Prevention of psychosis with a cognitive behavioural intervention in help-seeking young people with an at risk mental state for developing psychosis

Het gerandomiseerde onderzoek bij mensen met een ultrahoog risico op het ontwikkelen van een psychotische episode heeft aangetoond dat een cognitief gedragstherapeutische interventie in staat is het aantal transities naar psychose met ongeveer de helft terug te dringen gedurende een follow-up van 18 maanden. Ook het aantal mensen dat na 18 maanden vrij is van subklinisch psychotische ervaringen is groter in de experimentele conditie dan in de controle conditie.

Geindiceerde preventie van psychose binnen een hulpzoekende populatie in de tweede lijn van de geestelijke gezondheidszorg is aan te raden op grond van deze studie en andere gerapporteerde studies.

Samenvatting van de aanvraag

The aims of the proposed study are 1) to identify samples of help-seeking people operationally defined to be at high risk and 2) to evaluate the effectiveness of cognitive behavioural intervention in preventing transition to psychosis and/or in reducing the persistence of psychosis-like experiences. The effectiveness of the cognitive intervention will be contrasted with treatment as usual and studied in a randomised controlled trial of the collaborating centres of the department of Clinical Psychology of the VU University Amsterdam, Parnassia Psychiatric Institute The Hague and the Adolescent Clinic of the Academic Medical Centre of the University of Amsterdam. In The Hague all individuals who seek help for a mental problem in the age 18 to 35 will be screened with the Prodromal Questionnaire as will all individuals in Amsterdam who are suspected of psychosis and are referred to a well known speciality clinic. Both samples that are suspect of ARMS will be assessed with the CAARMS to discriminate below threshold ARMS from ARMS from psychotic subjects. The people with ARMS will be randomised into two groups with targeted CBT or TAU. The inclusion period will take place during 24 months. Randomisation will stop after 240 inclusions. Treatment consists of 25 sessions over 6 months. The effectiveness of the intervention will be measured using reliable and valid measures of psychotic symptoms. The project builds on significant work that has been conducted in Melbourne Australia and Manchester in the United Kingdom.
