SMARTsize@Work: A combination of a weight management program and nudging in the company cafeteria

Doel
Overgewicht en obesitas zijn een belangrijke gezondheidsbedreiging. Interventies die gewichtstoename voorkomen en die de prevalentie van overgewicht en obesitas reduceren zijn dringend nodig. De prevalentie van overgewicht is hoger bij mensen met een lage sociaaleconomie positie (SEP). Werkgevers zien steeds meer de noodzaak om hun werknemers gezondheidsbevorderende interventies aan te bieden. In dit project werden twee interventies doorontwikkeld: 1) een interventie die gericht is op duurzaam gewichtsmanagement (SMARTsize) en 2) een interventie die bestaat uit nudge-strategieën om in het bedrijfsrestaurant de gezonde keuze de meest aantrekkelijke en automatische keuze te maken (het Bedrijfsrestaurant 2.0). Het eerste onderdeel was een programma waarin werknemers met hulp van een diëtist gezonde eet-en beweeggewoontes aanleren en afvallen om tot een gezond gewicht te komen. Het tweede onderdeel was een aanpassing van het bedrijfsrestaurant waardoor de gezonde keuze de automatische en aantrekkelijke keuze wordt.
Aanpak/werkwijze
Het project combineerde een participatieve aanpak met een evidence-based benadering. Het project bestond uit 4 fasen: 1) Probleemanalyse, 2) Interventie-doorontwikkeling, 3) Implementatie en procesevaluatie, en 4) Valorisatie. In deze fasen werd een mix van kwalitatieve en kwantitatieve onderzoeksdesigns gebruikt. Eerst werden vragenlijsten en interviews afgenomen en focusgroepen georganiseerd. Vervolgens werden de bestaande interventies doorontwikkeld tot SMARTsize@Work op basis van de uitkomsten van deze verkennende studies. De implementatiestudie zou eigenlijk plaatsvinden op 10 werkvloeren maar door de corona-crisis was dit niet mogelijk. In plaats daarvan is de SMARTsize interventie geïmplementeerd binnen diëtistenpraktijken, vergezeld van een evaluatie.
Resultaten
Het project heeft geresulteerd in de doorontwikkeling van twee bestaande interventies tot de SMARTsize@Work interventie. Daarnaast hebben de verkennende studies geresulteerd in deze uitkomsten:
Studie 1:
Voedsel- en gezondheidsvaardigheden hangen met elkaar samen, maar zijn niet hetzelfde. Het verbeteren ervan vergt aandacht op beide en moet worden gericht op zowel volwassenen met een laag als gemiddeld opleidingsniveau.
Studie 2:
De implementatie van leefstijlprogramma’s voor praktisch opgeleide werknemers moet aangepast worden aan de werkomstandigheden en taken van deze werknemers.
Studie 3:
Volgens stakeholders faciliteert een aansluitende communicatiestrategie de duurzame implementatie van gezondheidsinterventies voor praktisch opgeleide werknemers, maar kan de medewerking van werknemers een barrière zijn.
Producten
Diverse praktische tools en factsheets voor professionals zijn opgeleverd. Tevens is SMARTsize april 2022 her-erkend door het Loket Gezond Leven als effectieve interventie. Materialen zijn te vinden op www.smartsizeprofessionals.nl. Daarnaast zijn de resultaten van het onderzoek ontsloten in wetenschappelijke publicaties.

Samenvatting van de aanvraag
THE PURPOSE OF SMARTsize@Work The purpose of SMARTsize@Work is to facilitate attractive, feasible and evidence-based weight management among employees with a low socio-economic position (SEP) and to promote healthy eating habits. To acknowledge existing tools and the great importance of both a participatory and an evidence-based approach we will further develop two existing evidence-based intervention programs: SMARTsize and the Company Cafeteria 2.0. Implementation tools will be developed and the implementation process will be evaluated at 10 worksites. THE RELEVANCE OF SMARTSIZE@WORK Overweight and obesity is the 2nd most important health threat worldwide. Therefore, we urgently need programs to prevent weight gain and reduce overweight/obesity. In addition, the prevalence of overweight and obesity is especially high among people with low SEP. In contrast, health promotors are not able to reach people with low SEP sufficiently and offer them acceptable interventions. In addition, low health literacy may hamper employees in their ability to process and apply information to improve weight management and dietary behaviors. This results in socio-economic differences in health. Besides, employers feel an increasingly need to take care of the health of their employees by offering them health-promoting lifestyle interventions. Also, the public opinion expresses a desire for sustainable weight management interventions, rather than following a temporary (crash)-diet. To tackle the above described problem, we will further develop two existing interventions: 1) an intervention that targets sustainable weight management (SMARTsize), and 2) a nudge-intervention to promote the healthy choice in the company cafeteria (Company Cafeteria 2.0). These interventions are the result of a systematic development process where stakeholders were closely involved. In addition, there is substantial evidence for the feasibility and the effectiveness of both SMARTsize and Company Cafeteria 2.0 in real life settings. SMARTsize will be adapted especially for low SEP employees and suitable for employees with low health literacy. Implementation tools for both interventions will be developed. The final SMARTsize@Work program will be based on the needs, preferences, abilities and opportunities of the employees and other stakeholders within the companies. ACTION PLAN To meet the final purpose of our project, we will apply a stepwise approach, based on stakeholder opinions, theory and evidence. The project consists of 4 phases: 1) Problem analysis, 2) Intervention development, 3) Implementation & process evaluation, and 3) Valorization. Within these phases, a mix of qualitative and quantitative study designs and methods will be applied to answer the following research questions: RQ1: What do employees of low SEP and poor health literacy feel is needed to make SMARTsize attractive, achievable and acceptable and to bring it in line with their motivations? RQ 2: What is the best way to motivate employees to participate in SMARTsize? How should details be communicated to them, and by whom? Also, which member or members of their social circle should be involved? RQ3: What obstructive and facilitating factors are involved when implementing the combination of interventions(SMARTsize@Work) at the various levels and for the various stakeholders (intervention, user, implementer, organization and socio-political context in the organization)? RQ4: What implementation tools are needed to ensure that future implementation of SMARTsize@Work proceeds as effectively as possible? RQ5: How is the implementation of SMARTsize@Work (using the implementation tools that have been developed) evaluated at various levels (according to the seven process evaluation components described by Saunders et al., 2005)? RQ6: How effective is SMARTsize@Work among those participating in the weight management part of the intervention? THE FUTURE OF SMARTsize@Work During the project, SMARTsize@Work will be implemented at 10 worksites which are catered by Sodexo. Several actions will be done to disseminate knowledge gained during the project. More importantly, we will disseminate educational strategies to promote proper implementation of SMARTsize@Work and implementation tools at other worksites and by other catering companies.
