CAVIA: Cerebral Amyloid Angiopathy: Vascular Imaging and fluid markers of Amyloid deposition.

Projectomschrijving

Een gezond stelsel van bloedvaten is van belang voor het goed functioneren van onze hersenen. Cerebrale Amyloïd Angiopathie (CAA) is een aandoening die zorgt voor de samenklontering van het schadelijke eiwit amyloïd-bèta in de hersenbloedvaten. Dit kan leiden tot kleine puntbloedingen, die het cognitief functioneren negatief beïnvloeden, of tot grotere, soms dodelijke, hersenbloedingen. CAA wordt ook vaak gezien in de hersenen van mensen met Alzheimer. Helaas is het nu nog niet goed mogelijk om CAA bij leven vast te stellen. In dit CAVIA project vonden we nieuwe signaalstoffen in het hersenvocht en bloed, waarmee we mogelijk de aanwezigheid van deze klontering kunnen aantonen. Tevens ontwikkelden we nieuwe MRI-technieken om CAA beter en vroeger te kunnen aantonen. Met de resultaten van dit project hebben we een eerste stap gezet in de ontwikkeling van methoden die vroeger en nauwkeuriger CAA aantonen. Deze ontwikkeling zal uiteindelijk bijdragen tot betere preventie en therapie van CAA.

Doel

Het doel van dit project is het ontwikkelen en valideren van methoden voor lichaamsvloeistoffen en neuroimaging om diagnostische hulpmiddelen te bieden aan artsen die het mogelijk maken om CAA tijdens het leven te detecteren, om de bijdrage van CAA aan cognitieve achteruitgang en dementie vast te stellen, en om potentiële toekomstige gepersonaliseerde therapie te vergemakkelijken.

Aanpak

In het onderzoek zoeken we naar nieuwe methoden om de stapeling van eiwitten in de bloedvaten van de hersenen aan te tonen. Dit doen we door op zoek te gaan naar “biomarkers” die het verband tussen eiwit stapeling en vermindering van de verstandelijke vermogens kunnen aantonen. Dit kunnen “alarmstoffen” zijn die zich in lichaamsvloeistoffen bevinden als dit eiwit zich gaat opstapelen, of “alarmsignalen” die deze stapeling zichtbaar maken op een MRI van de hersenen. Op basis van eerder onderzoek, gaan we gericht op zoek naar deze “alarmstoffen” en “alarmsignalen”.

Resultaten

  • Het project heeft geleid tot identificatie en validatie van nieuwe biomarkers voor CAA in verschillende domeinen (hersenvocht, bloed, hersenweefsel, neuroimaging) waarvoor nieuwe kwantitatieve immunoassays zijn ontwikkeld.
  • Er zijn nieuwe MRI sequenties ontwikkeld om de vasculaire functie in de hersenen te meten.
  • Er is een nieuw transgeen rat model ontwikkeld, met hierin verlaagde concentraties van het amyloid aangetoond, als functie van de leeftijd en progressie van CAA, lijkt een geschikt diermodel te zijn voor het bestuderen van CAA en voor de translatie van biomarker studies van mens naar dier en vice versa.
  • Er zijn innovatieve “targeted massa spectrometrie” methoden ontwikkeld voor het kwantificeren van potentiële biomarker eiwitten in hersenvocht.

