An analysis of influencing factors to the quality of care for patients with Borderline Personality Disorder (BPD): A pilot study for the successful implementation of guidelines for personality disorders in the Netherlands

Ter bepaling van de kloof tussen werkelijke zorg en optimale zorg bij de multidisciplinaire richtlijn Persoonlijkheidsstoornissen is onderzoek gedaan bij cliënten met een borderline persoonlijkheidsstoornis (BPS) bij tien gespecialiseerde GGz-instellingen. De werkelijke zorg is de behandeling die de cliënten in de verschillende instellingen ontvangen en de optimale zorg is de behandeling zoals aanbevolen in de richtlijn voor cliënten met een BPS. Met behulp van DBC gegevens is geanalyseerd hoeveel cliënten een optimale behandeling ontvingen. Het bleek dat nog geen kwart van de BPS cliënten de aanbevolen psychotherapie als eerste stap behandeling ontving. Met kwalitatief onderzoek zijn belemmeringen om de aanbevelingen uit de richtlijn in te voeren binnen de instelling in kaart gebracht. Capaciteitsproblemen en organisatorische problemen waren de belangrijkste belemmeringen voor toepassing van de belangrijkste aanbevelingen uit de richtlijn. Zorgpaden kunnen helpen bij de implementatie van de richtlijn en de efficiëntie en kwaliteit van de zorg verbeteren.

Het onderzoek betrof een kloofstudie tussen reguliere en optimale zorg bij de toepassing van de multidisciplinaire richtlijn persoonlijkheidsstoornissen, onderdeel borderline persoonlijkheidsstoornissen. De studie werd uitgevoerd in 10 grote en middelgrote ggz-instellingen in Nederland. Uit het onderzoek komt naar voren dat er geen grote kloof is wat betreft diagnostiek. De meeste cliënten ontvangen snel de juiste diagnose, dat is gemiddeld 85% van de onderzochte cliënten. Echter, er bestaat wel een kloof op het gebied van behandeling. Slechts een klein deel van de cliënten (22,5% gemiddeld)krijgt gelijk psychotherapie, terwijl psychotherapie in de richtlijn als voorkeurs behandeling wordt aanbevolen. De belangrijkste belemmeringen zijn een tekort aan psychotherapeuten en allerlei organisatorische problemen bij de instellingen.

Samenvatting van de aanvraag

Summary The high prevalence of Borderline Personality Disorder (BPD) both in the general population and in mental health care and forensic psychiatry populations make it a relevant disease to address. In addition, the cost of the disease to the individual and society and the many factors to consider when implementing new practice guidelines makes a problem analysis a logical and relevant step prior to a planned implementation study. Thus a knowledge of the existing diagnostic and treatment gap and the identification and understanding of the facilitators and barriers for the implementation of guidelines for BPD is crucial. The objectives of the study are: (1) Determine the gap between regular care and optimal care (diagnosis and treatment) for individuals with BPDisorder in specialized mental health organizations; (2) Identify and understand facilitators and barriers to change for the implementation of evidence-based guidelines for BPDisorder in specialized mental health organizations; (3) Develop and recommend guideline implementation strategies that are aimed at improving daily practice in specialized mental health organizations. The central questions of the study are: (1) To what extent are the key recommendations outlined in the guidelines for diagnosis and treatment of BPD applied in 10 specialized institutions for mental health? In other words how steep is the gap between optimal care and regular care for BPD in the Netherlands? (2) What are the barriers and facilitators to change for implementing the recommendations outlined in the guideline? Which of the identified factors can be influenced effectively by patients, professionals, and managers? (3) What are the main elements of potentially successful implementation strategies for participating specialized mental health institutions? The proposed study will be conducted using a mixed methods research design, in which qualitative and quantitative data are combined to answer the study questions. Ten specialized mental health institutes and patients over the age of 18 diagnosed with BPD (50 patients per institute for a total of 500 patients) is the population of interest for this study. The study consists of three sections: (1) a quantitative analysis of medical records derived from relevant patient databases; (2) a qualitative study using focus groups to identify and understand the influencing factors to be expected by relevant professionals, managers, and patients for the potentially successful implementation strategies; and (3) the design of implementation plans and a prediction of their consequent implementation outcomes and costs. The expected results of this study are: (1) A quantitative insight of the existing gap between regular care and optimal care for the diagnosis and treatment of BPD in specialized mental health organizations; (2) A qualitative insight of the facilitators and barriers for the implementation of the recommended guidelines in specialized mental health organizations, (3) Site-tailored implementation strategies for the participating specialized mental health organizations that can be used by the local teams trying to implement the guideline. These strategies are intended to lead to an implementation cost-effectiveness study. The time schedule of this study is as follows. The quantitative substudy will be 5 months, the qualitative substudy will be 4 months, the analysis and report will be two months, and the site-specific implementation strategies and design of an implementation study with a cost-effectiveness component will be one month.
