Positieve gezondheid kent geen grenzen in de wijk

Projectomschrijving

Blauwe zorg is niet minder, maar ánders ingerichte zorg in de wijk met de echte vraag van bewoners als uitgangspunt tegen lagere kosten. De aanpak wordt getest in vier lage SES-wijken in Maastricht.

Doel

Onderzocht is de kwaliteit van leven en kwaliteit van zorg van de inwoners, het werkplezier van professionals en de zorgkosten.

Aanpak

De aanpak van gezondheidsachterstanden is complex en vraagt om een integrale aanpak. In 2016 is in 4 lage SES wijken in Maastricht de integrale community aanpak Blauwe Zorg in de Wijk gestart, gebaseerd op het gedachtegoed van Positieve Gezondheid. De aanpak richt zich op:

  • Burgers
  • Professionals
  • Ontschotten van financiering

Met vragenlijsten, interviews, observaties en burgerwetenschappers zijn de effecten en de implementatie van de aanpak geëvalueerd. Met CBS data zijn de zorgkosten (2017-2019) bepaald.

Samenwerkingspartners

Blauwe zorg in de wijk is gefinancierd door de Gemeente Maastricht, Zorgverzekeraar VGZ en de Provincie Limburg. Het dagelijks bestuur wordt aangevuld door Zorg in Ontwikkeling (ZIO), Envida en Levanto. De GGD Zuid Limburg, Xonar, Radar en Burgerkracht zijn partijen die ook nauw zijn betrokken bij de implementatie van de pilot als ambassadeurs. Een deel van deze zorgaanbieders en financiers in de regio Maastricht- Heuvelland hebben zich in 2013 verenigd in het bestuurlijk netwerk ‘Alliantie Santé’.

Resultaten

Er is geen significant effect op de kwaliteit van leven. De totale zorgkosten in de wijken stijgen niet minder hard, ondanks kleine verschuivingen in kosten tussen de zorgverzekeraar en de gemeente. Samenwerken loont om de zorg anders/beter in te richten. Positieve Gezondheid is geborgd en de aanpak wordt verspreid in de regio.

Samenvatting van de aanvraag

Population ageing, high healthcare expenditures and transitions in the social domain pose challenges to Dutch healthcare and local authorities in terms of sustainability of care. This especially applies to the south of Limburg. The region Maastricht-Heuvelland is selected as one of the national pioneer sites, called Blue Care, to experiment with population management in simultaneously improving (experienced) quality of care, improving population’s health, and reducing per capita costs. Relevant authorities (e.g., municipality of Maastricht, health insurers VGZ and CZ, and various (local) partners involved in care and public health) joined forces at the strategical, tactical and operational level to improve quality of life of citizens in four deprived neighbourhoods of Maastricht characterised by among others low socioeconomic status, poor health, and high health care spending. An integrated community approach is set up, based on the working principles of positive health, to improve health-related quality of life of deprived/vulnerable citizens living in low socioeconomic neighbourhoods of Maastricht, thereby reducing health inequities. It aims to: - change citizen’s attitude, self-efficacy and behaviour to give meaning to their own lives and stimulate societal participation; - enable and support the professional freedom of health and social service providers to organise (preventive) support in the main interest of the individual citizen, above and beyond organisational interests or financial reason; - remove financial obstacles by virtually combining budgets for care financed in the social and public health domain (municipality of Maastricht) and the health insurer financed care (health insurance companies VGZ and CZ) on a population level, using the reimbursements of 2015 in these domains as an upper limit. Individual needs of citizens are the starting point for offering care or services instead of using available (partial) budgets as constraint. Coordination teams (consisting of GP, community nurse, representative of the local authority, community member, member for management/monitoring information) at the neighbourhood level decide on the allocation of integrated health and social services according to the citizen's needs and monitor the reimbursements to be assigned. Outcomes of the integrated community approach are examined in terms of the effectiveness on the quality of life of citizens and the influence on reimbursements. A prospective, quasi-experimental study within a natural experiment is designed, comparing a sample derived from ‘‘intervention’’ neighbourhoods that implement the integrated community approach with a matched sample derived from ‘‘comparison’’ neighbourhoods who do not implement this approach. Eligible to participate are citizens aged 12 years and older within the participating neighbourhoods, with a required sample size of 1,656 (828 per study group). Primary outcome measure is health-related quality of life, complemented with measures of population health (e.g., resilience, mental health), and experienced quality of care. Outcomes are assessed using questionnaires at baseline (T0) and at three-year follow-up (T3) in both study groups. Changes in total reimbursements at the population level in both study groups are assessed at T0 and T3 and compared with the national trend. Substitution in different type of costs is determined using per capita costs at T3 for clients enrolled in the integrated community approach. Also the process of embedding the integrated community approach in the pilot neighbourhoods will be examined. Quantitative (descriptive) and qualitative data methods are applied between T0 and T3, such as observations during citizen panel meetings; document review of 10 client files; semi-structured interviews and focus group interviews with members of coordination teams and other professionals involved; participatory observations during meetings of coordination teams and at the directors level. In de proeftuin Blauwe Zorg wordt een pilot uitgevoerd in 4 buurten in Maastricht met een lage sociaaleconomische status. Doel is het in gang zetten van een beweging vanuit positieve gezondheid waardoor levenskwaliteit van bewoners verbetert en gezondheidsverschillen verkleinen. De aanpak richt zich op burgers betekenis laten geven aan hun leven; stimuleren professionele vrijheid zorg/dienstverleners; samenbrengen budgetten sociale domein en zorgverzekeraar op buurtniveau. Een coördinatieteam wijst zorg/voorzieningen toe op basis van behoeften van de burger en monitort de vergoedingen. Uitkomsten van deze aanpak worden gemeten op T0 en FU na 3 jaar in termen van effecten op kwaliteit van leven (m.b.v. vragenlijst) en invloed op de vergoedingen binnen het sociale domein en Zvw gefinancieerde zorg door zorgverzekeraars CZ en VGZ. Het proces van het inbedden van de aanpak in de buurten wordt geëvalueerd via kwalitatieve methoden (oa interviews, participerende observaties, zorgdossiers).
