BIj een trombosebeen zit er een bloedpropje in de ader van het been. In ons onderzoek kijken we naar bloedpropjes die tot de lies zitten of hoger. De medicijnen die normaal worden toegediend lossen het bloedpropje niet op, dit doet het lichaam zelf. Maar soms gebeurt dit niet en kan het later zorgen voor klachten aan het been.
Met onze nieuwe behandeling gaan we het bloedpropje oplossen door in het stolsel oplosmiddelen te geven. Door het bloedpropje snel weg te halen, hopen we dat er minder schade optreedt. Na een jaar kijken we of de mensen last hebben van hun been. Dit vergelijken we met mensen die de nieuwe behandeling niet hebben gehad.

SUMMARY Objective: To assess whether ultrasound-accelerated catheter-directed thrombolysis (UACDT) combined with standard anticoagulant therapy in acute primary iliofemoral deep vein thrombosis (IFDVT) can significantly reduce postthrombotic syndrome (PTS) incidence after 1 year compared with standard anticoagulant therapy alone. The secondary objective is to investigate the effects on clot lysis, patency of the affected vein, valve function, recurrent thrombosis rate, quality of life (QOL) and medical costs. Study design: Prospective assessor-blinded randomized controlled multicenter intervention study with a follow-up of 1 year. Study population: All consecutive patients between 18-85 years with acute primary IFDVT presenting at the emergency or outpatient department of the participating centres. Intervention: UACDT combined with standard anticoagulant therapy (intervention group) versus standard anticoagulant therapy alone (control group). Outcome measures: primary; 1-year-PTS-incidence, secondary; clot lysis, patency of the involved vein segment, valve function, recurrent thrombo-embolism, QOL and medical costs. Sample size calculation: A sample size of 170 patients is required (85 per arm). Based on available data from literature a reduction in the incidence of PTS of 17% seems feasible after UACDT. Economic evaluation: Direct health care costs will be measured at t 0, 3, 6 and 12 months. For Patient Outcome Analysis, utilities will be calculated from the responses on the EQ-5D and SF-36 classification systems using the available multi-attribute utility functions. Time schedule: Activities to prepare for the start of the trial are already ongoing; inclusion: 0-10 months; follow-up: 10-22 months; analysis: 22-24 months. SAMENVATTING Doel: Onderzoeken of "ultrasound-accelerated" katheter-gestuurde trombolyse (UAKGT) in combinatie met standaard antistollingstherapie bij acute primaire iliofemorale diep veneuze trombose (IFDVT) de posttrombotisch syndroom (PTS) incidentie na 1 jaar significant kan verminderen vergeleken met antistollingstherapie alléén. Secundaire doelen zijn het bestuderen van de effecten op "clot lysis" (stolsel resolutie), “patency” (doorgankelijkheid van de betreffende vene), klepfunctie, recidiverende tromboses, kwaliteit van leven (KVL), en medische kosten. Studie opzet: Prospectieve, beoordelaar-geblindeerde gerandomiseerde multicenter interventie studie met 1 jaar follow-up. Studiepopulatie: Alle opeenvolgende patiënten tussen de 18-85 jaar met acute primaire IFDVT die zich presenteren op de eerste hulp of polikliniek van de deelnemende centra. Interventie: UAKGT met standaard antistollingstherapie (interventie groep) versus standaard antistollingstherapie alléén (controle groep). Uitkomstmaten: Primair: 1e-jaars-PTS incidentie; secundair: "clot lysis", ”patency”, klepfunctie, recidiverende trombo-embolieën, KVL en medische kosten. Sample size berekening: een sample size van 170 patiënten is vereist (85 per arm). Een reductie van 17% in PTS incidentie lijkt haalbaar, gezien de beschikbare data uit de literatuur na UACDT.
