What is the clinical and cost effectiveness of endoscopic sinus surgery (ESS) in adult patients with chronic rhinosinusitis with nasal polyps?

Bij mensen met neuspoliepen waarbij ontstekingsremmende neusspray onvoldoende helpt, volgt vaak een aanvullende behandeling met systemische medicijnen of een operatie. Deze studie vergeleek het effect van deze behandelingen op gezondheid en kosten.

Na 1 jaar leek opereren de kwaliteit van leven iets meer te verbeteren dan alleen medicijnen. Patiënten die geopereerd waren hadden ook minder neusklachten zoals neusverstopping en gebruikten per patiënt per jaar 300mg prednison minder. Hieruit kon worden geconcludeerd dat opereren van neuspoliepen een goede aanvullende behandeling is. Opereren is duurder dan alleen medicijnen en bleek in het eerste jaar niet kosteneffectief. In Nederland vinden we de behandeling kosteneffectief als een extra jaar van volledige gezondheid maximaal €20.000,- extra kost. Een neusbijholtenoperatie kostte in het eerste jaar € 55.054 en € 111.812 extra vanuit respectievelijk gezondheidszorg perspectief (diagnostiek en behandeling van ziekte) en maatschappelijk perspectief (waarbij ook wordt gekeken naar kosten zoals productiviteitsverlies).

Samenvatting van de aanvraag

OBJECTIVES: Current practice variance in adult patients with chronic rhinosinusitis with nasal polyps (CRSwNP) is not efficient. Endoscopic sinus surgery (ESS) is the most common ENT operation in adults in the Netherlands. The objective of the present study is to investigate the clinical effectiveness and cost-effectiveness of two regular used strategies (“ESS in addition to drug treatment (usual care)” versus “drug treatment alone”) in adults with CRSwNP with regard to improvement in health related quality of life (HRQoL: SNOT-22, EQ-5D-5L) at 12 months follow-up. HYPOTHESIS: We hypothesize that “ESS in addition to drug treatment” has no-limited health benefit in terms of HRQoL compared to “drug treatment” alone in the treatment of CRSwNP. Further, we expect ESS only to be cost effective in a subgroup of patients. STUDY DESIGN: A prospective open multi-centre randomized controlled trial in 2 x 119 adult patients with CRSwNP selected for ESS according to current medical practice in the Netherlands. Patients will be randomized to either a) ESS in addition to drug treatment, or b) drug treatment alone. Total follow-up will be 24 months. STUDY POPULATION: Consecutive adult patients (>17 years) with CRSwNP, selected by their ENT surgeon as candidates for primary or revision surgery in general and university hospitals. Patients will be excluded if the operation involves septal surgery without ESS, polypectomy without ethmoidectomy, turbinate surgery, radical surgery like medial maxillectomy, Denker surgery or DRAF III, or surgery for mucoceles. All patients eligible for ESS, primary or revision, will be asked to participate in the study. Within the general population of CRSwNP patients we will take differences in age, gender, smoking, social status and other relevant (patient) characteristics into account. INTERVENTION: Patients will be randomly assigned to: a) a surgical strategy consisting of ESS in addition to drug treatment (surgery) or b) a medical strategy consisting of drug treatment (drug treatment) alone. In this study we mean by endoscopic sinus surgery the surgery as done regularly by otorhinolaryngologists in the Netherlands. Usually the extent of the surgery is tailored to the extent of the disease. Those assigned to the surgical strategy will be offered endoscopic sinus surgery within 6 weeks of randomisation. STANDARD INTERVENTION TO BE COMPARED TO: Treatment strategy b) drug treatment alone (drug treatment). Drug treatment can be any treatment that is normally given to treat CRSwNP. Those assigned to the drug treatment strategy will be seen by the otorhinolaryngologist within 6 weeks of randomisation to evaluate the need for additional drug treatment. OUTCOME MEASURES: The primary study parameter/endpoint is HRQoL measured by the SNOT-22 at 12 months follow-up. Secondary outcomes are generic and disease specific HRQoL, cost-effectiveness, endoscopic assessment of the nose, olfactory function, nasal obstruction, asthma control and adverse events. SAMPLE SIZE CALCULATION/DATA ANALYSIS: The main goal is to establish the putative superiority of ESS in addition to drug treatment over drug treatment alone. The minimal clinically important difference for SNOT-22 is 8,9 points, sd 20,0 (1). Using a 5% significance level and power of 90%, allowing for 10% loss to follow-up, we need to recruit a total of 238 patients. COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS: We hypothese that a more tailored approach for ESS is associated with less medical and indirect costs. Cost-effectiveness of the interventions is compared by assessing cost per QALY. In addition a budget impact analysis (BIA) will be performed to determine the potential financial impact of more tailored treatment for patients with CRSwNP on national total health care costs in the future. This will be based on (clinical) data that reflect the size and characteristics of the population, the current treatment mix, the effectiveness of ESS, and the resource use and costs for ESS (surgery, post-operative care, exacerbations and reinterventions). ANTICIPATED HEALTHCARE EFFICIENCY GAIN: An estimated decrease of 30-50% of performed ESS would save 15-26 million euros annually in the Netherlands. TIME SCHEDULE: 2014 – 2018. 2 months start-up, 18 months inclusion, 24 months follow-up and 4 months analysis and reporting.
