IPPON onderzoek: Development, effectiveness and implementation of an injury prevention program

Bij judo, de meest beoefende vechtsport in Nederland, komen blessures vaak voor. Het gaat vooral om blessures aan de knie, enkel, schouder en hand/vingers. In andere sporten als voetbal en volleybal zijn sportspecifieke oefenprogramma’s effectief gebleken om blessures te verminderen. In dit onderzoek wordt bekeken of een judospecifiek oefenprogramma ook blessures kan voorkomen. Dit gebeurd in drie stappen. Allereerst wordt het judospecifiek oefenprogramma ontwikkeld. Dit programma wordt vervolgens getest door 30 judoka. In de tweede stap wordt het effect van het oefenprogramma op het aantal blessures bij judoka onderzocht. Dit wordt gedaan door metingen bij twee groepen (totaal 14 judoscholen). De helft van de judoka zal het oefenprogramma uitvoeren, de andere helft niet. Tot slot wordt de landelijke implementatie van het oefenprogramma voorbereid.

De ontwikkeling en effectiviteit van een judo-specifiek blessurepreventieve warming-up

Ondanks de hoge blessureprevalentie in recreatieve judoka’s, bestaat er nog geen op wetenschappelijk bewijs gebaseerd preventie programma in judo. Hierdoor hebben we de IPPON interventie ontwikkeld. De IPPON interventie is ontwikkeld volgens de systematiek van de Knowledge Transfer Scheme (KTS) richtlijnen. Het team bestond uit eindgebruikers (judoka’s en trainers), medici (sportartsen en orthopedisch chirurgen) en specialisten op het gebied van blessurepreventie (bewegingswetenschappers). De IPPON interventie is een trainer-gesuperviseerde warming-up dat zich focust op de preventie van schouder-, knie- en enkelblessures. De warming-up bestaat uit 36 verschillende oefeningen verdeeld over 3 categorieën 1) flexibiliteit en behendigheid, 2) balans en coördinatie en 3) kracht en stabiliteit.

De IPPON interventie hebben we geëvalueerd op haar effectiviteit op de blessure prevalentie in vergelijking met de reguliere warming-up in judo. Dit hebben we getest in een gerandomiseerd en gecontroleerd onderzoek. Alle judoka’s vanaf 12 jaar konden meedoen met het onderzoek. Deze judoka’s werden per judoclub verdeeld in twee groepen: een groep die de door de trainer gesuperviseerde IPPON interventie uitvoerde en een controle groep die hun normale warming-up voor de training deden. We hebben gekeken naar hoeveel blessures er optreden in beide groepen.
In totaal deden er 371 judoka’s mee aan de studie. In de IPPON groep registreerde gemiddeld 23% van de judoka’s een blessure. In de controle groep meldde gemiddeld 28% van de judoka’s een blessure tijdens de studie. Na berekeningen bleek de kans dat judoka’s uit de IPPON groep een blessure melden via het systeem 18% lager was dan de judoka’s uit de controle groep. Daarnaast bleek dat zowel judoka’s als trainers de IPPON interventie een succes vonden. De judo-specifieke blessurepreventieve warming-up liet geen significant verschil zien in de blessurevermindering. De 18% lagere kans op blessures in combinatie met de positieve ervaringen van eindgebruikers, laten zien dat de IPPON interventie mogelijk wel geschikt is voor judo als alternatief voor de warming-up

De blessure aantallen in judoka's zijn hoog. Om het aantal blessures te verminderen is de IPPON interventie ontwikkeld door een team bestaande uit judo experts, (sport)artsen en bewegingswetenschappers. De IPPON interventie is een warming-up programma van 12 oefeningen gericht op de preventie van schouder, knie en enkel letsels. De oefeningen zijn verdeeld in 3 categorieën: flexibiliteit en behendigheid, balans en coördinatie en kracht en stabiliteit. De IPPON interventie is beschikbaar via instructie video's, een instructieboek en fotokaarten met beschrijvingen. Na de ontwikkeling van dit programma is de volgende stap om de effectiviteit van de IPPON interventie te evalueren. Zorgt de IPPON interventie daadwerkelijk voor een vermindering van het aantal judo blessures bij judoka's? Deelnemende judoclubs worden ingedeeld in de interventie of controle groep. De interventie groep gebruikt de IPPON interventie als warming-up gedurende 6 maanden. De controle groep doet de reguliere warming up. Elke 2 weken worden alle judoka's gevraagd of ze een blessure hebben opgelopen. Na 6 maanden worden de opgelopen blessures in de interventie- en de controlegroep met elkaar vergeleken. Als blijkt dat de IPPON interventie blessures in judo verminderd, dan volgt een internationale implementatie.

Samenvatting van de aanvraag

BACKGROUND With a top-3 position on the priority list of sports with the highest incidence rate of injuries (4.3 per 1,000 hours) and top-5 position on both absolute number of injuries (180,000) as well as injuries presenting at the emergency department (4,400), combat sports should be a priority for injury prevention interventions. For judo, the most practiced combat sports, a specific prevention program based on existing proven effective exercises has been developed by experts who are both strongly embedded in judo practice, and world leading experts in judo research: the Judo 9+. Although sport-specific neuromuscular training programs have been shown to effectively reduce injury rates in various sports, the effectiveness and implementation of the judo 9+ program has not been studied yet. OBJECTIVES Main research objectives were defined by Dutch judo experts, who actively contributed to this proposal: 1. Translation of the Judo 9+ exercise programme to the Dutch recreational judo practice: the NL-Judo9+ 2. Evaluation of the effectiveness of the NL-Judo9+ exercise programme on injury risk profiles and injury rates 3. Preparation for national implementation of the NL-Judo9+ in recreational judo (if proven effective). Action plan objective 1 The existing Judo9+ will be translated to the Dutch recreational judo setting, tested and evaluated among judo schools. Overall, 30 participants will be included.. Following completion of the Judo 9+ program, coaches and judoka will complete questionnaires on three categories: 1. Content 2. Complexity and 3. Time exposure of the exercises on the Likert scale (0-100). Modification of the program will be guided by the evaluation of these questionnaires. Action plan objective 2 NL Judo 9+ will be a two-arm, researcher-blinded cluster-randomized controlled trial with a 6 months follow-up. 14 clusters of judo schools will be included (n = approximately 420 judo players). They will be randomized to a group that will perform the NL-Judo 9+ program (intervention group) or a control group that will do their warming-up as usual. The primary outcome measure is the overall player injury prevalence measured with the Oslo Sports Trauma Research Centre questionnaire. Action plan objective 3 A Knowledge Transfer Group of judo experts, which includes a representative of the Judo Association, a judo sports physicians, judo teachers, trainers and (ex)-judoka will evaluate and discuss the effective elements of the program and possible adaptations. Subsequently an implementation plan will be defined in order to prepare for a national implementation. RELEVANCE This project fits well within the scope of the call Sports injury prevention 2018: (i) contributes to preventing injuries in recreational sport, (ii) an existing intervention will be translated and tested for its effectiveness and successful implementation in a Dutch setting, (iii) combat sports are among the top-5 sports with the highest number of injuries, (iv) people with an immigrant background and/or a low economic status have an above average representation in the target population, (v) relevant stakeholders within the practice field are involved from the start of the project, (vi) if proven effective, the prevention program will be implemented nationally.
