Plusoptix: net zo effectief als het standaard oogonderzoek in de JGZ?
Vraagstuk
Om bij kinderen oogafwijkingen als een lui oog op te sporen, voert de jeugdgezondheidszorg (JGZ) een standaard oogonderzoek uit. Op de leeftijd van 3-6 jaar gebeurt dat onder andere via een kaart met plaatjes of symbolen. Het oogonderzoek kan ook met het autorefractie-apparaat Plusoptix, waarbij een soort foto van de ogen wordt gemaakt. Bij afwijkingen volgt uitgebreid onderzoek in het ziekenhuis. Werkt dit net zo goed?
Onderzoek
In dit project zijn de uitkomsten en kosten van beide manieren van het testen op oogafwijkingen vergeleken. De resultaten kunnen helpen het oogonderzoek bij de JGZ te verbeteren wat betreft kwaliteit en kosten.
Uitkomst
De resultaten hingen af van de leeftijd. Voor 3-jarigen is alleen het onderzoek met de Plusoptix beter, terwijl de kaart met symbolen op 5 en 6 jaar het beste alternatief lijkt. De Plusoptix-test duurt bij alle leeftijden meer dan de helft korter. Met beide testen lukte het niet om alle kinderen met een lui oog op te sporen. JGZ-medewerkers en ouders vonden de autorefractie-test goed uitvoerbaar en acceptabel.

BACKGROUND AND RELEVANCE The primary goal of vision screening in young children is the detection of amblyopia, also referred to as “lazy eye”, and risk factors for development of amblyopia requiring treatment. Amblyopia is a neural deficit in vision that is estimated to affect up to 3.4% of children. Child vision screening, a task of Preventive Youth Health Care (PYHC) in the Netherlands, reduces the prevalence of amblyopia. Current Dutch vision screening tests are time consuming, difficult to administer in young children and probably of low prognostic value. Instrument-based vision screening (e.g. plusoptiX) on the other hand is quick, and requires minimal cooperation of the child and could therefore be more cost-effective. PlusoptiX may also help to improve acceptance of the need for clinical consultation among parents, since its measurements are computer-aided and therefore could be perceived as more objective as compared to traditional tests. Several studies suggest that photoscreening using the plusoptiX may be a useful strategy to detect amblyopia in young children. However, to date, none have studied the validity using the plusoptiX in the Dutch population of children aged 3-6 years old, during regular PYHC practice, and compared study outcomes with current practices. AIM The aim of the current research project is to establish whether the current Dutch vision screening in children aged 3-6 years old in PYHC can be improved by using plusoptiX, with regard to costs and screening performance. We will also compare screening performances in specific subgroups of children (younger children, children with limited intellectual functioning and/or poor fluency in the Dutch language, high-risk groups for amblyopia). A secondary aim is to optimize plusoptiX referral criteria for the Dutch PYHC-setting. METHOD In order to allow PYHC managers and decision makers to make an informed decision on implementing optimal vision screening we will perform a validity study, a clinical study, and focus groups, in order to analyze costs and effects and assess the feasibility and acceptability of the plusoptiX in daily PYHC practice among professionals and parents, as compared to current vision screening. EXPECTED RESULTS If our results show that plusoptiX is more cost-effective compared to current vision screening, implementation of plusoptiX could lead to considerable cost reductions in Dutch PYHC, general health care or both. The results of this project will be included in the update of the Vision Guideline for PYHC, which is scheduled to take place in 2016-2018, thus enhancing implementation of the results. PROJECT GROUP Our project group has ample experience with cost-effectiveness research in PYHC. The project will be coordinated by TNO and will further consist of PYHC practice organizations Jong Florence and GGD Den Haag, and experts on vision screening and diagnosis (MCH Den Haag; Kind en Gezin Vlaanderen) and on cost-effectiveness in health care (LUMC).
