Neoadjuvant chemoradiotherapy plus standard surgery versus active surveillance for oesophageal cancer (SANO-trial)

Chemoradiotherapie gevolgd door chirurgie is de standaard behandeling voor patiënten met slokdarmkanker zonder uitzaaiingen. Bij 30% van de patiënten is de chemoradiotherapie zó effectief, dat geen tumor meer aantoonbaar is in het weefsel dat bij de operatie wordt verwijderd. Daarom rijst de vraag of een operatie in deze groep wel zinvol is, zeker gezien de risico’s van de operatie.
In de SANO-trial zal bij patiënten bij wie na chemoradiotherapie geen tumorrest meer kan worden aangetoond, ‘standaard opereren’ worden vergeleken met ‘actieve surveillance’. Patiënten in de actieve surveillance groep zullen streng worden gecontroleerd m.b.v. PET-CT en endoscopie. Deze patiënten worden alleen geopereerd als er na chemoradiotherapie nog tumorrest aantoonbaar is, zonder uitzaaiingen. Door beide groepen met elkaar te vergelijken kan worden beoordeeld of actieve surveillance leidt tot een vergelijkbare lange-termijn overleving, minder operaties, betere kwaliteit van leven en lagere kosten.

Samenvatting van de aanvraag

DOEL Het bepalen van de (kosten)effectiviteit van actieve surveillance na neoadjuvante chemoradiotherapie (nCRT) bij patiënten met een plaveiselcel- of adenocarcinoom van de slokdarm of slokdarm-maagovergang. HYPOTHESE Wij verwachten een non-inferieure overleving, 37% minder operaties, een betere kwaliteit van leven en betere (kosten)effectiviteit bij een strategie van actieve surveillance ten opzichte van standaard operatie. STUDIE OPZET Fase III multicentrische stepped wedge cluster gerandomiseerde trial. STUDIEPOPULATIE Operabele patiënten met een in opzet curatief te behandelen lokaal gevorderd slokdarmcarcinoom. INTERVENTIE Actieve surveillance na nCRT. Bij deze strategie ondergaan patiënten actieve surveillance na het einde van nCRT. Alleen patiënten met een sterke verdenking op of histologisch bewijs van locoregionaal recidief/residu zonder aanwijzingen voor afstandsmetastasen ondergaan een slokdarmresectie. GEBRUIKELIJKE ZORG Standaard slokdarmresectie 6-12 weken na het einde van nCRT. UITKOMSTMATEN De primaire uitkomstmaat is 2-jaars overall survival; secundaire uitkomstmaten zijn het percentage patiënten dat een operatie bespaard blijft, kwaliteit van leven, het percentage klinisch irresectable tumoren (cT4b), het percentage radicale resecties, postoperatieve complicaties, progression free survival, het percentage afstandsmetastasen en kosteneffectiviteit. SAMPLE SIZE Op basis van een geschatte gelijke 2-jaars overall survival van 67% in beide armen, een intra-correlatie coëfficiënt van 0.02 ter correctie van variatie tussen instituten, 12% drop-out, een percentage van 50% klinische complete responders na nCRT en de inclusie van 60 patiënten uit de preSANO-trial, bedraagt de totale verwachte (extra) inclusie 480 patiënten. KEA/BIA Er wordt een uitbreide kostenevaluatie verricht, zowel vanuit zorg- als sociaal perspectief, met als primaire uitkomstmaat overall survival en als secundaire uitkomstmaat kwaliteit van leven. Er zal een BIA worden verricht op basis van het Markov model. TIJDPAD De verwachte inclusieperiode is 36 maanden. Follow-up bedraagt 2 jaar. Patiënten blijven na de formele studieperiode gedurende minimaal 3 jaar in follow-up zodat langetermijnresultaten kunnen worden berekend. OBJECTIVE To assess the (cost-)effectiveness of active surveillance after neoadjuvant chemoradiotherapy (nCRT) for patients with squamous cell- or adenocarcinoma of the oesophagus or oesophago-gastric junction. HYPOTHESIS It is hypothesised that survival is non-inferior among patients who undergo active surveillance compared to those who undergo standard surgery, but that active surveillance results in 37% fewer operations and improved quality of life and is more cost-effective. STUDY DESIGN Phase III multi-centre stepped-wedge cluster randomised controlled trial. STUDY POPULATION Operable patients over 18 years of age with potentially curable locoregionally advanced squamous cell- or adenocarcinoma of the oesophagus or oesophago-gastric junction. INTERVENTION An active surveillance approach after completion of nCRT for carcinoma of the oesophagus. In this SANO (i.e. Surgery As Needed for Oesophageal cancer) approach, patients undergo active surveillance after completion of nCRT. Surgical resection is offered only to patients in whom a residual/recurrent disease is highly suspected or histologically proven, without distant dissemination. USUAL CARE Usual care consists of standard surgery 6-12 weeks after completion of nCRT. OUTCOME MEASURES The main study parameter is (2-year) overall survival; secondary endpoints include the percentage of patients who do not undergo surgery, quality of life, clinical irresectability (cT4b) rate, radical resection rate, postoperative complications, progression free survival, distant dissemination rate, and cost-effectiveness. SAMPLE SIZE Based on equal estimated 2-year overall survival rates of 67% in both trial arms, an intra-correlation coefficient of 0.02 to account for between-institution variation, a 12% drop-out, an expected percentage of 50% of all patients with a clinically complete response (cCR) after nCRT and inclusion of 60 patients with a cCR from the preSANO-trial, a total (additional) inclusion of 480 patients is required. CEA/BIA We will perform an economic evaluation from a health care and social perspective. The primary effect measure will be overall survival. The secondary outcome measures will be quality of life. We will perform a BIA. We will provide a valid framework (Markov model) that gives insight into budget consequences, by a range of predictions based on realistic estimates and scenarios of input parameter values. TIME SCHEDULE The inclusion period is estimated to last in total approximately 36 months. In order to determine the primary endpoint of 2-year overall survival, the study period will end 2 years after the last patient has been included. Patients will remain in follow-up for at least three more years after the end of the formal study to determine (ultra-)long-term survival.
