PerfectFit2.0 – optimising an effective intervention for workers in low socioeconomic position

Doel

Obesitas en ongezond gedrag komen relatief vaak voor bij werknemers in laag sociaaleconomische positie. Dit onderzoek evalueerde de effectiviteit en implementatie van de PerfectFit2.0 interventie.

Aanpak/werkwijze

PerfectFit2.0 bestaat uit een gezondheidsscreening en coaching met motiverende gespreksvoering. De effectiviteit en implementatie zijn geëvalueerd in een longitudinaal onderzoek met een beginmeting en een nameting na 6 maanden. Ook zijn werknemers geïnterviewd over gezondheidsbevordering op de werkplek. In totaal hebben 176 deelnemers meegedaan aan de beginmeting en de meting na 6 maanden. Werknemers die deelnamen aan coaching werden vergeleken met werknemers die hier niet aan deelnamen.

Resultaten

Van de werknemers met risicofactoren doet de meerderheid mee aan de coaching. Werknemers met coaching ondernamen 5x vaker acties ter verbetering van gezond gedrag en 11% meer werknemers stopten met roken. Er werden geen verbeteringen in gewicht of andere leefstijlkenmerken gevonden.

Samenvatting van de aanvraag

Obesity is 2.5 times more prevalent among workers in low socioeconomic position (SEP) than among workers in high SEP. Similar differences have been shown for unhealthy behaviours. Numerous health promotion interventions have been evaluated in the workplace setting, which seems to be a suitable setting to reach workers in low SEP easily. However, previous reviews suggest that these interventions are mainly effective for workers in high SEP. The results of the effective PerfectFit study are in line with this. Therefore, we will optimize the PerfectFit intervention for workers in low SEP. This study investigates the effectiveness and implementation of PerfectFit2.0, consisting of: - a health screening of cardiovascular risk profiling by biometric, lab and health behaviour measurement; - selection of workers with high cardiovascular risk; - tailored coaching using motivational interviewing; - personal health portal with video and audio messages. AIM PerfectFit2.0 aims to reduce the risk of diabetes and cardiovascular diseases by optimising the health promoting intervention for workers in low SEP and thereby contribute to reduce socioeconomic health inequalities. RESEARCH QUESTIONS Part 1: Development of PerfectFit2.0 1) What are the needs and preferences of workers in low SEP regarding engaging in healthy behaviour and participating in health promotion activities? [content of the intervention] 2) What are the barriers and facilitators of workers to participate in a workplace health promotion programme? [delivery of the intervention] Part 2: Implementation of PerfectFit2.0 3) Which individual and organisational factors contribute to actual participation and sustained participation in health promoting activities? 4) Which factors determine the quality of the implementation and what is the influence on the intervention effectiveness? Part 3: Evaluation of PerfectFit2.0 5) Does PerfectFit2.0 lead to healthier behaviours and better health after 6 and 12 months among workers in low SEP? 6) Does motivational interviewing increase actual participation in health promoting activities? METHODS DESIGN AND STUDY POPULATION The effectiveness and implementation will be evaluated in a longitudinal study with a baseline measurement, and follow-up measurements after 6 months and 12 months. In total, 250 workers of Erasmus MC, IJsselland hospital, and Scania will be included. INTERVENTION The following improvements will be used to optimize the PerfectFit intervention: - Tailored coaching by a lifestyle coach or occupational physician that is adapted to more suitable communication and learning styles of lower educated persons; - Using a combination of video and audio messages and text-based information to match health literacy; - Connection between personal health portal, workplace health promotion initiatives and health promotion activities in the neighbourhood. - Personal feedback on the quality of motivational interviewing of the coach throughout the study. The personal health portal will be extended with infographics presenting a menu showing the employee the opportunities in the workplace for healthy behaviour. As part of the development and prior to implementation, project groups withing the companies, consisting of workers, HR- and occupational health-professionals, and supervisors will discuss the communication and presentation of the PerfectFit2.0 intervention and the supporting personal health portal. Needs and preferences of the target group regarding communication and interventions will be explored using focus group interviews. EVALUATION Using a mixed-methods approach the effectiveness and implementation of PerfectFit2.0 will be investigated. IMPLEMENTATION The implementation process will be monitored using interviews with workers, the project leader within the participating companies, the occupational physician, and the lifestyle coach, and will focus on expectations, barriers and facilitators for these stakeholders. The implementation process (how is it developed) and the content (what is exactly implemented) will be distinguished. EFFECTIVENESS The primary outcome measures are: body mass index (BMI) and body weight. Secondary outcome measures are: physical activity, smoking cessation, reduced alcohol intake, cardiovascular risk, self-perceived health and stress level. To evaluate the effectiveness of the coaching and follow-up interventions, a state-of-the-art method for the evaluation of natural experiments will be used, namely by taking into account the propensity score. DISSEMINATION: Stakeholders for dissemination, such as Pharos and NVAB, participate in the impact group and engage as advisors for dissemination throughout the project. The following ideas for dissemination were suggested: - factsheets including infographics regarding success factors for implementation. - Social media: web page PerfectFit2.0 with short (animated) videos. - Documentation of the intervention for the centre of healthy living.
