From evidence-based to practice-based: (Cost)effectiveness of SLIM diabetes prevention intervention in a real-life setting.

SLIMMER is een gecombineerde leefstijlinterventie, gebaseerd op de bewezen effectieve SLIM-interventie. In onderzoekssetting is aangetoond dat SLIM het risico op diabetes met 50% verlaagt. Echter SLIM blijkt nog onvoldoende aan te sluiten bij de alledaagse praktijk. In deel I van dit project wordt SLIM vertaald naar de Nederlandse praktijksetting. De vertaalde draaiboeken worden getest in een pilot, begeleid met procesonderzoek om bereik, waardering, integriteit en succes- en faalfactoren vast te stellen. In deel II wordt de SLIMMER-interventie verder uitgerold in Apeldoorn, begeleid met effectonderzoek. De hoofdvraag is of SLIMMER in de Nederlandse praktijksetting net zo effectief is als SLIM in onderzoekssetting. Hiervoor wordt een effectiveness study uitgevoerd waarbij rekening wordt gehouden met de context en diverse uitkomstmaten in het intervention logic model worden gemeten. Dit project wordt uitgevoerd door Academische Werkplaats AGORA in intensieve samenwerking met gemeente Apeldoorn, Regionale Ondersteuningsorganisatie Caransscoop, Huisartsenzorg regio Apeldoorn, Thuiszorgorganisatie Vérian en Samenwerkende Fysiotherapeuten Apeldoorn.

Uit dit onderzoek blijkt dat het mogelijk is om diabetes effectief te voorkomen bij hoogrisicogroepen. Deelnemers aan SLIMMER volgden een programma van 10 maanden bij de fysiotherapeut en dietist. Daarna waren ze afgevallen, voelden ze zich fitter, en hadden ze betere bloedglucosewaarden. Daarmee kan SLIMMER een belangrijke bijdrage leveren om de stijging van diabetes in Nederland te voorkomen. Deze positieve resultaten zijn vergelijkbaar met eerdere resultaten van SLIM in klinische setting. SLIM toonde aan dat diabetes met 50% voorkomen kan worden door hoogrisicogroepen gezonder te laten eten en meer te laten bewegen. Bovendien blijkt uit interviews bij huisartsenpraktijken, dietisten en fysiotherapeuten dat SLIMMER goed uitvoerbaar is en dat 2 uur training voldoende is om met SLIMMER te kunnen werken. SLIMMER wordt door zorgverleners gewaardeerd met een 8,2 en door deelnemers met een 8,1. Zorgverleners waarderen met name de multidisciplinaire samenwerking, de goede kwaliteit en het feit dat het voor deelnemers echt wat oplevert. Deelnemers waarderen met name het sporten in de speciale SLIMMER groepjes. Maar ook de lange termijn is belangrijk. Bij SLIMMER wordt veel aandacht besteed aan de eigen kracht van deelnemers. Zodat ze hun gezonde gewoontes zelf kunnen volhouden, als de begeleiding door de huisarsenpraktijk, dietist en fysiotherapeut stopt. Zo gaan deelnemers met hun SLIMMER groepje kennismaken bij divers sportverenigingen in de buurt, schrijven ze hun eigen actieplan om gezond te blijven eten, en worden ze na 3 maanden uitgenodigd voor een terugkombijeenkomst. SLIMMER is de eerste bewezen effectieve praktijkinterventie voor diabetespreventie in Nederland. Op dit moment vinden nog analyses plaats om te bepalen of SLIMMER kosteneffectief is.

SLIMMER diabetes voorkomen is een programma voor mensen met een hoog risico op diabetes. Zij worden door hun huisarts uitgenodigd en vervolgens ontvangen zij 10 maanden begeleiding van een dietist. Ook sporten ze in wekelijkse SLIMMER groepjes onder begeleiding van een fysiotherapeut. Op dit moment zijn er meer dan 300 deelnemers in Apeldoorn en Doetinchem. Meer dan 50 zorgverleners zijn getraind. Er zijn actieve lokale stuurgroepen waarin ook de gemeente en zorgverzekeraar meedoen. De deelnemers zijn nu enkele maanden bezig met het programma en de reacties zijn positief. In 2013/2014 volgen resultaten over (kosten)effectiviteit in de praktijk.

