lethal forms of cancer. Fortunately, it is a disorder with a relatively long latent premalignant stage. This pathophysiology offers a clear ‘window of opportunity’ for early detection and effective treatment of the disease in an early phase. Colonoscopy is considered as the most effective method to detect the premalignant stage of colorectal cancer. This procedure, however, has several disadvantages, such as the large patient inconvenience, the possibility of serious complications and the high costs involves. There is, therefore, a broad consensus that colonoscopy should only be offered to individuals with an increased risk for colorectal cancer. In theory this strategy seems reasonable, in practise, however, it is rather dependent on the means available for high risk selection and the (cost-)effectiveness of the selection methods. Based on current knowledge, Faeces Occult Blood Test (FOBT) is the most recognized method to select high risk individuals. Unfortunately, with FOBT a considerable percentage of the tumours in the screened group is missed. Consequently, there is need for further refinement of the selection methods preceding colonoscopy. In this project a risk profiling algorithm for colorectal cancer is determined and validated a in a cross-sectional colonoscopy screening study to evaluate whether multifactorial risk profiling is a logical step to refine the preselection of high-risk individuals for colonoscopy. The results of this project will offer essential information for a fair decision regarding the ideal strategy and target group for colorectal cancer screening in the Netherlands.