Eindverslag

Stapeling van het zogenaamde “amyloïd β-eiwit” in bloedvaten van de hersenen (“Cerebral Amyloid Angiopathy”, CAA) is een belangrijke oorzaak is van dementie en hersenbloedingen bij ouderen. Deze eiwit stapeling komt voor bij een groot deel van de oudere bevolking en bij patiënten met de ziekte van Alzheimer. In het CAVIA project gingen we op zoek naar nieuwe methoden om CAA aan te tonen. In het project zijn de volgende resultaten behaald: Dit project heeft inderdaad geleid tot de identificatie van diverse nieuwe eiwit biomarkers in zowel hersenweefsel als hersenvocht, waarvoor nieuwe kwantitatieve immunoassays zijn ontwikkeld. We identificeerden norrin/NDP, ApoD en diverse andere eiwitten als mogelijke nieuwe biomarkers voor CAA. In hersenvocht van een nieuwe transgeen rat model werden verlaagde concentraties van het amyloid β40 aangetoond, als functie van de leeftijd en progressie van CAA. Hiermee lijkt dit een goed geschikt diermodel te zijn voor het bestuderen van CAA en voor de translatie van biomarker studies van mens naar dier en vice versa. Tevens werden innovatieve “targeted massa spectrometrie” methoden ontwikkeld voor het kwantificeren van potentiële biomarker eiwitten in hersenvocht. Concentraties van diverse amyloid β peptiden in bloed werden, met behulp van nieuwe assays, bepaald in twee grote populatie studies uitgevoerd in Rotterdam en Nijmegen. In deze studies is de relatie vastgesteld tussen bloed amyloid β peptiden en neuro-imaging biomarkers van CAA (kleine herseninfarcten en kleine bloedingen) alsmede relaties met cognitieve testen. Nieuwe MRI-sequenties zijn ontwikkeld om vasculaire functie in de hersenen te meten door het volgen van de respons van de bloedstroom en zuurstof in de hersenen in reactie op verhoogde CO2 in de inademingslucht. Deze metingen zijn toegepast in symptomatische HCHWA-D patiënten, waarbij een slechter functioneren van de hersenvaatjes is aangetoond. Tevens zijn nieuwe MRI-sequenties ontwikkeld die meerdere componenten van de hemodynamiek af beelden. Er is uitgebreid gesproken in focusgroepen met patiënten (of partners) met de ziekte van Alzheimer (via Alzheimer Nederland) en patiënten met CAA (via de Vereniging HCHWA-D en rechtstreeks via delegaties van patiënten met een hersenbloeding ten gevolge van CAA) over de verschillende scenarios en toepassingen van biomarkers. Het project heeft geresulteerd in een intensivering en bestendiging van privaat-publieke samenwerking, met name tussen het Radboudumc en ADx Neurosciences, en tussen het LUMC en Philips Healthcare. Tevens zijn er intensieve contacten met de Vereniging HCHWA-D opgebouwd. Samenvattend heeft het project geleid tot identificatie en validatie van nieuwe biomarkers voor CAA in verschillende domeinen (hersenvocht, bloed, hersenweefsel, neuroimaging). Als uitvloeisel van dit CAVIA zijn er diverse vervolgstudies gefinancierd. Het project heeft geleid tot een intensieve en productieve samenwerking tussen vier academische centra in Nederland (onder leiding van dr. M.M. Verbeek van het Radboudumc), twee buitenlandse onderzoekscentra en met twee private partners en patiëntorganisaties. Deze samenwerkingen worden ook na afrondng van het CAVIA project gecontinueerd.

In het derde jaar zijn de volgende resultaten behaald: - De biomarkers apoD, neuroleukin, HspB2 en PDGFRβ zijn uitgebreid bestudeerd op hun aanwezigheid in hersenvocht en bloed van een goed gekarakteriseerd cohort van controles en CAA patiënten. Diverse publicaties zijn in bewerking over deze resultaten. - Deze selectie van biomarkers wordt ook bestudeerd in hersenweefsel van overleden patiënten met CAA. NDP/Norrin, ApoD en C4B zijn specifiek aanwezig in CAA NDP/Norrin expressie is ook onderzocht in amyloid angiopathie bij prionziekten, angiopathy bij CARASAL en angiopathie bij CADASIL. Norrin blijkt een hele goede marker voor een angiopathie te zijn bij deze ziektebeelden en wordt niet in controles gezien. Verder onderzoek naar de waarde van deze marker in CSF/ bloed is dus belangrijk en wordt momenteel nader onderzocht in zowel het CAVIA project als in een VUmc project. - Met behulp van een nieuw ontwikkeld rat model van CAA werden de progressie van CAA ontwikkeling, en de daarmee gepaard gaande verandering van de biomarkers amyloid β40 en uPA in hersenvocht bestudeerd. - Nieuwe MRI-sequenties zijn ontwikkeld om vasculaire functie in de hersenen te meten door het volgen van de respons van de bloedstroom en zuurstof in de hersenen in reactie op verhoogde CO2 in de inademingslucht. Deze metingen zijn toegepast in symptomatische HCHWA-D patienten, waarbij een slechter functioneren van de hersenvaatjes is aangetoond. Publicaties hierover zijn in voorbereiding. - Concentraties van diverse amyloid beta peptiden werden, met behulp van nieuwe assays, bepaald in bloed in twee grote populatie studies in Rotterdam en Nijmegen. In deze studies is de relatie vastgesteld tussen bloed biomarkers en neuro-imaging biomarkers van CAA (kleine herseninfarcten en kleine bloedingen) alsmede het functioneren presteerden op cognitieve testen, met als doel om vast te stellen of er een relatie bestaat met CAA. Diverse publicaties zijn in bewerking over deze resultaten. - Er is uitgebreid gesproken in focusgroepen met patiënten (of partners) met de ziekte van Alzheimer (via Alzheimer Nederland) en patiënten met CAA (via de Vereniging HCHWA-D en rechtstreeks via delegaties van patiënten met een hersenbloeding ten gevolge van CAA) over de verschillende scenarios and toepassingen van biomarkers. De resultaten hiervan worden verwerkt in een publicatie.