Samenvatting van de aanvraag

The number of people with type 2 diabetes is sharply rising, causing global concern. However, clinical trials have shown conclusively that diabetes can be postponed or prevented among adults with prediabetes by a combination of a healthy diet and increased physical activity. One of these is the Dutch SLIM intervention, showing a 47% risk reduction of diabetes after 6 years of follow-up, at costs generally considered acceptable to society. Although such trials are highly promising, they are not easily applicable in real-life practice, due to the characteristics inherent to experimental conditions. Moreover, even if these trials were to be translated and implemented into practice, their effects and costs in real-life would be unknown since these are highly dependant on the actual context as well as intervention aspects such as e.g. subject screening and selection; intervention frequency and duration; intervention strategy, elements and materials; and professionals who deliver the intervention. As a result, public health practice and policy are not yet able to profit from the evidence of diabetes prevention interventions. To our knowledge, few studies affress this gap. Therefore, the aim of our project is to adapt the SLIM intervention into a SLIMMER intervention (SLIM iMplementation Experience Region gelre-ijssel) which suits the Dutch real-life setting, in intensive collaboration with relevant local parties. Subsequently, we will determine its (cost)effectiveness in daily practice, in order to deliver adequate information for policy decisions. This project is both of scientific and societal relevance. Firstly, it is of scientific relevance since it focuses on the question how to proceed from research to practice. The comparison of the (cost)effectiveness of SLIMMER in real-life setting to the (cost)effectiveness of SLIM in experimental setting fills a major gap in (international) diabetes prevention research. In our vision, interventions should be evidence-based, practice-based as well as policy-base to be suitable for large-scale implementation (Rose of Agora: quality model for public health interventions). Additionally, this project is of societal relevance as it brings partners from prevention and primary care together and provides health professionals/policy makers with feasible and (cost)effective instruments to address the problem of diabetes, one of the five national health priorities. To accomplish this objective, three main research objectives have been formulated. Firstly, a qualitative study will be performed in order to assess specific characteristics of real world (especially the setting of pilot municipality Apeldoorn) in relation to aspects of the SLIM intervention. Necessary adjustments will be made in close collaboration with local parties from prevention and primary care, e.g. general practicioners, dieticians, physiotherapists, the Regional Supporting Organisation of Primary Care (ROS), Home care Organisations (Thuiszorg) and the Community Health Service (GGD). Since these parties have been collaborating in a collective network since 2008, initiated by the local authorities and co-ordinated by AGORA, conditions for successful co-operation are already fulfilled. The adapted SLIMMER intervention will be tested for feasibility, adherence, implementation integrity and key factors of failure and success, in a pilot in 3 general practices with concurrent dieticians and physiotherapists. Subsequently, the effectiveness of the adapted SLIMMER intervention compared to no intervention/usual care will be evaluated in a non-randomised design with pre- and postmeasurements in two groups with incorporated process evaluation. General practices will be enrolled into either intervention (20 practices) or control group (20 practices) according to the selected implementation strategy (~20 subjects/ practice). Based on context and stakeholder analysis we expect selection bias of general practices by enthousiasm, but no selection bias for subject characteristics. Relevant measures of effectiveness will be assessed using an Intervention Logic Model including output indicators (adherence, implementation integrity) and initial outcomes (motivation, behaviour control/self-efficacy) in addition to the already available intermediate (dietary and physical activity behaviour) and long-term outcomes (overweight, glucose tolerance) for evaluating the SLIM intervention. For data collection available registries will be used with minimal additional measurements / questionnaires. Intervention duration will be 1,5 year with an extra follow-up measurement 6 months after discontinuation of the intervention. To conclude, cost-effectiveness of the SLIMMER intervention in real-life will be analysed using the results of the effectiveness study and detailed information of real costs in every-day practice. Effectiveness and cost-effectiveness of SLIMMER will be compared to the original SLM intervention (historical reference).
