In dit project is een interactieve infographic ontwikkeld: www.meerbereikmetpreventie.nl. Deze infopraphic ondersteunt professionals bij het vergroten van het bereik van preventieve interventies op het gebied van psychische aandoeningen.

Psychisch leed

Psychische aandoeningen, zoals depressieve en angststoornissen en middelenmisbruik, komen veel voor en veroorzaken veel leed. Preventieve interventies zijn effectief, maar worden nog onvoldoende gebruikt.

ZonMw projecten bundelen kennis

Negen preventieprojecten van ZonMw hebben gezamenlijk de ervaringen en lessen rondom het bereik van preventieve interventies in kaart gebracht. De voorkeuren van zes hoog-risico doelgroepen over hoe zij preventieve interventies aangeboden willen krijgen, is samengebracht in een infographic.

Professionals kunnen hier informatie, tips en voorbeelden op maat verzamelen aan de hand van vier vragen over hun situatie. Ook kunnen zij aan de hand van een quiz-format inzicht krijgen in de voorkeuren van de zes doelgroepen. Veel professionals raadplegen de infographic en deze blijft waarschijnlijk ook na 2018 toegankelijk. Uit het project blijkt wel dat verdere specificatie naar subdoelgroepen noodzakelijk is, evenals meer onderzoek naar effectieve bereikstrategieën en naar redenen waarom mensen wel of niet deelnemen.

Eindverslag

Psychische aandoeningen komen veel voor en veroorzaken veel leed. Onderzoek heeft aangetoond dat preventie van psychische aandoeningen loont: het is effectief en kostenbesparend. Preventieve interventies kunnen in potentie een behoorlijke impact te hebben op de volksgezondheid, maar ze worden nog niet voldoende gebruikt. Tot 2006 was het bereik van 1-2% en recent is dat getal gestegen naar 14%: er is ruimte voor verbetering.

In dit project zullen belemmerende en bevorderende factoren voor het bereik van preventieve interventies geïdentificeerd worden aan de hand van: de ervaringen en bevindingen van een negental ZonMw gefinancierde preventieprojecten die in dit project samenwerken, een meta-regressie analyse van gepubliceerde preventietrials, en een vignettenstudie om inzicht te krijgen in de voorkeuren van een zestal doelgroepen met betrekking tot hoe zij preventieve interventies aangeboden willen krijgen. Specifiek gaat het om (subgroepen binnen) de volgende doelgroepen: pas bevallen moeders, jongeren, werknemers, huisartspatiënten, ouderen en mantelzorgers.

Uiteindelijk doel is om een interactieve infographic te ontwikkelen en te evalueren dat professionals helpt om optimale bereikstrategieën te identificeren voor preventie van verschillende psychische klachten bij verschillende doelgroepen.

Uiteindelijk doel is om een online beslissingsondersteunend instrument te ontwikkelen dat professionals helpt om optimale bereikstrategieën te identificeren voor preventie van verschillende psychische klachten bij verschillende doelgroepen.

Samenvatting van de aanvraag

The common mental disorders, such as DSM-IV anxiety, depressive and alcohol use disorders (and their underlying risk factors, such as loneliness, lack of social support, chronic co-morbid medical illnesses) • are highly prevalent, • have detrimental impacts on family, social and professional role functioning, • are leading causes of disease burden, • have substantial economic consequences due to health care uptake and productivity losses via absenteeism and lesser efficiency while at work. Doing ‘nothing’ is not an option owing to the adverse impacts of these disorders on population health. Preventive interventions are available. Typically, a preventive intervention reduces the incidence by 20~30% and as much as 50% when offered in a stepped-care mode. Some of the preventive interventions are known to be cost-effective, especially when offered as low cost, self-help interventions. Under such conditions, prevention can even become cost saving, thus representing a viable business case for implementation. While such preventive interventions are on offer in the Dutch mental health care system, relatively few people make use of these interventions. For years on end, the number of participants of interventions to prevent depression formed a small fraction (1 or 2%) of the population that developed the disorder in a given year. Over the last couple of years the uptake rates rose to 14%. This is a significant improvement, but still one would like to see higher participation rates in preventive interventions. After all, the preventive interventions cannot deliver their promise of protecting and sustaining population health when relatively few people make use of these interventions. This research project aims to identify inhibiting and facilitating factors (see below) in the recruitment and participation process in order to generate ideas for more successful strategies to engage people in preventive activities. To that end, and complying with explicit preconditions laid out by ZonMw (see below), the study will be conducted along the following four steps: 1. The research teams of 9 recent prevention and implementation studies will jointly review the inhibiting and facilitating factors in recruitment & participation process as encountered in their studies. This step will cumulate in a brief report, listing and highlighting main stumbling blocks and best practices for enhancing recruitment & participation in preventive interventions. 2. Building on the insights obtained in the first step and in order to extend the empirical evidence-base, a review of the literature will be conducted. To be more precise, the CONSORT flow-charts of prevention trials will be studied to extract data on the number of people approached, screened, included, randomised into the prevention trial and the number of people who completed the intervention. We will use meta-regression techniques to regress these transition rates on the characteristics of (1) the recruitment strategy, (2) the target population, (3) type of intervention, (4) the way the intervention has been embedded in the wider organisational context, and (5) study characteristics. This step will result in a longer and empirically validated list of inhibiting and facilitating factors that play a role in recruitment and participation process. 3. The information gathered in steps 1 and 2 will be used to inform a number of panels of experts and end-users. Each panel will act as a focus group within a conjoint experiment to elicit preferences for certain recruitment and participation strategies. A first focus group will consist of the researchers that participated in step 1. The other focus groups will consist of potential target groups for preventive interventions as outlined in an upcoming Policy Green Paper such as (1) new mothers, (2) youngsters, (3) working population, (4) GP patients, (5) older people, and (6) informal care givers, and more specifically the vulnerable subgroups therein. Each of these panels will pass judgements on the appropriateness of strategies; and these strategies will be described as vignettes that were generated by making orthogonal combinations of the afore mentioned facilitating factors. It is worth noting that we will use the same formal and strict methodology that has often been used in medical decision-making and health care innovation. This step will cumulate in a decision-supporting tool that helps finding the most optimal strategy for recruiting and engaging participants in preventive interventions. 4. The decision aid will be pilot tested in prevention practice, by conducting in-company workshops directed at optimizing the recruitment strategy for three or four target groups. Prevention professionals will be trained to work with the tool and then evaluate its use. Lessons learned in the pilot will help to improve the tool.
