Development, implementation and evaluation of a tailored based physical activity intervention for the over fifties

Lack of physical activity is a major risk factor for diseases like CVD, Diabetes II, and osteoporosis (1,2). Recent studies have shown that around half of the general population in the Netherlands does not comply with the national physical activity guideline, and 14% are reported to be totally inactive. The amount of physical inactivity in older adults is even higher (3). Although regular physical activity has been demonstrated to be critical for the promotion of health and physical functioning as people age, persons over 50 years of age represent the most sedentary segment of the adult population (4). Studies have shown that only 10 to 30% of the older adults report regular exercise (4). Moreover, 50% of the older adults who are not physically active have no intention of starting to exercise regularly in order to increase their activity (4,5). Currently, in the Netherlands 32% of all residents are over fifty years of age (6). Stimulating physical activity for this large group is therefore of major relevance and will be the focus of this study. The general approach in this project will be the Intervention mapping approach (7).This project has three goals, which will be fulfilled during the three phases of the project. First, existing knowledge on the determinants of initiating and maintaining physical activity for the over fifties will be gathered, reviewed in depth, further explored and tested. This will be done by using qualitative and quantitative methods. In recent years many studies were done on determinants of physical activity for the elderly and on interventions to stimulate physical activity for the elderly. Knowledge and findings on relevant determinants of physical activity from these different studies will be gathered and reviewed. Experts in the field will be consulted to add additional relevant information. Based on this extensive review of recent projects and literature, a document will be developed in which all the relevant determinants are listed. A Delphi study among the experts will be carried out to complement the findings of the literature and project review. With focus group interviews among people of the target group we will enrich the expert based findings regarding relevant determinants of physical activity. To test and prioritise the value of the data gathered on determinants, a quantitative determinant study will be carried out. Second, based on the research on determinants, two intervention programs will be developed and pilot-tested in a small-scale study. The first intervention will be a tailored intervention targeted to raise awareness of lack of physical activity, and to stimulate initiation and maintenance of physical activity among the over fifties. The second intervention will consist of two parts: an extended version of the tailored intervention and local activities aimed to facilitate physical activity. The second tailoring intervention will additionally tailor on environmental determinants by giving tailored personalised advice on local possibilities and initiatives for physical activity for the over fifties. By combining this second tailoring intervention with an extra intervention that is focussed on organizing and stimulating specific local activities for older adults, it provides a combined intervention that can include many of the relevant factors associated with stimulating physical activity and adherence of the new behaviour. Third, the two interventions will be implemented in three randomly selected regions, using a longitudinal prospective cohort study design. Three groups will be compared: the control condition (with no intervention), and the two intervention conditions. It is expected that, in comparison to the control group, the first tailoring intervention will lead to significant more subjects becoming physical active, while the second version, with the additional local activities, will lead to less subjects experiencing environmental hindrances to be physical active, resulting in more subjects participating in (organised) physical activities, and therefore more subjects maintaining their physical activity in the long term. Next to the effect evaluation, cost-effectiveness and formative evaluations will be performed. The products of the project will be a dissertation, (inter)national publications, and two interventions ready to be implemented on a larger scale. In order to find the optimal conditions for further implementation of the developed interventions, intensive contact will be sought with the relevant organisations and the target group. Based on this cooperation, advices on the further implementation process will be communicated.
