E-learning program for professionals in primary care and welfare to improve the identification and management of adults at high risk of depression

Projectomschrijving

GGD Hollands Midden, LUMC en het Trimbos-instituut ontwikkelen een digitale bijscholing over depressiepreventie voor HBO-professionals. Het doel is dat hun kennis, motivatie, houding en vaardigheden verbetert om individuen met (een hoog risico op) depressieve symptomen te herkennen, te motiveren en te verwijzen.

Op grond van een literatuurstudie, interviews met professionals en een expertmeeting zijn de leerdoelen voor het scholingsprogramma vastgesteld, evenals de strategieën om deze doelen te bereiken. Het programma zal bestaan uit een aantal modules met een sterk interactief karakter.

In een proeffase zal een groep professionals het programma volgen en beoordelen in hoeverre ze het werkbaar en aantrekkelijk vinden. Ook zal het proefproject informatie geven over de kwaliteit van het gebruik van het programma en al enig inzicht geven in de effecten van het programma op de kennis, houding en gedrag van de professional.

Door de inzet van specifieke interventies bij mensen met een hoog risico op depressie kan het ontstaan van depressies worden voorkomen. Daarvoor is nodig dat depressieve klachten bijtijds worden herkend en er vervolgens toeleiding naar de interventies plaatsvindt. Professionals hebben hierin een belangrijk rol. De GGD Hollands Midden, het LUMC en het Trimbos Instituut (vanuit de Academische Werkplaats Publieke gezondheid Noordelijk Zuid-Holland) hebben daarom een E-learning Signaleren depressie ontwikkeld voor HBO-professionals in zorg en welzijn. Het e-learning programma leert professionals in zes lessen hoe cliënten met depressieve klachten kunnen herkennen, motiveren, adviseren en verwijzen.

Samenvatting van de aanvraag

Depression is a major health problem. The annual prevalence for adults is over 5%, an episode of depression lasts on average 6 months, it influences quality of life, and is associated with high societal costs. Therefore, increasing attention is paid to possibilities to prevent/reduce the incidence of depression. A high risk for a depressive disorder is the presence of symptoms that do not yet meet the criteria for a depressive disorder. The prevalence of a sub-threshold depression is about 16%. Although many effective interventions are available for individuals with (a high risk of) sub-threshold depression, the number of participants engaging in these interventions is limited. Early identification by professionals in primary care and welfare, followed by an adequate referral process, may be an important approach to increase the numbers of individuals at high risk of depression who engage in effective preventive interventions. A major barrier to the identification and management of depressive symptoms is insufficient training/knowledge to diagnose/counsel and/or to treat the patient. Health professionals may also lack confidence in their ability to diagnose depression and may lack willingness to change their approach to the identification process. Improving the skills of these professionals to identify and manage depressive symptoms can contribute to the prevention of depression. However, educating/training large groups of professionals from different organizations is a major task. Moreover, the training should be repeated for new employees, and continuing education and training is desirable. For these reasons, an e-learning program might be a successful and cost-effective tool to meet these requirements. The proposed project aims to develop an e-learning program to train professionals in primary care and welfare in identifying, discussing, motivating and referring individuals with (a risk of) depressive symptoms. The first step involves a systematic approach to develop the e-learning program, i.e. reviewing the literature and interviewing professionals to determine essential factors that need to be integrated in the program. Thereafter, the program will be designed, developed and pretested. Finally, the e-learning program will be piloted among 60 professionals working in primary care and welfare. This pilot study will evaluate: 1) whether the program improves knowledge, motivation, attitudes, and perceived social norms towards and skills in identification and management of depressive symptoms; 2) whether the program improves the identification and management practices of professionals in primary care and welfare; 3) the completeness and quality of use of the e-learning program by professionals in primary care and welfare; 4) the promoting and impeding factors in using an e-learning program for professionals working in primary care and welfare. Prior to and after producing the e-learning program, professionals will be interviewed (questionnaires and face-to-face interviews). During the e-learning program details on website visits and use will be recorded for each participant.
