Sociaaleconomische verschillen in roken, voeding en lichaamsbeweging dragen in belangrijke
mate bij aan sociaaleconomische gezondheidsverschillen. Verschillen in gezond en ongezond
gedrag zijn echter niet het gevolg van vrije individuele keuzes, maar worden voor een groot deel
gevormd en beïnvloed door allerlei omgevingsfactoren. Ongezond gedrag wordt bijvoorbeeld sterk
beïnvloed door financiële stress, onafhankelijk van het inkomensniveau. Omgaan met financiële
stress vergt veel aandacht en energie, wat ten koste gaat van de inspanning die nodig is om
constant gezonde keuzes te maken. Daarnaast blijkt dat de sociaal-culturele omgeving sterk
bijdraagt aan sociaaleconomische verschillen in gezond gedrag. In de huidige maatschappij wordt
een gezonde leefstijl vaak door hogere sociaaleconomische groepen gebruikt als middel om
zichzelf te onderscheiden van anderen. Deze culturele betekenis van gezond en ongezond gedrag
draagt in sterke mate bij aan sociaaleconomische verschillen in voeding en beweging.

Sociaaleconomische verschillen in roken, voeding en lichaamsbeweging dragen in belangrijke mate bij aan sociaaleconomische gezondheidsverschillen. Verschillen in gezond en ongezond gedrag zijn echter niet het gevolg van vrije individuele keuzes, maar worden voor een groot deel gevormd en beïnvloed door allerlei omgevingsfactoren. Ongezond gedrag wordt bijvoorbeeld sterk beïnvloed door financiële stress, onafhankelijk van het inkomensniveau. Omgaan met financiële stress vergt veel aandacht en energie, wat ten koste gaat van de inspanning die nodig is om constant gezonde keuzes te maken. Door de toenemende vergrijzing, en omdat veel chronische aandoeningen op latere leeftijd optreden, kunnen effectieve preventiemaatregelen die die aandoeningen voorkómen, leiden tot belangrijke volksgezondheidswinst. Ongezond gedrag is een belangrijke risicofactor voor veel chronische aandoeningen en komt meer voor onder mensen uit lagere sociaaleconomische groepen. Om effectieve preventiemaatregelen te kunnen ontwikkelen juist voor die groepen, moeten de oorzaken duidelijk zijn waarom deze verschillen in ongezond gedrag bestaan. In dit onderzoek wordt bestudeerd of de leefomgeving een rol speelt bij sociaaleconomische verschillen in lichamelijke activiteit, voeding, roken en alcoholgebruik. Vooral wordt de rol van de sociale omgeving bestudeerd, zoals de grootte en samenstelling van sociale netwerken. Daarnaast worden individuele preferenties voor gezondheidsgedrag bestudeerd en hoe deze verschillen voor hogere en lagere sociaaleconomische groepen.

Om antwoord te geven op de onderzoeksvragen zijn in 2014 vragenlijsten afgenomen binnen de longitudinale GLOBE-studie, de enige grootschalige Nederlandse studie specifiek gericht op het verklaren van gezondheidsongelijkheden. Deze studie is in 1991 gestart met een dataverzameling onder de bevolking van Eindhoven en omstreken, met volgende dataverzamelingen in 1997, 2004, 2011 en nu ook 2014. Hierop volgend zullen ook interviews en discrete keuze experimenten worden afgenomen bij een deel van de onderzoeksgroep.

In de vragenlijsten is nadruk gelegd op het in kaart brengen van de sociale en culturele omgeving van de deelnemers. Er is data verzameld over belangrijke sociale netwerk kenmerken, sociale steun en sociale invloeden, sociaal en cultureel kapitaal en gezondheidsgedrag.

Samenvatting van de aanvraag

With health problems mainly occurring in older adults (i.e. individuals aged >50 years) and with increasingly ageing populations, effective health promotion strategies in older adults can contribute substantially to population health. Because unhealthy behaviours in older adults are unequally distributed over socioeconomic groups, strategies need to be successful in lower socioeconomic groups particularly. What causes socioeconomic inequalities in health behaviours in older adults however, is still poorly understood. This study aims at making a substantial contribution to the understanding of socioeconomic inequalities in four health-behaviours in older adults, through the integration of environmental and individual determinants of health-behaviours. Specific aims are to investigate 1) the growing role of health-behaviours for socioeconomic inequalities in cause-specific mortality, 2) the importance of socio-cultural, physical, and economic environmental factors for socioeconomic inequalities in health behaviours, thereby considering socio-cultural factors the most important missing link in current understanding of inequalities in health-behaviours, and 3) the importance of preferences for behavioural attributes (e.g. costs, convenience, social aspects, health consequences, bodily sensations) to the explanation of these inequalities. The study will be integrated in the longitudinal GLOBE study - the only large scale Dutch study specifically designed to study health inequalities. The study population mainly resides in the south-east of the Netherlands (i.e. Eindhoven and surrounding municipalities). The GLOBE study consists of a baseline measurement conducted in 1991, with subsequent waves of data collection in 1997, 2004 and 2011. We propose to conduct a postal survey in 2014, with subsequent interviews among a subsample of postal survey participants in 2015. For the first aim, health-behaviour measurements over a 14-year time period will be used to assess their role for explaining socioeconomic inequalities in health (i.e. cause-specific mortality), also when taking other possible explanatory factors into account. Cox proportional regression analysis with age as the time scale will be used to estimate the hazard ratios for the association between socioeconomic position and mortality, and the mediating role of repeatedly measured health-behaviours. For the second aim, information on a multi-layered set of elements of the socio-cultural environment will be used as reported in the postal survey of 2014, and, additionally, geographic information systems and Google street view will be used to measure the physical environment. In separate models for each of the four health-behaviours, we will test the association with indicators of socioeconomic position (SEP). Then, we examine which socio-cultural environmental factors are differentially distributed across SEP groups, and related to the health-behaviour, and to which extent these factors mediate the association between SEP and the health-behaviour. Specific pathways from socio-cultural factors to health-behaviours will be explored via structural equation models, based on information collected in interviews among participants of the postal survey in 2015. For the third aim, Discrete Choice Experiments (DCE) will be conducted at the end of every interview. A DCE can only be done for one specific health-behaviour per respondent, as it takes about 10-15 minutes to conduct the DCE. Therefore, DCE’s will be conducted with regard to one type of behaviour (physical activity, diet, smoking or alcohol) with n=250 respondents for each behaviour. The DCE observations are analysed by a mixed logit regression model to take heterogeneity as well as correlation between the choice task completed by each individual into account. Differences in preference structures between socioeconomic groups of respondents and relations with environmental exposures (as reported by participants in the 2014 survey) and psychosocial cognitions from the Theory of Planned Behaviour (as reported in the interview) will also be analysed. Finally, knowledge resulting from the project will be exchanged on a continuous base with relevant stakeholders, for instance via CEPHIR, the academic workplace collaboration between Erasmus MC and Health Municipality Rotterdam (GGD). Main entry-points (regarding the socio-cultural environment and preferences for behavioural attributes) for health promotion strategies aimed at reducing socioeconomic inequalities in health-behaviours among older adults will be discussed during a final conference.
