Nederland neemt deel aan de Joint Action ADVANTAGE. Dit is een internationaal samenwerkingsverband, dat gericht is op de preventie van  kwetsbaarheid bij ouderen (frailty). Hiermee doelen we op de opeenstapeling van lichamelijke, psychische en/of sociale tekorten in het functioneren die de kans vergroten op negatieve gezondheidsuitkomsten zoals functiebeperkingen, ziekenhuisopname, of overlijden. 

Doel

Het doel van deze Joint Action is te leren van wat Europese landen doen aan preventie van kwetsbaarheid en zorg voor kwetsbare oudere mensen. Wij delen de Nederlandse kennis en cijfers over kwetsbaarheid bij oudere mensen met de andere deelnemende Europese landen.  Uiteindelijk doel is te komen tot een aanpak voor preventie van kwetsbaarheid bij ouderen, en meer inzicht te krijgen in het ontstaan, beloop en de gevolgen op de lange termijn. Dit draagt bij aan het langer gezond blijven van ouderen, waardoor ze langer zelfredzaam blijven. In totaal doen 24 landen mee aan deze Joint Action. Het RIVM neemt namens Nederland deel.

Samenvatting van de aanvraag

‘Managing Frailty. A comprehensive approach to promote a disability-free advanced age in Europe: the ADVANTAGE initiative — ADVANTAGE’ : application for participation of RIVM Introduction and aims The general aim and main outcome of the Joint Action Advantage (JA Advantage) is to identify core components of frailty, its natural course and its management. Based on this, a common European model for prevention and management of frailty will be developed. This should guide changes in the organization and implementation of prevention and care in the health and social systems, allowing each Member State (MS) to use a common framework for frailty tailored to their own context and care system. Contribution RIVM and workplan In this 3-year Joint Action RIVM will focus on Work Package 4 (WP-4): ‘Knowing frailty at individual level’ and contribute to Work Package 2 (WP-2) ‘Communication, Awareness and Dissemination’. The objectives of WP-4 are: - to standardize the assessment of frailty at an individual level by developing a common methodology to define, screen and diagnose this condition; - to identify clinical aspects of frailty (diseases, conditions and functional parameters), to assess patterns of multimorbidity and their interactions in the development and progression of frailty and to assess the impact of frailty in the outcome of chronic diseases and long term care conditions. To contribute to WP-4 RIVM will provide an overview of the definitions and conceptual models of frailty, and of the screening and diagnostic tools which have been developed and are currently used in the Netherlands. In addition, RIVM will summarize the main findings of Dutch studies of the association between frailty and the presence of (multiple) chronic diseases. RIVM will establish contact between the task leaders and PI’s of individual Dutch studies to see if they are interested to participate in the secondary analyses of already existing datasets to assess the association between frailty, chronic disease and disability. Link with NPO and Beter Oud We will seek input from experts of the Dutch National Care for the Elderly Programme (NPO) and the Beter Oud consortium (the thematic team ‘Early recognition’ from Beter Oud), to ensure that relevant knowledge within the NPO and Beter Oud will find its way into the JA Advantage and vice versa. To this end RIVM will establish a scientific advisory group which will be consulted for every task in the WP’s. The scientific advisory group and the thematic team ‘Early recognition’ from Beter Oud will be consulted to identify relevant NPO projects and other Dutch projects/initiatives to be included in the overviews. The overviews will be emailed to the scientific advisory group and the ‘Early recognition‘ team for comments before submission to the task leaders of WP-4. We will also make use of the Beter Oud website and Beter Oud newsletters to disseminate the progress and results of the JA Advantage. Timeline The involvement of RIVM in WP2 is more or less a continuous effort over the three years of the JA. For WP-4, the first draft of the literature synthesis and definition of frailty is planned to be circulated in May/June 2017. The final report from this first task is planned to be finished in December 2017. The RIVM activities for the other tasks will start in the second half of 2017, extending into the second and third year of the JA.
