UCAN CAN-DU: Canada Netherlands Personalized Medicine Network in Childhood Arthritis and Rheumatic Disease

Een uniek internationaal netwerk van Nederlandse en Canadese onderzoekers onder de naam UCAN CAN-DU moet binnen 5 jaar de basis leggen voor een personalised medicine behandeling van kinderen met jeugdreuma. Alle 6 academische kinderreumatologische centra in Nederland werken samen met alle 18 academische centra in Canada mee aan dit unieke project.

Jeugdreuma treft meer dan 20.000 kinderen in Canada en Nederland met een enorme impact op kind en gezin. Soms kost het veel tijd het juiste medicijn te vinden en uiteindelijk krijgt een derde biologische medicijnen. Hoewel deze kostbare biologische medicijnen het ziekteverloop hebben verbeterd, krijgen gebruikers vaker ernstige bijwerkingen en is staken vaak niet mogelijk zonder het krijgen van een terugval. Bij personalised medicine, behandeling op maat, kan mede door bloedbepaling (biomarkers) beter voorspeld worden wie welk medicijn nodig heeft, met een laag risico op ernstige bijwerkingen en op welk moment dit eventueel weer af te bouwen is.

Samenvatting van de aanvraag

Juvenile Idiopathic arthritis (JIA) is the most common immune mediated inflammatory musculoskeletal disease affecting more than 20.000 children in Canada and the Netherlands.JIA is devastating to the child and family.Many children with JIA will continue to have active arthritis as adults, some with severe disability. High risk children with arthritis often require advanced therapeutic agents. Although biologic therapy can dramatically improve disease outcomes, potential adverse events, including infections and malignancy, and their use imposes substantial burdens on patients,families and society. One in three paediatric and adult patients currently receives biologic therapy for arthritis without any treatment end-date. Recent work by our team has established that patients and treatment responses can be meaningfully classified using approaches that integrate comprehensive biologic and clinical data. Our biomarker discovery program is matureand poised to rapidly deliver robust and highly relevant results and deliver a simple predictive tool to guide therapy. New concepts of standardised disease activity outcomes, novel e-health based patient reported outcome and the real-time integration of individual biological profiles will enable us to learn from every child and family - we will inform practice while caring for children, accellerating tranformative care. Our group is at the fore-front of high quality data collection. we have assembled an inter-disciplinary that will capitalize on our international network's research infrastructure and harmosised data and sample collection and access to large,rigorously- characterised childhood arthritis cohorts. We currently lead these national and international consortia,which will provide biological samples and detailed patient data- our unique advantage for an unpredented value-added opportunity for collaboration and synergy in already established collaborations between Canadian and Dutch researchers.
