Single men on welfare: target group for prevention in public mental health care.

Dit project is gestart om inzicht te krijgen in de leef- en gezondheidssituatie van alleenstaande mannen met een bijstandsuitkering en om aangrijpingspunten voor preventie te identificeren. Het project Mankracht is een samenwerking met de Dienst Werk en Inkomen van de gemeente Amsterdam en AGIS. Het project is in 2008 gestart en wordt uitgevoerd door de GGD Amsterdam en INGeest/VU MC. De eerste meting is inmiddels afgerond en de resultaten zijn teruggekoppeld via een symposium. Een tweede meting onder dezelfde respondenten zal in 2012 plaatsvinden, met financiering van ZonMw.

In dit project is een deel van de doelgroep getraind om samen met de onderzoekers het onderzoek uit te voeren. Achtergrond van dit onderzoek is dat alleenstaande mannen met een uitkering een risicogroep vormen voor maatschappelijke uitval. Verschillende partijen maken zich zorgen over deze doelgroep, zoals de sociale dienst, het maatschappelijk werk, de verslavingszorg en de GGZ. Ook de gezondheidsmonitor biedt indicaties dat dit een doelgroep is waar zowel sociale als medische problemen samenkomen.

Project Mankracht beoogt inzicht te bieden in het proces van afglijden en opklimmen van alleenstaande mannen met een bijstandsuitkering. Op basis van dit inzicht worden aangrijpingspunten voor preventie geïdentificeerd. Het onderzoek binnen project Mankracht brengt met twee longitudinale metingen een steekproef van de doelgroep in beeld met betrekking tot demografie, ziektelast, persoonlijke, maatschappelijke en sociale hulpbronnen en indicaties van zorgwekkend functioneren. De eerste meting van het onderzoek is uitgevoerd van eind 2009 tot begin 2010 door de doelgroep zelf. Daartoe hebben circa 50 mannen uit de doelgroep deelgenomen in een activeringstraject en zijn getraind tot interviewer en klankbordgroep voor het onderzoek. Op basis van de eerste meting wordt de doelgroep beschreven. In 2012 wordt de tweede meting uitgevoerd. Samenvatting van de aanvraag

Single men on welfare constitute a high risk group for public mental health care. Little information is available about the process of deterioration into societal dysfunctioning, such as social exclusion and homelessness, for this group. This information is necessary to develop evidence-based preventive strategies to stop the process of deterioration and/or to stimulate the improvement of functioning for this group. In order to provide an understanding in these processes and identify crucial determinants in single men on welfare, a longitudinal prospective cohort study is proposed. The study population are single men (living alone in a non-institutionalised setting) receiving welfare from the Social Welfare Department (SWD) in Amsterdam between 23 and 65 years of age. Baseline data-collection is currently taking place and aims at including a random sample of 1000 respondents. Information is obtained through an interview based on a structured questionnaire and a short, more qualitative, interview. The questionnaire contains the following topics: illness (somatic and psychiatric, addiction, traumatic life-events), personal resources (self-esteem, mastery, coping, sense of coherence, life style (such as alcohol and drug use, exercise), willingness to seek and accept care, a.o.), social and societal resources (social support, formal social and medical care, participation), subjective and objective societal dysfunctioning (on the domains: unstable or no housing, problematic increasing debts, social exclusion, self-neglect, lack of daily structure and activities and normlessness/public nuisance) and quality of life. These domains all influence each other and may finally result in a spiral down into severe and intertwined problems within multiple life domains. The focus of the study is on prevention of societal dysfunctioning, therefore, this will be used as the outcome in the analyses, and illness and resources as the determinants. The more qualitative interview covers different topics among subgroups of men. Under supervision and within the theoretical framework of quantitative data collection, each interviewer selects a subject, develops a topic list, and performs the interviews among his respondents. The follow-up data-collection is planned two years after the baseline, and covers the same topics so that developments over time can be determined. The interviews are performed by peer-interviewers that follow an activitation trajectory as a first step back into an active participation in society. The peer-interviewers also act as an expert-group for the development of the questionnaire, the approach of the respondents and the interpretation and dissemination of the results. The results of the study will be analysed and reported on both the baseline and the follow-up data-collection. Results will be presented to other researchers through publications and presentations at conferences. In addition, the results will be disseminated to the work field in different ways involving the peer-experts. Work conferences will be held on the most important themes, in which, researchers, peer-experts, policy-makers, care-providers and prevention workers come together to decide on a strategy to develop and plan interventions. After four years we aim to achieve an understanding of single men on welfare, concerning the prevalence of illness, resources and functioning, and the interactions between these factors and developments over time. Based on this knowledge target areas for interventions are selected and communicated with other professionals in the field of public mental health care. In addition, 60 men on welfare will have taken a step forward towards active participation in society.
