Ontwikkelen en testen van een internetprogramma dat moet voorkomen dat mensen die overgewicht hebben zwaarder worden

Het doel van dit project is om een internetprogramma te maken dat volwassenen die overgewicht hebben helpt om niet zwaarder worden. Een beetje minder eten of wat meer bewegen kan al voldoende zijn om niet zwaarder te worden. Het internetprogramma helpt mensen daarbij. In het programma kunnen mensen vragen over zichzelf invullen. Ze krijgen dan persoonlijk advies over wat zij minder kunnen eten of hoe ze meer kunnen bewegen. Ook kunnen ze plannen maken voor hoe ze precies willen veranderen. In een onderzoek is getest of het programma voorkomt dat mensen met overgewicht zwaarder worden. Daarvoor is het gewicht van een groep mensen die het internetprogramma gebruikte vergeleken met dat van een groep die algemene informatie over eten en bewegen kreeg. Uit dit onderzoek bleek dat het gewicht in beide groepen gelijk was gebleven. Dit betekent dat het internetprogramma niet voldoende effectief was. Het kon zwaarder worden niet beter voorkomen dan de algemene informatie.

Samenvatting van de aanvraag

Development of effective obesity prevention interventions is an important goal of the ZonMw prevention program. Obesity is one of the world's most challenging public health problems, because of the rapidly increasing prevalence and because of the association with the ZonMw priority diseases; cardiovascular diseases, cancers and type 2 diabetes. There is an urgent need for obesity prevention interventions and an important group to target are overweight people (BMI >= 25 < 30 kg/m2), since this is the group directly at risk for becoming obese. Prevention of obesity in overweight people can be achieved by weight management, which includes weight maintenance and gradual loss of a small amount of weight. Weight management can best be achieved by adopting a healthy lifestyle, consisting of a favourable balance between energy intake and energy expenditure. Small but sustained changes in dietary intake and PA are often sufficient to achieve weight maintenance and even modest weight loss, but evidence shows that it is difficult to maintain such small changes. This indicates that powerful weight management interventions, that do not only motivate people to make changes, but also guide in initiating and maintaining changes, are needed. Self-regulation models and techniques from cognitive behavioural counselling for weight management provide strategies for behaviour initiation and maintenance, but these strategies have not been implemented in interactive, individualised interventions that are available to large groups of people. The Internet provides the opportunity to deliver interventions that include motivational, action initiation and maintenance strategies that can be provided in an interactive, individualised approach to large groups of people. We propose to develop such an individualised, action oriented, interactive, Internet-delivered weight management intervention for overweight adults that (I) focuses on weight management instead of on weight loss; (II) incorporates evidence based strategies for motivation, action initiation and maintenance instead of motivational strategies only; (III) uses the Internet to serve large numbers of people in an individual approach; (IV) is unique and distinct from existing interventions in the Netherlands. This proposal is submitted to the Innovation track of the ZonMw prevention program. The main purpose of the proposed project is therefore to develop an innovative intervention. We further propose an extensive formative evaluation, as well as a randomised controlled trial for the evaluation of short-term (one and six month) effects on the primary outcome measures of body mass index, waist circumference and skin fold thickness and on the secondary outcome measures of dietary intake, PA and behavioural determinants.