Samenvatting van de aanvraag

Cerebrovascular disorders are a major cause of cognitive decline and dementia in the elderly. One of the most prevalent cerebrovascular diseases in the elderly is cerebral amyloid angiopathy (CAA), characterized by vascular deposition of amyloid β protein (AbP). CAA is increasingly recognized as an important cause of cognitive decline or dementia in old age. It has a high prevalence (10-50% of the general population), which increases with age. Although CAA is recognized as a separate disease entity, there is also a strong association between CAA and Alzheimer’s disease (AD), with an approximate 80% prevalence of CAA in AD patients. The presence of CAA worsens cognitive impairment in AD, independent of the presence of AD pathology. Formation of CAA is most likely caused by failure of the normal clearance of AbP from the brain. The normal transport of AbP out of the brain parenchyma, across the blood-brain barrier, towards the circulation seems to be progressively deficient during aging due to, e.g. reduced interaction with cellular receptors, or enhanced aggregation of AbP, eventually leading to CAA. There is increasing insight that the development of cognitive decline due to AD passes through a subclinical phase of 15-20 years, during which AbP proteins start to accumulate in the brain, but without overt clinical signs and symptoms. It is most likely that similar mechanisms occur in CAA; a long preclinical phase of gradual AbP accumulation in the cerebral vasculature precedes clinical symptoms associated with CAA, such as cognitive decline. Once clinical symptoms become evident, CAA is widespread in the brain, but at that time intervention is likely too late. Therefore, biomarkers of early CAA development are clearly needed for early diagnosis which will – in turn – allow for early intervention. An effective treatment for the impaired AbP clearance or CAA development is currently lacking. Since the circulation is a relatively accessible gateway to reach the cerebral vasculature, CAA is an attractive and feasible target for intervention strategies, not in the least since local disturbance of the blood-brain barrier at sites of CAA will allow for more optimal penetration of active compounds. Early detection of CAA is a prerequisite for effective intervention, but the development of specific biomarkers for CAA is unfortunately lagging behind. Therefore, currently, the definitive diagnosis of CAA can only be made postmortem. Sensitive and specific diagnostic tests that allow detecting CAA during life are dearly needed. Such tests will (1) allow to establish a diagnosis of CAA at a higher level of confidence than is currently possible; (2) increase our understanding of the pathophysiology and natural history of the disease, which would significantly increase the chances of identifying targets for effective treatment; (3) help to assess the efficacy of candidate treatments in trials; (4) once effective treatments are available, early-stage diagnostic tests would aid in widening the therapeutic window of the treatment; and (5) finally, these tests would help to increase the safety of anticoagulation therapy in the elderly which are contra-indicated in CAA. Several neuroimaging-based tests have been advocated for detection of CAA in vivo. The presence of lobar (micro) hemorrhages (a.k.a. microbleeds) on CT and MRI and limited subarachnoid blood collections or superficial siderosis can be suggestive – but not final – and probably late proof for the disease. Recent work by members of our project team strongly suggests that more specific diagnostic tests are within reach: (a) in cerebrospinal fluid (CSF) of CAA patients, an abnormal AbP40/AbP42 ratio is observed, distinguishing “pure” CAA from AD and supporting the concept of defining a potential CAA-specific AbP profile; (b) several AbP peptides and non-AbP proteins have been demonstrated (by in vitro/ex vivo studies) to be associated with CAA formation, but not with senile plaques, and may serve as candidate biomarkers for CAA; (c) MRI measures of hemodynamic response to neuronal activation are delayed and suppressed in CAA, supporting the concept that CAA can be diagnosed via MRI sequences detecting functional consequences of CAA. Both these CSF and MRI techniques have the intrinsic power to detect early changes in CAA development. In summary, the aim of this project is to develop and validate body fluid and neuroimaging methodologies to provide diagnostic tools to clinicians that will allow to detect CAA during life, to establish the contribution of CAA to cognitive decline and dementia, and to facilitate potential future personalized therapy.
