CRyoballoon Ablation versus mediCal thErapy in patients with Heart Failure and atrial fibrillation: a multicenter randomized clinical trial, the RACE-8-HF trial

Atriumfibrilleren (AF) is een veelvoorkomende hartritmestoornis waarbij het hart onregelmatig en vaak snel klopt. AF kan voorkomen bij verder gezonde mensen, maar kan ook samengaan met hartfalen. De combinatie van hartfalen en AF zorgt vaak voor veel klachten en een slechtere prognose.

AF kan behandeld worden met medicijnen of met een ablatie, een ingreep waarmee de prikkels die AF veroorzaken geblokkeerd worden. Bij mensen met hartfalen wordt nu nog meestal voor medicijnen gekozen, hoewel een ablatie ook een goede behandeloptie kan zijn.

Met dit onderzoek wordt uitgezocht of een ablatie bij patiënten met hartfalen zorgt voor een betere prognose en minder ziekenhuisopnames, beroertes en klachten dan de gebruikelijke behandeling met medicijnen. Daarnaast worden de kosten van een ablatie en de gebruikelijke behandeling met elkaar vergeleken. Hiervoor worden 600 patiënten verdeeld over de twee behandelgroepen. Daarna wordt enkele jaren gecontroleerd hoe het met ze gaat.

Samenvatting van de aanvraag

Nederlandse samenvatting VRAAGSTELLING Leidt bij patiënten met hartfalen (HF) en paroxismaal of persisterend atriumfibrilleren (AF) vroeg invasief ingrijpen door middel van cryoballonablatie tot minder sterfte, ongeplande cardiovasculaire ziekenhuisopnames en beroertes dan behandeling met standaard medicamenteuze therapie? Leidt deze vroege ablatie zo op lange termijn tot lagere zorgkosten? HYPOTHESE Verwacht wordt dat vroege cryoballonablatie zorgt voor minder AF, wat op zijn beurt leidt tot betere klinische uitkomsten bij HF patiënten. Door minder ziekenhuisopnames en minder zorgconsumptie zullen de zorgkosten in de ablatiegroep lager zijn, ondanks de hogere initiële kosten van de ablatie zelf. STUDIE OPZET Multicenter, gerandomiseerde, open label klinische trial. STUDIEPOPULATIE Symptomatische patiënten met HF met verminderde ejectiefractie (<40%) en paroxismaal of persisterend AF. INTERVENTIE AF ablatie door middel van cryoballonablatie. GEBRUIKELIJKE ZORG Standaard medicamenteuze zorg. UITKOMSTMATEN De primaire uitkomstmaat is een gecombineerd eindpunt van mortaliteit, cardiovasculaire ziekenhuisopnames en beroertes (time-to-event analyse). Er zijn drie secundaire uitkomstmaten: gecombineerd eindpunt van mortaliteit, aantal cardiovasculaire ziekenhuisopnames en beroertes (recurrent-event analyse); hiërarchisch eindpunt van mortaliteit, cardiovasculaire ziekenhuisopnames, beroertes en HF klachten; en kosteneffectiviteit. SAMPLE SIZE BEREKENING/DATA-ANALYSE Om verschillen in het primaire eindpunt met voldoende power en significantie aan te tonen worden 600 patiënten geïncludeerd. Het primaire eindpunt wordt geanalyseerd met een Kaplan-Meier analyse en log-rank test. De eerste twee secundaire eindpunten worden berekend met respectievelijk het Andersen-Gill model en de Finkelstein-Schoenfeld methode. KOSTENEFFECTIVITEITANALYSE/BUDGET IMPACT ANALYSE De kosteneffectiviteitanalyse wordt uitgevoerd door een trial-based economische evaluatie vanuit maatschappelijk perspectief en een Markov model. Quality adjusted life years (QALYs) worden bepaald met prognostische gegevens uit de studie en kwaliteit van leven vragenlijsten. De totale kosten (zorgconsumptie, productiviteitsverlies) worden berekend met behulp van vragenlijsten en ziekenhuisgegevens. De budget impact analyse wordt verricht vanuit de perspectieven van maatschappij, gezondheidszorg en verzekeraar. TIJDPAD De inclusieperiode duurt 30 maanden, de mediane follow-up is 24-36 maanden. Analyses en manuscript kosten 6 maanden. De totale studieduur wordt dan 60 maanden. English summary RESEARCH QUESTIONS Does early invasive therapy consisting of cryoballoon ablation result in decreased rates of mortality, unplanned cardiovascular hospitalisations and stroke in heart failure (HF) patients with paroxysmal or persistent atrial fibrillation (AF), when compared to standard medical therapy? Does early AF ablation thus lead to lower health care related costs in the long term? HYPOTHESIS Reducing the burden of AF by cryoballoon ablation is anticipated to result in improved clinical outcome in HF patients. The avoided hospitalisations and healthcare resource utilisations are expected to ensure lower costs in the AF ablation group, despite initial higher costs of the procedure. STUDY DESIGN Multicentre, randomised, open label clinical trial. STUDY POPULATION Symptomatic patients with HF with reduced ejection fraction (<40%) and paroxysmal or persistent AF. INTERVENTION AF ablation using cryoballoon therapy. USUAL CARE Standard medicinal care. OUTCOME MEASURES The primary outcome measure is a combined endpoint of mortality, cardiovascular hospitalisations and stroke (time-to-event analysis). Secondary outcome measures are a combined endpoint of mortality, number of cardiovascular hospitalisations and stroke (recurrent-event analysis); hierarchical endpoint of mortality, cardiovascular hospitalisations, stroke and HF complaints; and cost-effectiveness. SAMPLE SIZE/DATA-ANALYSIS To demonstrate differences in the primary endpoint with sufficient power and significance, 600 patients will be included. The primary endpoint will be analysed using Kaplan Meier analyses and log-rank testing. The first two secondary endpoints are analysed using the Andersen-Gill model and the Finkelstein-Schoenfeld method, respectively. COST-EFFECTIVENESS ANALYSIS/BUDGET IMPACT ANALYSIS Cost-effectiveness analysis will be performed using a trial-based economic evaluation from a societal perspective and Markov modelling. Quality adjusted life years (QALYs) are assessed using survival data and quality of life questionnaires. Total costs (medical consumption, productivity loss) are assessed using questionnaires and hospital data. Budget impact analysis is performed from a societal, health care provider and health care insurer perspective. TIME SCHEDULE The inclusion period is 30 months, mean follow-up duration is 24-36 months. Analyses and manuscript cost 6 months. Therefore, total study duration is 60 months.
