Convalescent plasma as a treatment for high-risk outpatients administered within 7 days after COVID-19 disease onset (CoV-Early)

In de CoV-Early studie wordt onderzocht of het toedienen plasma van ex-coronapatiënten leidt tot een sneller herstel bij coronapatiënten die nog niet zijn opgenomen in het ziekenhuis. Omdat eerder onderzoek in Nederland geen voordeel liet zien van een behandeling met plasma in een later stadium van de ziekte (in het ziekenhuis), dienen wij het in deze studie snel (ten laatste zeven dagen na het begin van de ziekte) toe.

Onderzoek

In dit onderzoek krijgt één groep patiënten plasma met antistoffen en krijgt een tweede groep, de controlegroep, plasma zonder antistoffen tegen het coronavirus. Dit is noodzakelijk om de werkzaamheid van plasma betrouwbaar te kunnen onderzoeken. Om mee te kunnen doen moet een patiënt minstens 70 jaar zijn of 50 tot 69 jaar oud zijn en daarnaast tot een risicogroep behoren.
De studie wordt door het Erasmus MC en het LUMC gecoördineerd en is in samenwerking met Sanquin opgezet.

Samenvatting van de aanvraag

The COVID-19 pandemic puts unprecedented pressure on health care systems. It results in hospital and in particular ICU capacity problems and caused a delay in the diagnosis and treatment of other life-threatening diseases. In the absence of an effective vaccine, a widely available treatment against SARS-CoV2 that is able to reduce disease severity and the need for hospital admission is urgently needed. We designed the Cov-Early study to evaluate the effect of convalescent plasma (convP, i.e. plasma from ex-COVID-19 patients that contains virus neutralizing anti-SARS-CoV2 antibodies) on a 5-point disease severity scale ranging from symptom free over hospital admission and death. Although observational data suggest that ConvP may be effective as a treatment for SARS-CoV1, MERS and influenza, so far there are no data from randomized clinical trials that have proven its efficacy. The Dutch ConCOVID study demonstrated that 80% of the patients already have neutralizing antibodies at the time of hospital admission. Therefore, the Cov-Early study is designed as a nationwide multicenter double-blind randomized clinical trial that focuses on patients early in there COVID-19 disease course. 700 SARS-COV-2 RT-PCR positive patients will be randomized to the standard of care plus 300ml of non-convalescent fresh frozen plasma or 300ml of convP. Patients included are outpatients, symptomatic for <8 days and fulfill predefined criteria that have been demonstrated to be associated with a higher risk of a poor outcome (i.e. age 70 or older or age 50 in combination with certain comorbidities). ConvP is retrieved by Sanquin from COVID-19 recovered donors. Only ConvP with high titers of SARS-COV-2 neutralizing antibodies will be used. Simultaneously with the COV-Early trial we plan a phase I PK study. If needed the dose of ConvP or the minimum titer of antibodies will be changed by a study amendement.
