Breast density as indicator for MRI screening in women at high risk for breast cancer: a randomized clinical trial

Dit project bestaat uit twee studies: de DENSE studie en de FAMRISc studie. Hieronder vindt u per studie een samenvatting.

Resultaten 1e ronde DENSE studie: MRI screening bij vrouwen met zeer dicht borstklierweefsel

Vraagstuk

Vrouwen met heel dicht borstklierweefsel hebben een verhoogde kans op borstkanker. Een bijkomend probleem is dat bij hen aanwezige tumoren door dit dichte klierweefsel moeilijker te vinden zijn. In de DENSE studie wordt bestudeerd of deze vrouwen gebaat zijn bij aanvullend MRI onderzoek na de landelijke borstscreening.

Onderzoek

Ongeveer 4800 vrouwen vormden de onderzoeksgroep die zo’n aanvullend MRI onderzoek ondergingen na reguliere screening. Hun resultaten werden vergeleken met die van vrouwen die alleen de reguliere screening ondergingen.

Uitkomst

De resultaten van de eerste MRI screeningsronde van de DENSE studie laten zien dat bij deelnemers aan het bevolkingsonderzoek borstkanker die zeer dicht borstklierweefsel hebben MRI kan helpen bij het vroegtijdig ontdekken van borstkanker. De borstkankers worden in een eerder stadium gevonden. Het komt daarna ook minder vaak voor dat borstkanker buiten het screeningsonderzoek wordt ontdekt (de zogenaamde intervalkankers). Deze resultaten zijn ingevoerd in rekenmodellen die laten zien dat dit op termijn tot een reductie in borstkankersterfte moet leiden en dat dit kan op een manier die kosteneffectief is volgens (inter)nationale normen. Tegelijkertijd is er ook een toename van overdiagnose en fout-positieven. Met de resultaten van de vervolgrondes die nog gaande zijn kunnen de lange-termijn kosten en baten nog beter worden ingeschat.

 

Beter MRI-screening of mammografie bij vrouwen met verhoogd borstkanker risico? FaMRIsc studie

Vraagstuk

Kunnen vrouwen met verhoogd risico op borstkanker beter met MRI of met mammografie worden gescreened ? En is de keuze afhankelijk van de dichtheid van het borstklierweefsel.

Onderzoek

Vrouwen van 35-60 jaar met een risico van 20% of meer op borstkanker ten gevolge van borstkanker in de familie, zonder BRCA1/2 mutatie, werden na loting gescreened met jaarlijks MRI en om het jaar mammografie of met jaarlijks mammografie.

Uitkomst

MRI-screening ontdekte de borstkankers belangrijk eerder dan mammografie en minder vaak met aangedane okselklieren. MRI had wel een hoger risico op fout-positieve uitslagen (die extra onderzoek nodig maken). De FaMRIsc studie toont dat bij vrouwen met een verhoogd borstkanker-risico (≥20%), screening met MRI het risico op borstkanker sterfte meer kan reduceren dan mammografie. Dit kan voor acceptabele kosten, maar wel met enig risico op over-diagnostiek. MRI blijkt bij iedere borstdichtheid effectiever.

Samenvatting van de aanvraag

In the Netherlands and many other countries, it is now recommended that women with a BRCA1 or 2 mutation are screened by yearly MRI between age 25-60. Less than 5% of all breast tumours are related to such mutations. Having a clear family history of breast cancer, but no BRCA1 or 2 mutation, or having very dense breasts are more common breast cancer risk factors with equally extremely high risk. These high risk groups are currently screened with mammography, although sensitivity of MRI was much higher in all risk and age groups in the Dutch MRISC and comparable international studies. Especially in women with high mammographic density, in whom breast cancer risk is highest, the sensitivity of mammography is seriously impaired, leading to many missed cases. MRI is likely to lead to better detection of breast tumours in these groups. The limitation of all previous MRI screening studies is that they do not contain a comparison group; all participants received both MRI and mammography. Therefore, we cannot empirically assess at which stage tumours would have been detected by either test or whether MRI would reduce the number of interval tumours. We need randomized controlled trials (RCT) to define the ultimate healthcare policy. OBJECTIVE To assess, based on a woman’s risk profile, mammographic density and age, the most cost-effective screening method, mammography or MRI, for large subgroups at high risk. RESEARCH QUESTIONS For this proposal we distinguish two high-risk groups: those with a clear family history (but no BRCA1/2 mutation) and those with extremely dense breasts. 1. What is, in women with >20% familial Cumulative LifeTime Risk (CLTR), the cost-effectiveness of MRI + mammography compared to mammography, and is this dependent on the breast density score? 2. What is, in women aged 50-75 years and who show >75% mammographic density, the cost-effectiveness of biennial screening with MRI + mammography compared to mammography alone? PLAN OF INVESTIGATION An RCT will be carried out in each high risk group, comparing a screening strategy where MRI is added to current practice: 1. High risk due to family history: Women aged 30-55 years, with >20% familial CLTR but no known BRCA1 or 2 mutations. Intervention: ‘yearly MRI and clinical examination + mammography every other year’(n=1,000) versus ‘yearly mammography and clinical examination’ (n=1,000). Results will be stratified by mammographic density to examine whether cost-effectiveness of the screening strategies is dependent on mammographic density. 2. High risk due to extremely dense breasts: Participants in biennial mammographic breast cancer screening programme, aged 50-75 years, with >75% mammographic density. Intervention: After a negative screening mammogram, ‘additional MRI’ (n=3,000) versus ‘no further follow-up until next scheduled screening examination two years later’ (n=12,000). The intervention will be carried out for at least four years, i.e. 4 screening rounds in the family history risk group and 2 screening rounds in the extreme density risk group. Primary outcomes are: the numbers of tumours detected at screening examinations and in between screening examinations (interval tumours), and the stage distribution at diagnosis in the different trial arms, with application of density scores for the analyses of results in the family history risk group. Other outcomes are the sensitivities and proportions of false-positive results. Breast cancer mortality reduction will be estimated using breast cancer microsimulation models (MISCAN). Costs will be calculated per quality adjusted life-year gained. DATA COLLECTION Measurement of mammographic density is done using a fully-automated method (1) on full-field digital mammograms, the routine mammography technique in the hospitals but also in the Dutch screening programme. Registration of all screening results for all participants will continuously be performed: i.e. BI-RADS classifications of MRIs and mammographic examinations, outcome of clinical examinations, diagnostic work-up, tumour stage at diagnosis and other tumour characteristics. The occurrence of interval tumors will be known through linkage with the national cancer registry. For all participants all screening and additional investigations necessary for the final diagnosis will be recorded and the costs calculated, based on Dutch health care resources. RELEVANCE Our proposal aims at innovation of the current ‘one size fits all’ breast screening approaches, by incorporating, besides familial risk information and age, also information on mammographic density. In the current screening strategies women with high mammographic density are seriously underserved because they run the highest breast cancer risk and the sensitivity of mammography is strongly impaired in these women. Personalized screening is expected to increase the numbers of life years gained and could lead to more efficient health care.
