Vraagstuk

OCTN2-deficiëntie (OCTN2) is een erfelijke stofwisselingsziekte die leidt tot een laag carnitine in het bloed. Hierdoor kunnen onder andere lage bloedsuikers en hartproblemen optreden, waar patiënten aan kunnen overlijden. Behandeling bestaat uit het geven van carnitine. OCTN2 is sinds de uitbreiding van hielprikscreening (2007) een toevalsbevinding. Carnitine wordt via de placenta van de moeder aan haar kind gegeven. Als een baby een lage carnitine concentratie in het bloed heeft kan dit ook veroorzaakt worden door een lage carnitine bij de moeder bijvoorbeeld omdat zij OCTN2 heeft. Er bestaat ook een kans dat door de huidige manier van screenen baby’s met OCTN2 gemist worden en zijn overleden.

Onderzoek en verwachte uitkomst

Omdat veel mensen met OCTN2 nooit klachten ontwikkelen is het niet duidelijk of iedereen die opgespoord wordt via hielprikscreening ook behandeld moet worden.
Is het nuttig en haalbaar om OCTN2 screening op te nemen in de hielprikscreening? Wij gaan met dit onderzoek deze vragen beantwoorden.

Samenvatting van de aanvraag

Introduction Newborn screening is widely regarded as a successful public health initiative that leads to improved treatment and disease outcomes. With this in mind the “Gezondheidsraad” in 2015 wrote their advice to include OCTN2 deficiency in the Dutch newborn screening program. New insights since then warrant re-evaluation of the suitability of this disorder, especially concerning incidence, genotype and clinical presentation in the Netherlands. These are currently all unknown and therefore it is not possible to predict improvement of outcome of individuals with OCTN2 deficiency. Currently, OCTN2 deficiency is an unintended finding of the national newborn screening program. Since 2007 seven disorders included in the screening program are detected by specific acylcarnitine profiles which are measured by tandem mass spectrometry (MS/MS). Carnitine depletion hinders the acylcarnitines formation. When carnitine concentrations (meaning free carnitine expressed as C0) are below 5 micromol/l, acylcarnitine profiles cannot be evaluated reliably and these disorders can theoretically be missed. Information on blood free carnitine levels is therefore necessary to ensure reliable screening. Individuals with OCTN2 deficiency (synonyms primary carnitine deficiency, carnitine transporter disorder (CTD), carnitine uptake defect CUD) lose carnitine in urine and as a consequence have low carnitine levels, detectable in dried blood spots (DBS). Because of the transplacental transport of free carnitine from mother to infant the carnitine concentrations of a newborn reflect the carnitine status of the mother. Low carnitine levels in the DBS of a newborn in the newborn screening program may indentify OCTN2 deficiency in the mother, instead of in the newborn. Whether these mothers should be considered patients at risk for complications of their OCTN2 deficiency, even though they are completely asymptomatic, and thus require life long treatment with carnitine, remains yet to be determined. On the other hand infants with OCTN2 deficiency might be missed due to a high carntine concentration in their bloodspot. Aim • To investigate whether addition of OCTN2 deficiency to the newborn screening program of the Netherlands is useful and feasible. When OCTN2 deficiency does not meet the Wilson and Junger criteria for population screening, and free carnitine remains crucial in the newborn screening program • To define a strategy has how to perform the diagnostic approach of newborns with an abnormal screening result with a low carnitine concentration. Method Comparison of clinical characteristics, genetic and biochemical studies of OCTN2 deficient patients identified born before 2007 and diagnosed on clinical signs symptoms, individuals diagnosed by NBS with OCTN2 deficiency and individuals with OCTN2 deficiency missed by NBS.
