Intravenous-to-oral antibiotic switch therapy for suspected neonatal infections: clinical efficacy, safety and cost-effectiveness

Pasgeborenen zijn gevoelig voor bacteriële infecties. Bij een verdenking op of bewezen infectie krijgen zij minimaal 7 dagen antibiotica via een infuus. Infuusbehandelingen bij oudere kinderen worden vaak omgezet naar drank. De behandeling kan dan thuis worden voortgezet. Bij pasgeborenen werd dat tot nu toe meestal niet gedaan.
Studie
In de RAIN-studie zijn 507 voldragen pasgeborenen met een sterke verdenking op bacteriële infectie gedurende 48 uur behandeld met een infuusbehandeling, waarna de helft doorging met deze behandeling en de andere helft overschakelde op antibioticumdrank (amoxicilline/clavulaanzuur).
Resultaten
Uitbehandeling met antibioticumdrank lijkt niet minder veilig en effectief dan een volledige infuusbehandeling bij pasgeborenen. Er werden voldoende hoge antibioticumbloedspiegels bereikt na toediening van de drank. De opnameduur kon verkort worden met 3,5 dagen. De behandeling is naast effectief ook kostenbesparend en heeft een positieve invloed op de kwaliteit van leven van het jonge gezin.

Pasgeborenen zijn gevoelig voor bacteriële infecties. Bij dergelijke infecties krijgen zij minimaal 7 dagen antibiotica via een infuus in het ziekenhuis. Infuusbehandelingen bij oudere kinderen worden vaak omgezet naar drank of tabletten. De behandeling kan dan thuis worden voortgezet. Kleine studies suggereren dat doorbehandelen met antibioticumdrank ook bij pasgeborenen voldoende kan zijn.
Het doel van dit onderzoek is het vaststellen dat doorbehandeling van een bacteriële infectie bij pasgeborenen met antibioticumdrank net zo goed werkt als een volledige infuusbehandeling. In dit onderzoek worden voldragen pasgeborenen gedurende 48 uur behandeld met een infuusbehandeling, waarna de ene groep doorgaat met deze behandeling en de andere groep overschakelt op antibioticumdrank (amoxicilline/clavulaanzuur). De totale behandelingsduur voor beide groepen is 7 dagen. Bij dit onderzoek zijn 16 ziekenhuizen in Nederland betrokken. Wanneer de uitbehandeling met een antibioticumdrank veilig thuis kan plaatsvinden heeft dit een positief effect op de kwaliteit van leven; de opnameduur zal korter zijn en de normale gezinssituatie wordt sneller bereikt. De verwachting is tevens een aanzienlijke kostenbesparing voor de gezondheidszorg.

Neonates are susceptible for bacterial infections. In case of probable infection (negative cultures but elevated inflammatory parameters and/or clinical symptoms), intravenous (IV) antimicrobial treatment is usually administered for 7 days. In older children, IV antibiotics are often switched to oral antibiotics after clinical improvement. Neonatal oral antibiotics seem a promising therapy based on a few published studies, but firm evidence on the equivalent effectiveness and safety is absent. Therefore, a large randomised controlled study needs to be performed. When intravenous-to-oral antibiotic switch therapy in neonatal infections is considered safe and effective, it can have huge effects on the quality of life, admission days and costly in-hospital resource use. The objectives of this research project are: 1. To demonstrate non-inferiority of intravenous-to-oral antibiotic switch therapy in clinically stable neonates with probable bacterial infection compared to a complete course of intravenous antibiotic therapy 2. To describe the pharmacokinetics of oral amoxicillin/clavulanic acid in neonates 3. To quantify the cost-effectiveness of oral antimicrobial switch therapy in neonates This non-inferiority RCT includes 2 x 290 neonates (0-28 days, GA > 35 weeks; > 2 kg) with probable bacterial infection. These neonates initiate with empiric intravenous antibiotics for 48 hours, whereafter eligible neonates will be randomised to continue with intravenous antibiotics or to switch to oral antibiotics (amoxicillin/clavulanic acid), total treatment duration 7 days. For the first 30 neonates in the oral switch group, a detailed PK profile will be constructed. Participants will be followed closely by an online diary, phone calls and OPD visit. The primary outcome is bacterial re-infection within 28 days after finishing treatment. Secondary outcomes are hospitalization duration, costs, pharmacokinetic profile of oral amoxicillin, quality of life, side effects.
