Improving health of employees with a low socio-economic position (SEP): a citizen science study at the workplace

Doel

Veel werknemers met een lage sociaaleconomische positie (SEP) ervaren gezondheidsproblemen als gevolg van een ongezonde leefstijl en fysiek zwaar werk. Gezondheidsbevorderende interventies bereiken deze werknemers vaak niet doordat interventies niet goed aansluiten bij de mogelijkheden en behoeftes van deze werknemers. Volgens de sectorinstituten en aangesloten bedrijven zijn er interventies nodig die zijn afgestemd op de situatie van deze werknemers. Citizen science is een 'bottom-up' aanpak waarvan is aangetoond dat die de leefstijl en gedrag van mensen met een lage SEP in achtergestelde wijken kan verbeteren. Om de gezondheidsinterventies beter te laten aansluiten bij de doelgroep is de Citizen Science aanpak gebruikt.

Aanpak/werkwijze

De Citizen Science aanpak is uitgevoerd in een haven- en bouwbedrijf en geëvalueerd. Met deze aanpak zijn werknemers met een lage SEP en andere relevante stakeholders (zoals de werkgever) nauw betrokken bij alle stappen van onderzoek, zoals het ontwikkelen en implementeren en uitvoeren van gezondheidsinterventies.

Resultaten

Uit gesprekken met werknemers, management, andere stakeholders en een pilot test, kwamen 3 basiselementen voor de toepassing van Citizen Science op de werkvloer naar voren: kennis en vaardigheden, sociale steun en cultuur binnen het bedrijf en bewustzijn over gezondheid. Uit vragenlijsten, interviews, dagboekjes en een poll kan geconcludeerd worden dat de Citizen Science aanpak in een bedrijfssetting in principe toepasbaar is en kan leiden tot een passend programma. Werknemers zijn positief over de aanpak en de mate van betrokkenheid. Blijvende aandacht voor betrokkenheid en een veilige cultuur zijn aandachtspunten.

Producten

Met dit onderzoek is een stappenplan ontwikkeld waarmee werkgevers met Citizen Science aan de slag kunnen om een passende gezondheidsinterventie op te stellen. Daarnaast zijn de onderzoeksresultaten verwerkt in wetenschappelijke publicaties en een presentatie.

Samenvatting van de aanvraag

PROBLEM In the agricultural and construction sector many employees with a low socio-economic position (SEP) experience health problems due to unhealthy lifestyle behaviors and physically demanding work. Health promotion interventions often do not reach low SEP employees due to a poor fit to their specific needs and possibilities. The sector institutes and affiliated companies of the agricultural and construction sector have expressed the need for interventions tailored to the needs of low SEP employees. CITIZEN SCIENCE The citizen science approach is a ‘bottom-up’ approach that has been shown to be acceptable and feasible to facilitate healthy lifestyle, and to improve health literacy and behavior of low SEP adults in disadvantaged neighborhoods. Thus, this is a promising approach to reach and involve low SEP employees in the tailoring and implementation of workplace health promotion interventions. In this proposal, employees are citizen scientists, which implies that they are not the investigated OBJECTS but are SUBJECTS and thereby have control about the decision making process and have co-ownership of the project. This leads to a higher chance of accepting the results and actual complying with the tailored health promotion intervention. The employer and sector organizations that arrange occupational health care will be involved in all stages of the study, and thereby it produces a ‘socially robust’ tailored intervention supported by all persons involved. PURPOSE The first aim is to optimize the bottom-up approach ‘citizen science’ to the workplace for low SEP employees in the agricultural and construction industry. The second aim is to conduct the citizen science approach and implement the resulting health promotion interventions at the workplace. Finally, feasibility of the citizen science approach and implementation of health promotion interventions will be evaluated. METHOD STUDY POPULATION Low SEP employees, their employers, sector organizations. STUDY 1: OPTIMIZATION OF THE CITIZEN SCIENCE APPROACH TO THE WORKPLACE The citizen science approach has been proven feasible for low SEP adults living in disadvantaged neighborhoods. However, it is unknown which elements of the citizen science are appropriate for the workplace. Therefore, we will perform a series of interviews and pilot studies to optimize the citizen science approach to the workplace. STUDY 2: CONDUCTING CITIZEN SCIENCE APPROACH AND IMPLEMENTATION OF TAILORED HEALTH PROMOTION INTERVENTION Based on the outcomes of study 1, the citizen science approach will be optimized to the workplace. We expect that it will exist of the following four phases: 1) community meetings with low SEP employees to determine knowledge and skills regarding health and lifestyle; 2) workshops for low SEP employees to improve their knowledge and skills regarding health and lifestyle and educate them to take part in the citizen science approach. One workshop before phase 3 and one during phase 3; 3) determination of potential ways to improve lifestyle as well as barriers and facilitating factors at work for a healthy lifestyle by low SEP employees themselves; and 4) decision making by all stakeholders with regard to selecting health promotion interventions. Thereafter, implementation of tailored health promotion interventions. STUDY 3: EVALUATION OF CITIZEN SCIENCE APPROACH AND TAILORED HEALTH PROMOTION INTERVENTIONS The framework of Nielsen and Randall will be used to evaluate feasibility of the citizen science approach at the workplace and implementation of tailored health promotion interventions. We will describe the failure and success factors as well as what works, for whom, in what respects, to what extent, in what contexts, and how. We will end up with a user manual of the citizen science approach and a roadmap to use the citizen science approach in other occupational settings. RELEVANCE Potential benefits include improved lifestyle and health of low SEP employees, thereby indirect contributing to reducing socioeconomic health inequalities. It also yields benefits for employers and society, e.g. by reduced productivity loss and health care costs. IMPLEMENTATION AND KNOWLEDGE DISSEMINATION Activities that contribute to knowledge dissemination include 1) presentations, workshops, and news releases for a broad audience; 2) news releases via flyers and existing communication channels (e.g. social media, app, website) of involved stakeholders; 3) invitational conference; 4) scientific articles; and 5) presentations at national (e.g. “BG dagen”) and international scientific congresses. Implementation will be ensured by providing a step-by-step roadmap to conduct the citizen science approach based on the process evaluation and interactive workshops between researchers and people from practice. Implementation of health promotion interventions will be realized by making use of the existing structures of the sector institutes, such as the periodic occupational health assessment.
