Vraagstuk

Mensen met slechthorendheid kunnen op de werkvloer met allerlei problemen te maken krijgen. Ze zijn bijvoorbeeld eerder vermoeid. Het Arbo Audiologisch Spreekuur helpt deze groep bij het verbeteren van de werksituatie. Bijvoorbeeld door aanpassingen aan de werkplek of het takenpakket, of door hulpmiddelen in te zetten. Wat is de (kosten)effectiviteit van het Spreekuur in vergelijking met gebruikelijk zorg?

Onderzoek

Van in totaal 136 slechthorende werknemers ging de ene helft naar het Spreekuur, de andere helft ontving gebruikelijke zorg. De onderzoekers bestudeerden het effect van het Spreekuur op onder andere herstelbehoefte, het functioneren op het werk, werkvoldoening en verzuim.

Uitkomst

Er is tussen interventie- en controlegroep geen verschil gevonden in ‘herstelbehoefte na het werk’, de belangrijkste uitkomstmaat. Veel deelnemers hadden aan het begin van de studie al een heel goede herstelbehoefte. Wel blijkt het spreekuur effectiever in het verhogen van ‘zelfacceptatie’ en het ‘reduceren van inadequaat copinggedrag’, twee essentiële elementen van kwaliteit van leven. Het spreekuur is niet kosteneffectiever dan gebruikelijke zorg.

Het Arbo Audiologisch Spreekuur is een transmuraal multidisciplinair zorgprotocol gericht op behoud, facilitering en/of verbetering van de werksituatie van mensen met problemen op het werk als gevolg van slechthorendheid. In deze studie is de (kosten) effectiviteit van het spreekuur vergeleken met gebruikelijk zorg en is het implementatieproces van het spreekuur geëvalueerd.
In totaal deden 136 slechthorende werknemers mee die random werden verdeeld over twee groepen. De ene helft (N=68) doorliep het spreekuur, de andere helft (N=68) ontving gebruikelijke zorg. Het implementatieproces is bestudeerd in de interventiegroep (N=68).
De primaire uitkomstmaat was herstelbehoefte. Secundaire uitkomstmaten waren communicatiestrategieën, coping met slechthorendheid, stress en self-efficacy. Data werden verzameld vóór de interventie (op baseline) en 3,6,9 en 12 maanden erna. Het spreekuur werd adequaat uitgevoerd en alle stappen in het protocol werden goed nageleefd. Na afloop was er geen verschil in herstelbehoefte tussen de interventiegroep en de gebruikelijke zorg groep. Wel bleek het spreekuur effectiever dan gebruikelijk zorg voor het verhogen van ‘zelf acceptatie’ en het ‘reduceren van inadequaat coping gedrag’, twee essentiële elementen van kwaliteit van leven. Het spreekuur bleek niet kosten-effectiever dan gebruikelijke zorg.

Samenvatting van de aanvraag

Background Hearing impairment is one of the most prevalent chronic disabilities. It has significant negative effects on work performance and is associated with high levels of need for recovery after work, sick leave and early retirement. These consequences are related to high costs. Care as Usual is insufficiently equipped to effectively address the vocational difficulties of employees with hearing problems. The Vocational Enablement Protocl (VEP) is a transmural multidisciplinary care protocol aimed at maintaining, facilitating, or improving the employment situation for people with hearing loss. Objective The main research question is: Is the VEP cost-effective in terms of reducing the need for recovery after work compared to Care as Usual? Secondary research questions focus on the effects of the VEP on sick leave, work productivity (Prodisq), quality of life (EQ5D), coping with hearing impairment (CPHI) and patient’s satisfaction (PSQOH). Method: A randomized controlled trial will be performed with 80 participants in the control (Care as Usual group) and 80 participants in the treatment (VEP) group. Here, we will collaborate with three companies: Corus, KLM and VU University. Outcomes will be measured at baseline, post-treatment, 6 months post treatment and at 12 months. Data analysis Data will be analyzed according to the “Intention To Treat Principle”. A General Linear Model with repeated measures will be applied for each the outcomes. Potential confounders will be taken into account. Economic evaluation Cost-effectiveness/utility will be evaluated from the societal perspective. Bootstrapping will be used. Transfer of knowledge The results of this study will be published in (inter)national peer reviewed journals, presented at conferences and disseminated among professionals and relevant stakeholders. Planning/Time schedule Month 1-12: organisation of the study, recruitment, baseline measurements, allocation to VEP or Care as Usual (CAU); Month 12-24: Recruitment, allocation to VEP or CAU, post measurements, follow-up measurement. Month 24-36: Post measurements, follow-up measurements, data-entry, data-analysis. Month 36-48: data analysis, publication of the results.
