Prostaatkanker is de op één na meest voorkomende kanker bij mannen wereldwijd. Met de komst van nieuwe geneesmiddelen is de overleving sterk verbeterd.

Studie en resultaten

Het REFINE-project heeft laten zien dat de behandeling geoptimaliseerd kan worden door: inzet van biomarkers en alternatieve doseerstrategieën. Met circulerend tumor DNA (ctDNA) lijkt het mogelijk om vroegtijdig patiënten te identificeren die niet reageren op abirateron of enzalutamide in uitgezaaide castratie resistente prostaatkanker.
Het onderzoek laat tevens zien dat zowel voor abirateron als enzalutamide de dosis geoptimaliseerd kan worden. Voor abirateron zou afhankelijk van de setting (voor of na chemotherapie), een lagere dan wel hogere dosering beter zijn. De standaarddosering van enzalutamide lijkt momenteel te hoog.
Door combinatie van biomarkers en alternatieve doseerstrategieën kan de behandeling van prostaatkanker in de toekomst verder geoptimaliseerd worden.

 

Samenvatting bij start

Een groot aantal patiënten ontwikkelt op enig moment gemetastaseerd castratieresistent prostaatkanker (mCRPC). Het doel van het REFINE-project is om bij patiënten met gemetastaseerd castratieresistent prostaatkanker vast te stellen of het vroeg en gecombineerd meten van farmacokinetische (geneesmiddelblootstelling) en farmacodynamische (circulerend tumor m(icro)RNA) biomarkers gebruikt kan worden om patiënten te identificeren die adequaat behandeld worden, suboptimaal behandeld worden of initieel resistent zijn voor abiraterone acetaat of enzalutamide. Tevens wordt onderzocht of patiëntkarakteristieken van invloed zijn op de geneesmiddelblootstelling.

De resultaten moeten ertoe leiden dat patiënten in de nabije toekomst zullen worden behandeld in een individueel afgestemde optimale dosis en beschermd zullen worden tegen langdurige behandeling met ineffectieve en dure therapie.

Samenvatting van de aanvraag

The aim of the current project is to early identify those patients who are de novo resistant or sub-optimally benefit from novel anti-androgen therapy for metastatic castration resistant prostate cancer (mCRPC). Prostate cancer is the most prevalent cancer in male patients. Since 2011 four new treatment options have been added to the armamentarium; abiraterone acetate and enzalutamide next to Radium 223 and cabazitaxel. More treatment options for this prevalent tumor are a very encouraging development but simultaneously introduces new challenges - how to optimally use all of these treatment modalities. In this proposal we want to focus on the two novel anti-androgen drugs that are used for the treatment of chemo-naïve patients with mCRPC. The aim of this project is to determine whether timely assessment of drug exposure combined with early tumor response markers can be used to optimize and individualize the therapy with the novel anti-androgen drugs. The ultimate goal is to optimize anti-androgen treatment by early recognition of de novo resistance or suboptimal dosing in individual patients with mCRPC. Here fore, the current large gap between treatment onset and response evaluation needs to be narrowed, and a better understanding of the interaction between pharmacokinetic and pharmacodynamic-biomarkers is required.
